Manual flagging failed to identify pseudohyperkalemia in acute myeloid leukemia: case report.

BACKGROUND: Pseudohyperkalemia is well known in acute or chronic lymphocytic leukemia, but it is very rare in acute myeloid leukemia (AML). The lab flagging system for leukocytosis to prevent pseudohyperkalemia may not work. CASE PRESENTATION: A 55 year-old white man with AML was sent to emergency department for transfusion due to severe anemia. Blood test showed severe leukocytosis and elevated potassium. Repeated blood test showed his potassium was even higher. Anti-hyperkalemic medical treatment was given. He was then diagnosed with pseudohyperkalema. INVESTIGATION: I was repeatedly reass

[Hydroxychloroquine-not always a harmless drug for off label use in dermatology].

Hydroxychloroquine is used for treatment of inflammatory diseases. It is considered to have few adverse effects. We report on a woman who developed a severe skin rash after intake of hydroxychloroquine, which she received for treatment of her lichen planopilaris. Based on the clinical, laboratory and histological findings the diagnosis of a drug reaction with eosinophilia and systemic symptoms (DRESS)-like drug reaction was established. Our case illustrates that hydroxychloroquine can lead to severe adverse effects in rare cases and that patients receiving this drug must be thoroughly informed

Identifying acute lymphoblastic leukemia mimicking juvenile idiopathic arthritis in children.

OBJECTIVE: Acute lymphoblastic leukemia (ALL) may present with arthritis implying the risk of being misdiagnosed as juvenile idiopathic arthritis (JIA). The aim of this study was to identify predictors for ALL based on clinical and laboratory information. METHODS: This cross-sectional, retrospective study compared clinical presentation and laboratory results of 26 children with ALL and arthritis versus 485 children with JIA (433 non-systemic, 52 systemic JIA). Using a Bayesian score approach the findings were evaluated by calculating odds ratios (OR) and lnOR as a measure of diagnostic weight

An Automated Cartridge-Based Microfluidic System for Real-Time Quantification of BCR::ABL1 Transcripts in Chronic Myeloid Leukemia: An Italian Experience.

Chronic myeloid leukemia (CML) is a clonal myeloproliferative disorder caused by the BCR::ABL1 fusion gene, resulting from a reciprocal translocation between chromosomes 22 and 9. Quantification of BCR::ABL1 transcript levels in peripheral blood by RT-qPCR represents the gold standard for molecular response (MR) monitoring, providing essential clinical information on treatment efficacy. Xpert® BCR-ABL Ultra is a fully automated in vitro diagnostic test that quantitatively detects e13a2 and e14a2 BCR::ABL1 transcripts using a single-use cartridge that integrates RNA extraction, cDNA synthesis,

Prognostic value of ECG T/R ratio and clinical predictors in septic shock.

BACKGROUND: Septic shock remains a leading cause of mortality in critically ill patients. Electrocardiography (ECG) is a rapid, non-invasive tool, and the T-wave to R-wave amplitude ratio (T/R ratio) has been proposed as a marker of electrolyte disturbances and myocardial stress. Its prognostic value in septic shock, however, is unclear. OBJECTIVE: To evaluate the association between the admission T/R ratio and short-term mortality in patients with septic shock and to identify independent predictors of mortality. METHODS: We conducted a single-center, retrospective observational study of 319

Parkinson's Disease Tremor Detection in the Wild Using Wearable Accelerometers.

Continuous in-home monitoring of Parkinson's Disease (PD) symptoms might allow improvements in assessment of disease progression and treatment effects. As a first step towards this goal, we evaluate the feasibility of a wrist-worn wearable accelerometer system to detect PD tremor in the wild (uncontrolled scenarios). We evaluate the performance of several feature sets and classification algorithms for robust PD tremor detection in laboratory and wild settings. We report results for both laboratory data with accurate labels and wild data with weak labels. The best performance was obtained using

On-Demand Patient-Specific Phenotype-to-Genotype Ebola Virus Characterization.

Biosafety, biosecurity, logistical, political, and technical considerations can delay or prevent the wide dissemination of source material containing viable virus from the geographic origin of an outbreak to laboratories involved in developing medical countermeasures (MCMs). However, once virus genome sequence information is available from clinical samples, reverse-genetics systems can be used to generate virus stocks de novo to initiate MCM development. In this study, we developed a reverse-genetics system for natural isolates of Ebola virus (EBOV) variants Makona, Tumba, and Ituri, which hav

The Emerging Drugs Network of Australia: A toxicosurveillance system of illicit and emerging drugs in the emergency department.

OBJECTIVE: The unprecedented rise in synthetic drugs, many containing unknown toxic agents, has made timely analytical diagnosis more difficult, and has reduced the confidence of clinicians providing ED management to this population of patients. This has also impacted the quality of evidence informing harm reduction responses. The Emerging Drugs Network of Australia (EDNA) brings together emergency physicians, toxicologists and forensic laboratories to establish a standardised ED toxicosurveillance system in Australia. METHODS: Blood analysis of intoxicated patients will be conducted by foren

Atypical optic neuritis: An overview.

Optic neuritis (ON) refers to conditions that involve inflammation of the optic nerve. Various autoantibodies have been found, which are associated with central nervous system inflammatory disorders and have provided much information about the immune targets and mechanisms that impact the prognosis, treatment, and recurrence of atypical ON. Therefore, neurologists and ophthalmologists together should work to find out clinical, laboratory, and imaging findings that may provide important clues to the etiology of atypical ON and its management. Various biomarkers have been identified to confirm a

Cerebrospinal fluid analysis and the determination of oligoclonal bands.

This document presents the guidelines for the cerebrospinal fluid (CSF) analysis and the determination of oligoclonal bands (OCBs) as pivotal tests in neuroinflammatory pathologies of the central nervous system. The guidelines have been developed following a consensus process built on questionnaire-based surveys, internet contacts, and discussions at workshops of the sponsoring Italian Association of Neuroimmunology (AINI) congresses. Essential clinical information on the pathologies in which the CSF analysis is indicated, and, particularly, on those characterized by the presence of OCBs in th

Rapid identification of Staphylococcus aureus from BacT/ALERT blood culture bottles by direct Gram stain characteristics.

The rapid identification of Staphylococcus aureus from positive blood cultures provides important clinical and therapeutic information. Using criteria based on direct Gram stain characteristics, an experienced microscopist was able to distinguish S aureus from other staphylococci isolated from BacT/ALERT blood culture bottles with an overall sensitivity of 89% and specificity of 98%. Furthermore, this method was readily taught to a clinical microbiologist who had not previously used the method first hand. Laboratories using the BacT/ALERT blood culture system should become familiar with these

Syndromes of disseminated intravascular coagulation in obstetrics, pregnancy, and gynecology. Objective criteria for diagnosis and management.

This article presents current understanding of the causes, pathophysiology, clinical, and laboratory diagnosis, and management of fulminant and low-grade DIC, as they apply to obstetric, pregnant, and gynecologic patients. General medical complications leading to DIC, which may often be seen in these patients, are also discussed. Considerable attention has been given to interrelationships within the hemostasis system. Only by clearly understanding these pathophysiologic interrelationships can the obstetrician/gynecologist appreciate the divergent and wide spectrum of often confusing clinical a

Full fingerprint hyperspectral imaging of prostate cancer tissue microarrays within clinical timeframes using quantum cascade laser microscopy.

One of the major limitations for clinical applications of infrared spectroscopic imaging modalities is the acquisition time required to obtain reasonable images of tissues with high spatial resolution and good signal-to-noise ratio (SNR). The time to acquire a reasonable signal to noise spectroscopic scan of a standard microscope slide region of tissue can take many hours. As a trade-off, systems can allow for discrete wavenumber acquisitions, sacrificing potentially vital chemical bands in order to reach specific acquisition targets. Recent instrumentation developments now allow for the full

Cost comparison of Transcatheter and Operative Pulmonary Valve Replacement (from the Pediatric Health Information Systems Database).

Outcomes for transcatheter pulmonary valve replacement (TC-PVR) and operative pulmonary valve replacement (S-PVR) are excellent. Thus, their respective cost is a relevant clinical outcome. We performed a retrospective cohort study of children and adults who underwent PVR at age ≥ 8 years from January 1, 2011, to December 31, 2013, at 35 centers contributing data to the Pediatric Health Information Systems database to address this question. A propensity score-adjusted multivariable analysis was performed to adjust for known confounders. Secondary analyses of department-level charges, risk of re
