Clonidine, an alpha adrenergic agonist which causes a diuresis in experimental animals, was studied in unanesthetized, conscious Brattleboro rats heterozygous or homozygous for hereditary hypothalamic diabetes insipidus to determine if the diuresis was due to alpha adrenergic inhibition of antidiuretic hormone (ADH) release or to another mechanism of action. Heterozygous rats given clonidine s.c. in doses of 50 to 300 mu/kg b.w. exhibited a prompt dose-related diuresis. Trazodone, a non-tricyclic drug with antidepressant properties, diminished the central hypotensive response to clonidine in a dose-dependent manner. Both drugs were infused into the left vertebral artery of chloralose-anaesthetized cats. Intravenously injected trazodone did not reduce the central hypotensive effect of clonidine. Clonidine usually serves as a tic-suppressing agent in patients with Tourette syndrome. A case study is presented in which clonidine at subclinical concentrations led to an exacerbation of the tics and caused severe systemic heat sensations. Involvement of functional abnormalities of the hypothalamus in Tourette syndrome is hypothesized.

A further episode culminated in respiratory arrest and hypoxic ischemic encephalopathy with permanent mental regression. We report on a 44-year-old female who developed spontaneous right coronary artery dissection associated with the use of a clonidine transdermal patch. The lesion was successfully treated with percutaneous coronary intervention with placement of three bare metal stents. The patient had an uneventful recovery. To our knowledge, this is the first reported case of spontaneous coronary artery dissection associated with clonidine effect.
