"There's more to recovery than just weight gain": Ethical challenges for justice and respect in adjudication of capacity for patients with anorexia nervosa.

Anorexia nervosa (AN) is a complex psychiatric illness with a high mortality rate. When patients with AN refuse medical treatment, it may be ethically justifiable to provide treatment in their best interests if they are found incapable to make the decision themselves. In Ontario, Canada, any person deemed incapable has the right to challenge that decision at the Consent and Capacity Board (CCB), an independent tribunal that adjudicates hearings under six Acts, including the Health Care Consent Act. Recognising both the complexity of capacity assessment in the context of AN and widespread stigm

Exploring the process of social support among parents of children with cancer in the sociocultural context of Iran: a grounded theory study protocol.

INTRODUCTION: The incidence of cancer is increasing across all age groups including children. This disease and its costly treatment affect not only the patient but their entire family-especially parents as primary caregivers-leading to psychological distress, physical exhaustion and financial strain. Social support serves as a vital protective factor for parents facing their child's cancer diagnosis. As a complex, context-dependent phenomenon emerging from social interactions, its process requires exploration within diverse cultural settings. Thus, this study aims to explain the process of soc

Effects of manual therapy on oral opening, swallow function and upper quarter mobility in Chilean survivors of head and neck cancer: a study protocol for a controlled, randomised study (MAnual ThErapy for Oral Opening (MATEO) study).

INTRODUCTION: Head and neck cancer (HNC) accounts for over 4% of global cancer incidence, yet the oncological treatment induces several sequelae such as oral dysfunction, cervical and shoulder impairments or pain that are not well addressed. Thus, survivors of HNC (sHNC) perceive a decrease in their quality of life (QoL). This study protocol aims to investigate the effects of manual therapy (MT) to determine the effectiveness and safety on oral opening, swallow function and upper quarter mobility, cervical muscle strength, pain, functionality and QoL of sHNC. METHODS AND ANALYSIS: A randomise

Global Health Immersions in Nursing Education: Evaluating Impact and Recommendations for Programming.

AIM: Global health immersion programming is cited as supporting nursing students' cultural competency; it is also historically grounded in colonialism. This study explored nursing students' perspectives on the benefits and challenges of global health immersions. Programming suggestions are based on student feedback and immersion literature. DESIGN: Qualitative, cross-sectional. METHODS: Nine semi-structured interviews were conducted with nursing students who participated in global health immersions. Interviews were inductively coded for recurring themes, focusing on immersion benefits, chall

Comparison of binocular game and patching in treating mild to moderate anisometropic amblyopia: a study protocol for a randomized controlled trial.

BACKGROUND: Amblyopia, as a neurodevelopmental preventable visual disorder, affects approximately 1.1 % in Asia. A binocular approach to treating amblyopia has been recently proposed. Whether the binocular playing game treatment is comparable to patching treatment needs further randomized clinical trials. To address this, the present research, designs, develops, and evaluates a new binocular game to treat amblyopia. METHODS: This study has been designed as a non-inferiority, randomized, two parallel-group, controlled trial. Forty-four patients between 4 and 12 years diagnosed with amblyopia w

The Case Against Legalizing Medically Assisted Suicide or Voluntary Active Euthanasia in Islamic Regions.

This essay presents arguments against legalizing medically assisted suicide (MAS) or voluntary active euthanasia (VAE) in Islamic regions. MAS and VAE should be determined by particular regions based on their respective mainstream moral cultures. Taking the Islamic regions of Bangladesh as examples, the essay argues that MAS or VAE should not be legalized in such regions because it would contradict the Islamic classical moral convictions, authentic interpretations, and relevant bioethical codes that prioritize saving human lives and providing quality palliative care rather than MAS or VAE for

Experiencing abortion rights in India through issues of autonomy and legality: A few controversies.

Abortion laws in India, like other laws, are premised on the 1861 British Penal Code. The Medical Termination of Pregnancy Act was passed in 1971 to circumvent the criminality clause around abortion. Yet the law continues to render invisible women's right to choose. Legal procedures have often hindered in permitting abortion, resulting in the death of a mother or the foetus. Despite the latest techno-medical advances, the laws have remained stagnant or rather restrictive, complicated further by selective female foetus abortions. Legal resistance to abortion-seeking after 20 weeks gestation adv

The Only Way Home is With a Hole in the Neck: A Palliative Tracheostomy at the End of Life.

IntroductionIn patients with tumor-related airway obstructions, there are unique challenges for the medical team to deliver quality palliative care and meet end-of-life goals.Case DescriptionAn elderly woman with oropharyngeal cancer was hoping to qualify for a clinical trial. However, after multiple admissions for airway obstruction, she elected to pursue a palliative tracheostomy so that she may die comfortably at home, in a state several hours away.DiscussionPatients with unique complications at the end-of-life benefit from interventions that seem "unnecessarily invasive" that allow them to

Syphilis and human experimentation from World War II to the present: a historical perspective and reflections on ethics.

Even after the Nuremberg code was published, research on syphilis often continued to fall far short of ethical standards. We review post-World War II research on this disease, focusing on the work carried out in Guatemala and Tuskegee. Over a thousand adults were deliberately inoculated with infectious material for syphilis, chancroid, and gonorrhea between 1946 and 1948 in Guatemala, and thousands of serologies were performed in individuals belonging to indigenous populations or sheltered in orphanages. The Tuskegee syphilis study, conducted by the US Public Health Service, took place between

Trial to assess the tolerability of using felodipine to upregulate autophagy as a treatment of Huntington's disease (FELL-HD): a phase II, single-centre, open-label, dose-finding trial protocol.

INTRODUCTION: Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder that presents with a progressive movement disorder along with cognitive and psychiatric problems. It is caused by a Cytosine-adenin-guanine (CAG) expansion in exon 1 of the huntingtin gene which codes for mutant huntingtin (mHTT) that over time accumulates in cells, causing dysfunction and then death through new toxic gain-of-function mechanisms. Autophagy has been shown to be critical for the degradation of diverse intracytoplasmic aggregate-prone proteins that cause neurodegenerative disease, includin

Intellectual and physical disability, household living arrangements and COVID-19 outcomes in Scotland: protocol for a retrospective cohort study.

INTRODUCTION: While evidence consistently demonstrates elevated COVID-19 risks among people with intellectual and physical disabilities, less is known about how household living arrangements shape these inequalities. Residential setting and household size influence exposure, capacity to isolate and reliance on care yet have rarely been examined jointly with disability status at a population scale. This study aims to estimate disability-related inequalities in COVID-19 infection, hospitalisation and mortality in Scotland and to assess whether residential setting and household size modify these

Factors influencing preference for short-term over long-term contraceptives among women in northern Uganda. A mixed-methods study.

INTRODUCTION: Modern contraceptives have contributed to reduction of unintended pregnancy among the women of reproductive age (15 to 49) globally. Use of long-term reversible contraceptives is low (13%) in Uganda yet they are cost-effective. This study aimed to identify factors influencing preference of short-term reversible contraceptives over long-term methods among women of reproductive age attending the family planning clinic at Lira Regional Referral Hospital (LRRH). METHOD: We performed a mixed-methods cross-sectional study, in which structured interviews were administered to 220 family

Female RNA concussion (FeRNAC) study: assessing hormone profiles and salivary RNA in females with concussion by emergency departments in New Zealand: a study protocol.

BACKGROUND: Females of reproductive age with concussion report a greater number of symptoms that can be more severe and continue for longer than age matched males. Underlying mechanisms for sex differences are not well understood. Short non-coding Ribonucleic Acids (sncRNAs) are candidate salivary biomarkers for concussion and have been studied primarily in male athletes. Female sex hormones influence expression of these biomarkers, and it remains unclear whether a similar pattern of sncRNA expression would be observed in females following concussion. This study aims to evaluate recovery time,

Parent perspectives on neonatal research without prior consent: a mixed-methods interview study.

BACKGROUND: Recruitment for clinical research in the neonatal intensive care unit (NICU) is difficult, which may lead to low enrollment rates, systematically skewed samples, and nongeneralizable results. Research without prior consent (RWPC), defined as obtaining consent only after infants have been randomized to a treatment arm, may improve enrollment but raises ethical challenges. There is a need to better understand how parents with lived experience of having an infant in the NICU consider RWPC’s impact on critical ethical values related to informed consent, including decision-making, respe

Corneal and conjunctival injury seen in urgent care centres in Israel.

PURPOSE: Corneal and conjunctival injuries (CCI) comprise a large portion of the cases presenting to hospital-based emergency departments (ED) with ocular involvement. Urgent Care Centres (UCC) offer community based emergency care at lower cost than hospital-based emergency departments (ED) and with greater temporal convenience than primary care office settings. While CCI prevalence and treatment at hospital-based EDs has been well studied, this is the first report, to our knowledge, on CCI demographics and aetiology presenting to UCCs. METHODS: This retrospective study was approved by the in

Exploring stress and coping among black women in early midlife with elevated blood pressure in a safety-net health system: a qualitative focus group protocol.

INTRODUCTION: Black women in early midlife experience disproportionate exposure to stress and elevated cardiovascular risk, including hypertension. Despite this, few stress management interventions are designed with and for this population. This study aims to explore the lived experiences of stress and coping among black women in early midlife with elevated blood pressure to inform the codesign of a culturally relevant, multilevel stress management intervention. METHODS AND ANALYSIS: We will conduct one-time, semistructured focus groups with black women aged 35-50 who have elevated blood pres

Effects of long non-coding RNA myocardial infarction-associated transcript on retinal neovascularization in a newborn mouse model of oxygen-induced retinopathy.

Whether long non-coding RNA myocardial infarction-associated transcript is involved in oxygen-induced retinopathy remains poorly understood. To validate this hypothesis, we established a newborn mouse model of oxygen-induced retinopathy by feeding in an oxygen concentration of 75 ± 2% from postnatal day 8 to postnatal day 12, followed by in normal air. On postnatal day 11, the mice were injected with the myocardial infarction-associated transcript siRNA plasmid via the vitreous cavity to knockdown long non-coding RNA myocardial infarction-associated transcript.
