Adaptation to family-based treatment for Medicaid-insured youth with anorexia nervosa in publicly-funded settings: Protocol for a mixed methods implementation scale-out pilot study.

BACKGROUND: Family-based treatment (FBT) for anorexia nervosa is an evidence-based treatment, but its effectiveness is untested among socioeconomically disadvantaged and racially diverse youth. Adapting FBT may facilitate "scale-out" for Medicaid-insured youth served in publicly-funded settings and potentially improve outcomes for more diverse populations. METHODS: This mixed methods effectiveness-implementation Hybrid Type 3 pilot study protocol included a planning period in collaboration with the San Francisco Department of Public Health, culminating in a two-day in-person FBT training for

Improving Access to Human Milk Through a Breast Pump Program for Hospitalized Infants at a Large, Urban Safety Net Hospital.

Human milk is the optimal source of nutrition for infants, especially those born preterm or requiring neonatal intensive care unit care, offering physiologic and psychological benefits for both infants and lactating parents. Despite its importance, access to high-quality electric breast pumps remains a barrier, particularly for underserved populations. To address this, the Lactation Group (LG) at Henry Ford Hospital, a large urban hospital in Detroit, established a Pump Program providing electric pumps to parents facing financial, insurance, or logistical challenges. In the United States, acce

The osteoporotic fracture prevention program in rural areas (OFRA): a protocol for a cluster-randomized health care fund driven intervention in a routine health care setting.

BACKGROUND: Fragility fractures are one of the leading causes for disability in old people. The main underlying mechanisms are osteoporosis and falls. Evidence-based measures to prevent either falls or fractures are available. However, coordinated preventive approaches combining bone health and fall prevention are rare. The objective of the study is to evaluate a health care fund driven program, which encourages insured persons to adhere to national guidelines regarding bone health and physical activity and falls prevention. The health care fund cooperates with the 'German Association of Rural

Real-World Cancer Genomic Profiling at a Cooperative Hospital for Cancer Genomic Medicine in Japan.

BACKGROUND: Comprehensive genomic profiling (CGP) tests have been covered by insurance in Japan since 2019, and their use in cancer genomic medicine (CGM) has expanded since then. Although extensive data are available from core hospitals, real-world data from community-based cooperative hospitals are limited. METHODS: Using data from 514 consecutive patients with advanced cancer who underwent CGP testing at our institution between June 2019 and March 2025, we investigated the proportion of cases receiving therapeutic recommendations, the rate of drug administration based on CGP test results,

Physicians' perspectives on continuity of care for patients involved in the criminal justice system: A qualitative study.

BACKGROUND: In 2016, over 11 million individuals were admitted to prisons and jails in the United States. Because the majority of these individuals will return to the community, addressing their health needs requires coordination between community and correctional health care providers. However, few systems exist to facilitate this process and little is known about how physicians perceive and manage these transitions. OBJECTIVE: The goal of this study was to characterize physicians' views on transitions both into and out of incarceration and describe how knowledge of a patient's criminal just

The integration of the global HIV/AIDS response into universal health coverage: desirable, perhaps possible, but far from easy.

BACKGROUND: The international community's health focus is shifting from achieving disease-specific targets towards aiming for universal health coverage. Integrating the global HIV/AIDS response into universal health coverage may be inevitable to secure its achievements in the long run, and for expanding these achievements beyond addressing a single disease. However, this integration comes at a time when international financial support for the global HIV/AIDS response is declining, while political support for universal health coverage is not translated into financial support. To assess the risk

Changes in publicly insured general practitioner consultations before and after workplace injury: interrupted time series by consultation duration and care plans in Australia.

BACKGROUND: General practitioners (GPs) are central to the treatment and management of injured workers. Australian workers' compensation funds GP consultations for work-related injuries. Almost all other GP consultations are subsidised by public healthcare insurance (Medicare Benefit Scheme). This study aimed to examine changes in publicly-insured GP consultations before and after injury for injured workers with long-duration claims, focusing on consultation duration types and GP-provided mental health and chronic diseases management plans. METHODS: This retrospective cohort study using inter

Risk factors for direct heat-related hospitalization during the 2009 Adelaide heatwave: a case crossover study.

Adelaide experienced an extreme and prolonged 13 days heatwave in summer 2009. The health impacts of this heatwave included an almost 14-fold increase in direct heat-related hospital admissions. This study aims to investigate the risk factors for this extra health burden. A case crossover study was conducted in metropolitan Adelaide to compare the characteristics of patients from the heatwave (exposure) period and non-heatwave (control) periods before and after. Direct heat-related hospitalizations were identified based on the ICD-10 codes (X30, T67, and E86). Patients' data, including age, ge

Well-Being of Ambulatory Adults With Cerebral Palsy: Education, Employment, and Physical Function of a Cohort Who Received Specialized Pediatric Care.

Introduction: The transition from pediatric health care and school systems presents enormous challenges for young adults with cerebral palsy (CP). The lack of strong societal support during this seminal life event is well-documented and leads many adults with CP to struggle with independence, higher education, and employment. Despite the relatively high prevalence of CP, information about the experiences and function of adults with CP in our society continues to be limited. The purpose of this project was to describe well-being by assessing education, employment, physical function, walking act

Navigating sexual and reproductive health among Venezuelan women sex workers in Peru: Qualitative explorations of structural vulnerability and resistance.

Migration to Peru from Venezuela has reshaped engagement in informal labour for many women who turn to sex work as a survival strategy amid legal precarity and systemic exclusion. Yet little is known about how migrant sex workers navigate sexual and reproductive health (SRH) in this context. In 2023, we conducted 34 semi-structured interviews with Venezuelan migrant cisgender women sex workers living in Lima, Peru. Interviews explored migration trajectories, working conditions, SRH needs, and experiences seeking care. Analysis was guided by the Structural Vulnerability Framework, contextualizi

Practice-based research examining effectiveness of exposure-based CBT for youth in a community mental health setting.

We examined the naturalistic effectiveness of exposure-based cognitive behavioral therapy (Ex-CBT) for pediatric anxiety and obsessive-compulsive disorder in a community mental health setting. We also characterized adaptations made to Ex-CBT and whether treatment factors varied by whether youth were Medicaid recipients or not. To do so, we conducted a three-year, retrospective chart review of consecutively treated youth in an Ex-CBT treatment center embedded in a community mental health setting (N = 94; 72.3 % Medicaid recipients, 68.1 % female). We abstracted baseline demographic and clinical

A Call for Youth Voice to Support Engagement in Care for 18- to 29-Year Olds Living with HIV in the US South.

Youth with HIV (YWH) face challenges in achieving viral suppression, particularly in the Southern United States, and welcome novel interventions responsive to community needs. The Theory of Planned Behavior (TPB) describes factors that influence behavior change, and the Positive Youth Development (PYD) supports youth-focused program design. We applied TPB and PYD to explore factors supporting care engagement and challenges for YWH in South Texas. We conducted 14 semi-structured interviews with YWH and 7 focus groups with 26 stakeholders informed by TPB, PYD, and themes from a youth advisory bo

Multilevel factors influencing HIV care and management in the Deep South: insights from HIV care professionals.

The United States Deep South bears a disproportionate HIV burden, with low rates of retention in care and viral suppression among people living with HIV (PWH). Factors influencing HIV care engagement can be understood through the modified social ecological model (MSEM), which comprises five interconnected layers: individual, network, community, policy, and HIV epidemic stage. We interviewed thirty HIV care professionals from Alabama, Mississippi, and Louisiana to identify optimal, culturally relevant approaches for engaging PWH in care. Using MSEM for qualitative analysis, we identified salien

A narrative review on cost considerations in early intervention for deaf and/or hard-of-hearing children in Africa.

Early intervention (EI) is essential for the language, social, and educational development of deaf and/or hard-of-hearing (DHH) children. In African countries, however, the implementation of EI remains significantly constrained by cost considerations and systemic service gaps. This narrative review synthesizes findings from 26 peer-reviewed publications to explore how cost influences access to and sustainability of EI services in Africa. Seven interrelated themes were identified: (i) high out-of-pocket expenses that limit family access to services; (ii) inadequate public funding and heavy reli
