Rare Ectopic Pregnancies, Cases, Confusions, and Clues.

Ectopic pregnancies are implanted outside the normal location within the uterus; mostly (about 95%) within the fallopian tube but might also rarely be seen within abnormal locations in the uterus, in the ovaries, peritoneal cavity, or even retroperitoneally. These pregnancies are particularly challenging to diagnose. The outcome in most ectopic pregnancies is guarded unless an early diagnosis is made and dealt with by an experienced multidisciplinary team. The management of rare ectopic pregnancies is further complicated by difficulties in diagnosis, delay in diagnosis, and greater risk of ble

Clarify Confused Nodes via Separated Learning.

Graph neural networks (GNNs) have achieved remarkable advances in graph-oriented tasks. However, real-world graphs invariably contain a cert … ation metrics and low efficiency. In this paper, we first propose a new metric, termed Neighborhood Confusion (NC), to facilitate a more reliable separation of nodes. We observe that node groups with different levels of NC values exhibit certain differences in intra-group accuracy and visualized embeddings. These pave the way for Neighborhood Confusion-guid …

Alternative to the statistical mass confusion of testing for "no effect".

It should not be controversial to argue that the proximate goal of measuring something is to figure out how big or small or fast or slow it is. Estimates of effect size can be used to build models of how cells work and to test quantitative predictions. Unfortunately, in cell biology, quantification is nearly synonymous with null-hypothesis significance testing. The hypothesis being tested is universally assumed to be the hypothesis that there was no effect. Framing every experiment as an attempt to reject the no-effect hypothesis is convenient but doesn't teach us about cells. In this manuscri

The effect of face masks on confusion of emotional expressions.

While the use of personal protective equipment protects healthcare workers against transmissible disease, it also obscures the lower facial … recognition accuracy was lower for images with masks than images without masks in both experiments. Confusion rates were not uniform: masked disgust and sadness had the greatest increase of confusion from their unmasked conditions, happiness and surprise had the least confusion in all conditions, and both fear and anger had generally low recognition in mask …

On a confusion about there being two types of consciousness.

Progress in the scientific study of consciousness has been impeded by several fundamental controversies. One pertains to a major divide between theories: sensory versus cognitive. Here, we argue that the key to resolving this controversy is to reevaluate the conceptual distinction proposed by Block in 1995 between phenomenal consciousness (P) and access consciousness (A). We propose that P and A should not be understood as two different types of consciousness, but as two necessary conditions for consciousness. We illustrate how this conceptual shift allows making substantial progress in answer

Confusing cause and effect in public health policy.

When studying the literature on current public health policy challenges, a number of rankings can be found, created from data collected and processed by different methods and from different sources, ordering the challenges by their significance. Inevitably, in global studies, there are errors caused by averaging data across populations that differ significantly in population size and the causes of the decline in disability-adjusted life years (DALYs), potentially in combination with years of life lost (YLL) and years of life lived with disability (YLD), that may serve as indicators of the heal

Wernicke's Encephalopathy Presenting With Confusion in a Patient With Schizophrenia.

Wernicke's encephalopathy (WE) is a neuropsychiatric condition caused by thiamine deficiency often associated with alcoholism. Other less common causes include prolonged gastroenterology problems or dietary insufficiencies associated with hyperemesis gravidarum, bariatric surgery, and eating disorders. Prolonged WE without proper treatment can lead to the chronic and irreversible condition, Wernicke-Korsakoff syndrome. Despite being known for its classic triad of clinical symptoms (nystagmus/ophthalmoplegia, gait ataxia, and confusion), WE patients more commonly present with non-specific sympt…

Osteoid osteoma: A pathology that is confused with ankylosing spondylitis.

Osteoid osteoma is a small and benign osteoblastic tumor seen typically in males aged below 25 years. Although it is rarely seen in the vertebrae, it should be considered in the differential diagnosis of spondyloarthropathies, especially for those which occur in young people, along with back and lumbar pain, which increases during the night. Early diagnosis is essential to alle-viate symptoms and prevent the risk of structural spinal deformities, such as scoliosis. Here, we describe the case of a 28-year-old man with vertebral osteoid osteoma that was misdiagnosed as ankylosing spondylitis and

A case of Löfgren's syndrome confused with decompression sickness.

A broad differential diagnosis is important to provide appropriate care. This may be challenging for conditions like decompression sickness (DCS) which can be easily confused with other conditions. In suspected DCS, treatment may be an important part of the diagnosis. An improvement in symptoms after hyperbaric oxygen treatment (HBOT) is consistent with a DCS event. However, HBOT may also impact symptoms in other conditions, including Löfgren's syndrome (LS). LS, a poorly understood, clinically distinct phenotype of sarcoidosis, is a complex, multi-system granulomatous inflammatory condition.

The confused oncologic patient: a rational clinical approach.

Purpose of reviewThe purpose of this review is to provide a practical clinical approach to confusion in the patient with cancer. Confusion in the cancer population has a broader differential diagnosis than in the general medical population. The clinician must consider the usual differential diagnoses as well as causes unique to the cancer patient including direct complications from the cancer and indirect complications related to cancer treatment.Recent findingsIn the recent age of precision medicine, the oncologist now utilizes the genomic profile of both the patient and the tumor to provide …

Why is Alzheimer disease confused with other dementias?

Alzheimer disease (AD) is the most common cause of dementia. The cardinal manifestation of AD is progressive loss of memory. However, there are some nonamnestic presentations of AD, also called atypical AD. Symptoms of AD can sometimes start suddenly. In the presence of atypical symptoms or sudden onset, it may be difficult to distinguish AD from other dementias. We would like to discuss the confusing features of atypical AD that mimic other dementias. In this review, the literature associated with confusing features of AD, suggesting other dementia syndromes, is reviewed. In addition, a case

A case of pleural paragonimiasis confused with tuberculous pleurisy.

Here, we report a case of pleural paragonimiasis that was confused with tuberculous pleurisy. A 38-year-old man complained of a mild febrile sensation and pleuritic chest pain. Radiologic findings showed right pleural effusion with pleural thickening and subpleural consolidation. Adenosine deaminase (ADA) activity in the pleural effusion was elevated (85.3 IU/L), whereas other examinations for tuberculosis were negative. At this time, the patient started empirical anti-tuberculous treatment. Despite 2 months of treatment, the pleural effusion persisted, and video-assisted thoracoscopic surgery

Medial perirhinal cortex disambiguates confusable objects.

Our brain disambiguates the objects in our cluttered visual world seemingly effortlessly, enabling us to understand their significance and to act appropriately. The role of anteromedial temporal structures in this process, particularly the perirhinal cortex, is highly controversial. In some accounts, the perirhinal cortex is necessary for differentiating between perceptually and semantically confusable objects. Other models claim that the perirhinal cortex neither disambiguates perceptually confusable objects nor plays a unique role in semantic processing. One major hurdle to resolving this ce
