The authors report the case of a 54-year-old male who was found to have a large intracranial chondrosarcoma at the site from which chondromas had been partially removed twice, 6 and 10 years previously. On the third admission, the second through tenth and the twelfth cranial nerves on the right side were involved. Computed tomographic scans showed a large mass in the right middle and posterior fossae and the right ethmoid sinus. Angiography demonstrated an extradural mass in the right middle fossa. The tumor in the middle and posterior fossae was subtotally removed, and second, third, and eigh

Between June 1990 and May 1993, 17 acoustic tumors were removed by the middle cranial fossa approach with attempted hearing preservation. Measurable hearing was restored in 13 patients (76.5%) and useful hearing (PTA < 50 dB, SDS > 50%) in 10 patients (58.8%). In addition, significant hearing improvement, greater than 10 dB in pure-tone average, was noted in 6 patients (35.3%). In the hearing improvement group, useful hearing was restored after tumor resection in two patients with poor preoperative hearing. These six patients with hearing improvement exhibited several findings in common: small

BACKGROUND: Arachnoid cysts most frequently occur in the middle cranial fossa and when they are symptomatic, patients present with central nervous symptoms. Nevertheless, a large proportion of arachnoid cysts are incidentally diagnosed during neuroimaging in cases with nonspecific symptoms.
REPORT OF CASES: The cases of two males with middle cranial fossa arachnoid cysts with nonspecific inner ear symptoms were retrospectively reviewed. The first patient presented with mild headache, nausea, vertigo, unsteadiness, and tinnitus on the left ear while the second patient's main complaint was left

The skull base is uniquely placed to absorb anteriorly directed forces imparted either via the midfacial skeleton or cranial vault. A variety of skull base fracture classifications exist. Less well understood, however, is fracture extension beyond the anterior cranial fossa (ACF) into the middle and posterior cranial fossae. The cases of 81 patients from two UK major trauma centres were studied to examine the distribution of fractures across the skull base and any relationship between the vector of force and extent of skull base injury. It was found that predominantly lateral force to the cran

OBJECTIVE: The objective of this study was to demonstrate that trigeminal schwannomas (TSchs) located in different cranial fossae can be resected entirely through Meckel's cave which is expanded by the tumor by taking either an endoscope-assisted pterional epidural approach (EA-PEA) or an endoscope-assisted lateral suboccipital retrosigmoid approach (EA-LSRA). Additionally, we describe a modified classification based on Jefferson's system to determine the surgical approach.
METHODS: This is a retrospective study of 19 patients with TSchs in different cranial fossae who underwent EA-PEA or EA-

BACKGROUND: Combined endonasal and transorbital multiportal surgery has been recently described for selected skull base pathologies. Nevertheless, a detailed anatomic description and a quantitative comprehensive anatomic study of the skull base areas where these 2 endoscopic routes converge, a so-called connection areas, are missing in the scientific literature.
OBJECTIVE: To identify all the skull base areas and anatomic structures where endonasal and transorbital endoscopic avenues could be connected and combined.
METHODS: Five cadaveric specimens (10 sides) were used for dissection. Quali

Objective: To investigate the anatomical and surgical approaches to middle cranial fossa through orbital lateral wall under endoscope. Methods: Cadaveric formalin fixed specimens and fresh colored silicone injected specimens were used for this study. All anatomic technical measurements were performed under 0° and 30° endoscope and infrared rays navigation. The surgical approach was designed with the bony opening on the lateral wall of orbit through which the lateral side of the middle cranial fossa could be directly entered under endoscope. One case of recurrent meningioma was performed throug

BACKGROUND: Petroclival lesions pose a significant neurosurgical challenge due to involvement or close proximity to important neurovascular structures. Chondrosarcomas are rare lesions that can affect these areas.
CASE DESCRIPTION: A 24-year-old male with 3 months history of poor coordination, imbalance, left-sided face hypoesthesia, facial palsy House-Brackmann Grade 2, and 6th cranial nerve palsy with diplopia. Hearing was preserved. Preoperative images showed a 5.5 cm multilobulated enhancing extra-axial mass centered in the left petroclival region with extension into middle and posterior

A 47-year-old woman was referred to the neurological surgery department after a self-limiting episode of dizziness and headache. Magnetic resonance imaging showed an extra-axial mass in the right petrous apex, suggesting a meningioma. We chose the anterior petrosal approach (APA) because meningioma is a benign tumor with brainstem compression, and our goal was total removal for potential cure of the disease. The APA is appropriate for lesions located in the superior petroclival region with a narrow dural tail, above cranial nerves VII and VIII. The posterior petrosal approach is for large petr

In case 1, the tumor was incidentally found in the right petrous bone, middle cranial fossa and cerebello-pontine angle. T1 weighted MRI demonstrated a low intense mass and T2 weighted imaging demonstrated hyperintensity. Through the subtemporal extradural approach, an epidermoid in the middle cranial fossa was partially removed. Postoperative course was uneventfull but an episode of rhinorrhea occurred 15 months later. Bone-window CT scan disclosed air cells of the petrous bone were exposed to the previous surgical cavity. Using the same approach, an epidermoid was totally removed. With the s

OBJECTIVE/HYPOTHESIS: Generally, patients with small acoustic neuroma have less facial palsy after its removal. The middle cranial fossa approach is mainly applied to the small acoustic neuroma and tumor size does not influence the prognosis of facial palsy. The internal auditory canal cannot be fully opened in the middle cranial fossa approach, and the facial nerve is tightly attached in the fundus. According to these anatomical factors, we hypothesized that acoustic neuromas located away from the fundus might be removed with less facial nerve damage. We investigated the distance between the

Bone-borne trans -sutural distraction osteogenesis (TSDO) is widely used to treat midfacial hypoplasia in children with cleft lip and palate; however, its effects on the cranial base are still poorly understood. The authors aimed to study morphological changes in the cranial base after TSDO. Pre and postoperative computed tomography (CT) images of cleft lip and palate children with midfacial skeleton dysplasia who underwent TSDO were collected retrospectively, and their corresponding 3-dimensional models were measured. Results showed no significant change in the length of the anterior or poste

OBJECTIVE: Trigeminal schwannomas are rare neoplasms with an incidence of less than 1% that require a comprehensive surgical strategy. These tumors can occur anywhere along the path of the trigeminal nerve, capable of extending intradurally into the middle and posterior fossae, and extracranially into the orbital, pterygopalatine, and infratemporal fossa. Recent advancements in endoscopic surgery have suggested a more minimally invasive and direct route for tumors in and around Meckel's cave, including the endoscopic endonasal approach (EEA) and endoscopic transorbital superior eyelid approach

The middle cranial fossa approach to lesions of the geniculate ganglion and internal auditory canal preserves cochlear function and affords access to the lateral internal auditory canal. The labyrinthine portion of the facial nerve tends to course near the basal turn of the cochlea, just beneath the middle cranial fossa floor, and is usually dissected in this approach. To determine the distance from the labyrinthine portion of the facial nerve to the basal turn of the cochlea, measurements were obtained in the temporal bones of 24 subjects (48 ears) 9 to 76 years of age. These subjects had no

The venous sinus of Kelch (VSK) represents a rare but clinically significant venous variant of the middle cranial fossa, with very limited documentation in the existing literature. This study aimed to understand the morphology, course, and morphometry of VSK along with associated variations in the drainage pattern of the superficial middle cerebral vein (SMCV) and middle meningeal sinus (MMS). This study was conducted over the period from 2022 to 2025 at Zoram Medical College and Hospital, Aizawl, Mizoram. In the present study, VSK was observed in 9.26% of the 54 middle cranial fossae examined

BACKGROUND: Trigeminal schwannomas with middle and posterior fossa extension are difficult to treat, particularly after prior surgery or stereotactic radiosurgery (SRS). A minimally invasive interdural approach such as the endoscopic lateral transorbital approach (ELTOA) may offer safer access in these settings.
CASE DESCRIPTION: We present the case of a 33-year-old woman with a history of partial resection of a left trigeminal schwannoma through a pterional approach and adjuvant SRS, who developed progressive left V1 paresthesia and trigeminal neuralgia. Neurological examination revealed hyp

A 14-year-old boy and a 11-year-old boy presented with subdural hematomas as complications of preexisting arachnoid cysts in the middle cranial fossa, manifesting as symptoms of raised intracranial pressure. Both had a history of heading the ball in a soccer game about 7 weeks and 2 days before the symptom occurred. There was no other head trauma, so these cases could be described as "heading injury." Arachnoid cysts in the middle cranial fossa are often associated with subdural hematomas. We emphasize that mild trauma such as heading of the ball in a soccer game may cause subdural hematomas i

BACKGROUND: The Bonebridge is an active transcutaneous bone conduction implant recommended as a surgical option for adults and children (aged 5-18 years). Successful implantation of the Bonebridge is often restricted by an insufficient amount of temporal bone to house the transducer in the paediatric patient.
METHOD AND RESULTS: In this unique paediatric case, bilateral Bonebridge devices were implanted simultaneously in the right sinodural angle and the left middle cranial fossa.
CONCLUSION: The simultaneous implantation of bilateral Bonebridge devices was well tolerated in this paediatric
