COVID-19-The impact of variable and "low normal" pulse oximetry scores on Oximetry@Home services and clinical pathways: Confounding variables?

COVID-19 Oximetry@Home services have been commissioned nationally. This allows higher-risk patients with mild COVID-19 symptoms to remain at home, being supplied with a Pulse Oximeter to measure their oxygen saturation (SpO2 ) two to three times daily for two weeks. Patients record their readings manually or electronically which are monitored by a clinical team. Clinical decisions, using an algorithm, are based on SpO2 readings in a narrow range with 1-2 point changes potentially affecting care. In this article, we discussed the problem that multiple factors affect SpO2 readings, and that some

[Practical management of nosebleed].

This article presents the guidelines for the treatment of epistaxis in an out-patient setting in Sweden. Nasal hemorrhage from the anterior part of the nasal cavity is best treated with silver nitrate cauterization or electrocautery. Bleeding from the posterior part of the nasal cavity often requires treatment with a pneumatic nasal tamponade, RapidRhino, or a posterior nasal packing with a Foley catheter. When a patient requires treatment with posterior nasal packing it is recommended that a contact be taken with the nearest ENT clinic. The guidelines described are used by otorhinolaryngologi

A nurse-led pathway to treat self-harm injuries.

Self-harm occurs frequently in psychiatric hospitals and other mental health and outpatient settings. Many patients at the Ayr Clinic are at risk of self-harm and often access the local accident and emergency service for minor injury care. The medical and nursing response to people who repeatedly self-harm, given increasing pressures and dwindling A&E resources, can often be one of impatience, frustration and hostility. A nurse-led pathway was developed by the Ayr Clinic and NHS Ayrshire and Arran to fast-track the assessment and treatment of such injuries and improve health professionals' att

[Present and future of tuberculosis care in regions].

As the incidence of active tuberculosis in Japan declines and the healthcare environment changes, restructuring of the medical care system for tuberculosis is required. According to a questionnaire survey in Hiroshima Prefecture, experiences in tuberculosis (TB) care and knowledge, such as standard treatment and DOT, is insufficient in the local medical institutions designated for tuberculosis care. Regional coordination between the tuberculosis hospital and the regional private practitioners will be one of the important issues in proper TB care. In order to strengthen coordination, Higashi-hi

Optimising the management of pulmonary arterial hypertension patients: emergency treatments.

Pulmonary arterial hypertension (PAH) is a rare and potentially fatal disease whose management is usually restricted to a few specialised centres. As patients do not necessarily live in the neighbourhood of these centres, daily care and emergencies have to be delegated to first and second lines. Treatment guidelines do not usually provide recommendations for acute emergency situations as evidence is scarce. This short review provides a description of our therapeutic protocols based on available data. A model of transmural organisation of care for PAH patients, currently applied in Belgium, is

[WHO-classification of malignant lymphomas].

Because of the multiple developmental stages of lymphocytes and the complexity of the immune system, the corresponding neoplasms, the malignant lymphomas, represent a complex group of diseases. The clinical grouping of the KIEL-classification, distinguishing high grade from low grade lymphomas, has been abandoned in the WHO-classification as it is partly misleading. Especially, mantle cell lymphoma which, according to its morphology, represents a low grade lymphoma, is characterized by an extremely aggressive clinical course. The therapy of malignant non-Hodgkin lymphomas has become more and mor

Dysphagia lusoria: a case study.

Dysphagia lusoria is described in the literature as difficulty swallowing because of a "jest of nature." The "jest of nature" is a birth defect encompassing any aortic root vascular anomaly that causes esophageal dysphagia. Persons with dysphagia lusoria can be categorized according to their specific subclavian anomaly (ie, depending on the presence of an aneurysm, occlusive disease, or esophageal compression). All patients with this anomaly have an aberrant subclavian artery in a transposed position that courses posterior to the esophagus. The operative approach to repair this condition has b

Pathway for the Diagnosis and Management of Pericardial Disease: Contemporary Perspective.

Pericardial disease represents a diverse clinical spectrum, including acute pericarditis, pericardial effusion, cardiac tamponade, effusive-constrictive pericarditis (CP), and CP, which vary significantly in presentation, urgency, and management. Recent consensus guidelines from the European Society of Cardiology and the American College of Cardiology have enhanced clinical phenotyping, multimodal imaging techniques, and targeted immunomodulatory therapies; however, their implementation in clinical practice remains variable. We introduce our updated pathway-based framework that structures diag

Implementing a Structured Head and Neck Cancer Care Pathway in an Electronic Health Record: Iterative Process and Effects on Data Quality.

The digital availability of health data not only improves processes in primary care, but it also facilitates the evaluation of healthcare delivery. Nevertheless, the preprocessing of data for secondary use is still time-consuming and expensive, particularly in head and neck cancer (HNC), where patients undergo complex multidisciplinary treatment trajectories. Therefore, we have looked further into the effects on data quantity and quality following the implementation of structured care pathways. Leveraging data extracted from these care pathways, we assessed the potential of real-time quality-o

Enhancing patient safety and risk management through clinical pathways in oncology.

The increasing emphasis on risk management and quality assessment in hospital management necessitates strict adherence to government and international standards. Those standards are particularly important, and certification requirements further underscore this need. The objective of this paper is to outline the application of the INTegrated Audit Model in a Comprehensive Cancer Center. This model represents a revised version of Tracer Methodology, for audit surveys, aimed at aligning audit activities with UNI EN ISO 9001:2015 and Joint Commission Standards for quality, patient safety and clini

Informing Implementation of a National Integrated Clinical Pathway for Low Back Pain in Ireland: A Pre-Implementation Qualitative Study With General Practitioners.

INTRODUCTION: Ireland's Health Service Executive is developing a new national integrated low back pain (LBP) pathway spanning primary and secondary care to improve LBP healthcare. Clinical pathways are frequently employed to optimise clinical outcomes and resource use but are challenging to implement. Context-specific implementation planning, leveraging implementation science and its conceptual frameworks, should inform successful implementation. AIM: To explore General Practitioner (GP) perceptions of the new LBP pathway, using their on-the-ground knowledge to assess barriers and facilitator

[Diagnostic-therapeutic care pathways for patients with cardiac amyloidosis - SIC/ANMCO Consensus document. Edited by the Italian Cardiac Amyloidosis Network (RIAC)].

The perspective on cardiac amyloidosis has deeply changed over the last 10 years following major advances in diagnosis and treatment options. This heterogeneous disease requires the interaction among experts of different specialties and subspecialties. Suspicion of disease, timely recognition and confirmation of final diagnosis, prognostic stratification, overall management and therapeutic strategies represent ongoing challenges in the clinical setting. Missing or delayed diagnosis may have significant impact on patient outcome, especially in light-chain amyloidosis. The present inter-society

Determinants of Care Pathways for C-PTSD Patients in French Psychotrauma Centers: A Qualitative Study.

In 2018, the International Classification of Diseases (ICD-11) established a novel nosographic category within the stress-specific disorders known as complex post-traumatic stress disorder (C-PTSD). Characterized by distinctive clinical attributes and a limited response to conventional PTSD treatments, C-PTSD has prompted the reconsideration of care methods. Our study's purpose was to explore the intricate factors shaping the care pathways for individuals suffering from C-PTSD. We used a grounded theorization technique involving professionals across a range of specialized French psychotraumato

A novel Canadian multidisciplinary acute care pathway for people hospitalised with a diabetic foot ulcer.

This manuscript describes the implementation and initial evaluation of a novel Canadian acute care pathway for people with a diabetic foot ulcer (DFU). A multidisciplinary team developed and implemented an acute care pathway for patients with a DFU who presented to the emergency department (ED) and required hospitalisation at a tertiary care hospital in Canada. Processes of care, length of stay (LOS), and hospitalisation costs were considered through retrospective cohort study of all DFU hospitalizations from pathway launch in December 2018 to December 2020. There were 82 DFU-related hospital
