OBJECTIVE: To present an updated review of the most common upper respiratory infections (URI) in children seen by the pediatrician in outpatient clinics, for better diagnostic and therapeutic decisions.
SOURCES: References from Medline database were reviewed. The most relevant articles were selected.
SUMMARY OF THE FINDINGS: Acute rhinopharyngitis, sinusitis, streptococcal tonsillitis and viral croup are presented in a concise and critical view. Differential and etiological diagnosis limitations and the abusive use of antimicrobials in these illnesses are also discussed.
CONCLUSIONS: URI ar…

A 45 year old man was admitted to our hospital due to dyspnoea. At the age of 1 yr, he had undergone orotracheal intubation for croup diphtheria resulting in dysphonia. He was in good health up to the age of 39 yrs when exertional dyspnoea arose. He had had irregular therapy by aerosol without improvement. At admittance, the flow-volume curve indicated an impairment at high flows (peak expiratory flow (PEF) and maximum expiratory flow at 75% (MEF75%)). Fibreoptic bronchoscopy revealed a thin and smooth diaphragm, with a small (7-8 mm) central orifice in the subglottid area. CO2 laser therapy i…

Presented here is a case of unilateral pulmonary edema following acute subglottic edema after removal of an endotracheal tube. A 3-year-old boy, diagnosed as having nondiphtheric croup and pectus excavatum deformity, was scheduled for repair of a cleft lip. No complication occurred during the operation. After removal of the endotracheal tube, he showed dyspnea and cyanosis and was later found to have acute subglottic edema. After reintubation of the trachea, frothy pink fluid was discharged from the tube, and chest roentgenogram showed a right-sided alveolar infiltrate. Many factors may cause …

A retrospective analysis was completed of the charts of the 1,727 patients with the diagnosis of croup seen during two years (1985 and 1986) in the Emergency Department of The Hospital for Sick Children in Toronto. Cough (91.4%) was the most common presenting symptom. Other presenting signs and symptoms included stridor (57.5%), chest wall retraction (38%), coryza and wheezing (18.5%) and tracheal tug (15%). The majority of patients were treated on an ambulatory basis while 377 (21.7%) were admitted for further management. Inpatient treatment included racemic epinephrine (120 patients) and par…

We retrospectively reviewed the manifestations of influenza A2 in 83 hospitalized young children. Our purpose was to define the spectrum of clinical illness in this age group. Findings included fever (91%), vomiting or diarrhea (49%), pharyngitis (34%), pneumonitis (29%), otitis media (24%), conjunctivitis (13%), croup (13%), and bronchiolitis (6%). Neuromuscular manifestations occurred in 16 patients (19%) and included seizures, apnea, opisthotonos, and myositis. Three children had cerebrospinal fluid pleocytosis. Children younger than 3 months of age had fever less often and gastrointestinal…

Always ask about hoarseness and quality of voice in a history of any child presenting with cough or asthma-like symptoms. Children presentin … o emphasise whether or not there is chronic hoarseness in order to differentiate the diagnosis from croup. Juvenile Laryngeal Papillomatosis may present with cough, pneumonia, dysphagia, or stridor, as well as hoarseness. These patients are often misdiagnosed as having asthma or allergies.

In a retrospective study of hospitalised patients (children aged between 0 and 4 years) covering four years the dependence of the incidence of croup (infectious laryngitis) and obstructive bronchitis on air pollution was investigated. For this reason the incidence of morbidity in residential areas with various levels of SO2 and sedimenting dust were compared. The results show a definitive connection between a high level of SO2 and dust, respectively and a high rate of morbidity. This connection was particularly marked in areas with simultaneous high levels of both SO2 and dust. The effects of …

Epidemics of measles had been seen biennially over the past two decades at Princess Mary Hospital, Auckland, until 1980. After a four year gap, there has been a further epidemic. The medical records of 201 measles cases seen between July 1984 and August 1985 were reviewed. Maori and Pacific Island children were over-represented (86%), as were children aged less than 2 years (53%). Two children died. The most common complications were otitis media (98 cases), pneumonia (70 cases) and croup (28 cases, for which five children were ventilated). The epidemic was chiefly due to low immunisation rate…

In the incidence patterns of cholera, diphtheria and croup during the past when they were of epidemic proportions, we document a set of cycles (periods), one of which was reported and discussed by A. L. Chizhevsky in the same data with emphasis on the mirroring in human disease of the ~11-year sunspot cycle. The data in this study are based on Chizhevsky’s book The Terrestrial Echo of Solar Storms and on records from the World Health Organization. For meta-analysis, we used the extended linear and nonlinear cosinor. We found a geographically selective assortment of various cycles characterizin…

This report focuses on the clinical course and treatment of an infant male who had a progressively enlarging tongue mass initially thought to be an infantile hemangioma but was later found to be an infantile fibrosarcoma. Treatment included surgical excision with anticipated difficult mask ventilation with active rhino/enterovirus infection bronchiolitis and recent croup. Complete surgical excision is the mainstay of treatment, and the patient did have negative margins after complete surgical re-excision. The patient has surveillance MRI scans and remains without fibrosarcoma recurrence. This …

Flexible bronchoscopy is an important procedure for evaluating the pediatric airway, allowing a dynamic view from the trachea through the lower bronchi. The flexible bronchoscope offers greater maneuverability than the rigid bronchoscope and may be done in conjunction with other procedures, such as bronchoalveolar lavage, transbronchial biopsy, endobronchial ultrasound, electrocautery or laser treatments. Common indications for flexible bronchoscopy include recurrent croup or pneumonia, aspiration, foreign body and chronic cough. Although bronchospasm, transient fever, infection and pneumothor…

A complex of methods for the detection of Mycoplasma pneumoniae and Mycoplasma hominis in children and adults with respiratory diseases (acute, chronic and obstructive bronchitis, pneumonia, recurring croup, bronchial asthma), as well as in children frequently having acute respiratory diseases, has been worked out and tested. Both infective agents are frequently detected in the above mentioned pathological processes as monoinfection or mixed infection. Mycoplasma antigens are capable of prolonged (up to 1 year) persistence in the patient body in spite of etiotropic therapy and the presence of …

Foreign body (FB) aspiration is a critical paediatric emergency, often requiring timely diagnosis and intervention to prevent complications. We report the case of a three-year-old female presenting with persistent cough and breathing difficulty, initially managed as croup. Imaging revealed a subglottic FB, necessitating tracheostomy for airway stabilization. During the first rigid bronchoscopy attempt, the FB migrated into the left main bronchus, likely due to manipulation and the use of an uncuffed tracheostomy tube. A second bronchoscopy successfully removed the FB, a 2.5*2 cm bone chip, usi…

Human parainfluenza viruses (HPIVs) are the second most common cause of hospitalization in children, causing upper respiratory tract illness (URTI) and lower respiratory tract illness (LRTIs) in infants and young children. Common presentations include common cold, laryngotracheobronchitis (croup), bronchitis, and pneumonia. In immunocompetent adults, their effect is usually limited to mild upper respiratory tract illness with spontaneous recovery. However, elderly and immunocompromised adults are at risk for severe infection manifesting as epiglottitis, bronchiolitis, pneumonia, and on rare oc…

An 88-year-old woman presented to our emergency room with complaints of fever, coryza, barking cough and generalised fatigue for 2 days. Physical examination showed stridor, tachypnoea with use of accessory muscles of respiration on admission. Laboratory tests were unremarkable except for monocytosis with a normal total white cell count. Rapid influenza diagnostic test was positive for influenza A. Chest X-ray showed subglottic narrowing of the trachea suggestive of steeple sign. A diagnosis of influenza A-induced croup was made. She was given humidified oxygen, nebulised racemic epinephrine, …
