Cryptococcosis is a mycotic disease caused by the yeast Cryptococcus sp. It is associated with significant mortality and morbidity, including long-term neurological sequelae. It is not known whether the high morbidity observed is related to a delay in diagnosis. This study was carried out to provide current information on neuromeningeal cryptococcosis at the Infectious Diseases Clinic in Fann Teaching Hospital in Dakar. Epidemiological, clinical, biological and therapeutic data were collected retrospectively from files of patients treated between 1999 and 2003. Cryptococcosis is the third most common deep mycosis in Japan. Cryptococcus neoformans is known to grow in pyres of pigeon feces. Chicken feces in Thailand were tested for whether C. neoformans could be isolated, because there is considerable prevalence of cryptococcal meningitis in patients with HIV in that country. An immunocompetent 62-year-old woman presented with cutaneous lesions over her right arm and the hypogastrium. The diagnosis of cutaneous cryptococcosis was made on the basis of the demonstration and isolation of Cryptococcus neoformans from the aspirated pus. The patient responded to fluconazole treatment.

Cryptococcosis was diagnosed in a 17-year-old male llama that had been euthanatized following an acute onset of neurologic disease. Tissues affected included the brain, spinal cord, lung, and kidney. The character of the leukocytic response varied from minimal to pyogranulomatous. Cryptococcosis has not been previously reported in a llama, although the infection has been described in 2 other species of New World camelids. The pathogenesis of cryptococcosis is briefly reviewed.

Methods for the diagnosis of cryptococcosis have been established, including serotyping and serodiagnosis. Slide agglutination tests with factor sera, the phenol oxidase test, and the growth test at 37C are used for rapid identification of Cryptococcus neoformans. We identified 140 strains and found that 86, 10, and 4% of the isolates were serotypes A, D, and A-D, respectively. Twelve of 14 serotype D strains were isolated from cutaneous cryptococcosis.
