Diaphragm microvascular plasma PO2 measured in vivo.

Phosphorescence quenching techniques measure microvascular PO2 without direct surgical manipulation of the tissue. At a given arterial PO2, microvascular PO2 reflects the local O2 uptake-to-O2 delivery ratio, i.e., VO2/QO2. We evaluated the potential of phosphorescence quenching to determine microvascular PO2 in the rat costal diaphragm (PdiaO2). PdiaO2 and arterial blood gases were monitored across transient changes of inspired O2 among 21, 10, and 100% and also during hypotensive states evoked by progressive phlebotomy. After a transit delay, PdiaO2 responded rapidly to alterations of inspir…

Lambert-Eaton myasthenic syndrome involving the diaphragm.

Inspiratory muscle function was assessed in a patient with the Lambert-Eaton myasthenic syndrome that developed in association with a bronchogenic carcinoma. Repetitive maximal inspiratory pressure measurements and the electromyographic response to phrenic nerve stimulation established involvement of the inspiratory muscles in general and the diaphragm specifically in this condition.

Clinical significance of severe isolated diaphragm weakness.

We studied six patients with isolated bilateral paralysis or severe weakness of the diaphragm, present for 2 to 60 months (mean = 25), to document the clinical and respiratory sequelae of the condition. Severe diaphragm dysfunction was confirmed by the demonstration of the very low maximal transdiaphragmatic pressure (Pdi) generated by either a sniff (13 +/- 6 cm H2O, normal 148 +/- 24) or a static inspiration (11 +/- 8, normal 108 +/- 30) and during bilateral phrenic nerve stimulation (0.8 +/- 2.0, normal 22 +/- 4). Resting arterial blood gases were normal (SaO2 = 95 to 97%) and no oxygen des…

Diaphragm insertion increases human vaginal oxygen tension.

The normal insertion of a contraceptive diaphragm into the vagina of five young women induced a dramatic increase in the vaginal oxygen tension (131 +/- 13 mm Hg, mean +/- SD, n = 5), which slowly fell at an initial rate of 0.9 +/- 0.2 mm Hg/min. Withdrawal of the diaphragm immediately restored vaginal oxygen tension to the normal hypoxic state (11 +/- 12 mm Hg). The change in gaseous equilibrium created by the trapped air of the diaphragm could have significant actions on the microbial activity and balance of the vagina.

Familial bilateral paralysis of diaphragm. Adult onset.

At age 50 two homozygote twin brothers developed bilateral paralysis of the diaphragm. No infectious, metabolic, degenerative or proliferative disorders of the neuromuscular system and no thoracic diseases which could explain this lesion were detected during four years of follow-up. It appears thus to be an isolated lesion of genetic origin. To the best of our knowledge, no similar cases have been reported in literature.

Relationship between diaphragm length and abdominal dimensions.

We examined the relationship between changes in abdominal cross-sectional area, measured by respiratory inductive plethysmography, and changes in length in the costal and crural parts of the diaphragm, measured by sonomicrometry, in nine supine, anesthetized dogs. During passive inflation, both parts of the diaphragm shortened and abdominal cross-sectional area increased. During passive deflation, both parts of the diaphragm lengthened and abdominal cross-sectional area decreased. We subsequently used the relationship between costal and crural diaphragmatic length, respectively, and abdominal …

Acute viral polymyositis with predominant diaphragm involvement.

A 20-month-old mentally retarded girl developed an acute upper respiratory infection, followed by breathing difficulties, leading to death. Picorna virus-like particles were demonstrated in a mildly affected quadriceps femoris muscle biopsy, while the necropsy findings demonstrated an acute polymyositis with predominant diaphragm involvement. The mental retardation was due to micropolygyria of the brain. Initial respiratory difficulties are an unknown feature of polymyositis. In this case the upper respiratory infection, possibly caused by a Coxsackie B2, is evoked as responsible for this unus…

ACR Appropriateness Criteria® Suspected Diaphragm Dysfunction.

Diaphragm dysfunction includes diaphragmatic eventration, weakness, and paralysis, a set of disorders which are often challenging to diagnose clinically due to their varied presenting symptoms including dyspnea, positional orthopnea, cough, or even chest pain. Given the clinical challenge in diagnosing these conditions, diaphragm dysfunction is often suggested incidentally on initial imaging for workup of these symptoms. Imaging can play an important role both in raising the initial suspicion for diaphragm dysfunction as well as in making the diagnosis through real-time evaluation of the diaph…

Minimally Invasive Treatment of Traumatic Diaphragm Rupture.

BACKGROUND: Diaphragmatic rupture is a rare but potentially life-threatening injury that can result from blunt or penetrating trauma. Timely diagnosis is critical to prevent serious complications, yet clinical and radiological assessments are often inconclusive. CASE PRESENTATION: We report the case of a 39-year-old male patient who sustained blunt thoracoabdominal trauma in a motor vehicle accident. The patient presented with respiratory distress and generalized abdominal tenderness. Imaging revealed a left diaphragmatic defect with herniation of abdominal organs into the thoracic cavity. S…

Giant Pancreatic Lymphangioma Herniating Above the Diaphragm.

Lymphangiomas of pancreatic origin are rare and can often be found with abdominal symptoms or incidentally on imaging. A 46-years-old female presented with epigastric abdominal pain and vomiting, and twenty-pound weight loss over 3 months. Computed tomography of the abdomen showed a septated mass of the distal pancreas measuring 25.4 cm in largest diameter and extending superiorly into the mediastinum via a hiatal hernia. Endoscopic ultrasound demonstrated abutment but no invasion of the esophagus and stomach. She underwent an exploratory laparotomy with retroperitoneal mass excision, distal p

Regional diaphragm motion analysis via dynamic MRI.

Breathing-related movement analysis is important in the study of many disease processes. The analysis of diaphragmatic motion via thoracic imaging in particular is important in a variety of disorders. Compared to computed tomography (CT) and fluoroscopy, dynamic magnetic resonance imaging (dMRI) has several advantages, such as better soft tissue contrast, no ionizing radiation, and greater flexibility in selecting scanning planes. In this paper, we propose a novel method for full diaphragmatic motion analysis via free-breathing dMRI. Firstly, after 4D dMRI image construction in a cohort of 51 …

Synbindin Downregulation Participates in Slit Diaphragm Dysfunction.

INTRODUCTION: Synbindin, originally identified as a neuronal cytoplasmic molecule, was found in glomeruli. The cDNA subtractive hybridization technique showed the mRNA expression of synbindin in glomeruli was downregulated in puromycin aminonucleoside (PAN) nephropathy, a mimic of minimal-change nephrotic syndrome. METHODS: The expression of synbindin in podocytes was analyzed in normal rats and 2 types of rat nephrotic models, anti-nephrin antibody-induced nephropathy, a pure slit diaphragm injury model, and PAN nephropathy, by immunohistochemical analysis and RT-PCR techniques. To elucidate…

Diaphragm Disease: NSAID-Induced Small Bowel Stricture.

The use of nonsteroidal anti-inflammatory drugs (NSAIDs) is ubiquitous. However, it remains an oftentimes under-recognized risk factor for the development of strictures and small bowel obstruction. Herein we describe the case of a 63-year-old female with a prolonged course of abdominal pain and occult anemia found to have a diaphragmatic stricture in the small bowel related to chronic NSAID use.

Concurrent dual-scopic management of diaphragm disease.

With the increasing appliance of minimally invasive approaches in both thoracic and general surgery, more and more diaphragmatic conditions are being managed with laparoscopy or thoracoscopy. Which one is adopted depends on the disease, facility support and surgeon's preference. However, in difficult scenarios such as adhesions, uncertain tumour location or bulky herniated organs, addition of another approach could be beneficial. Here we describe 2 cases, one with metastatic tumours within the diaphragm and another with diaphragmatic hernia in which concurrent thoracoscopic and laparoscopic, s…

Diagnostic ultrasound imaging of mouse diaphragm function.

Function analysis of rodent respiratory skeletal muscles, particularly the diaphragm, is commonly performed by isolating muscle strips using invasive surgical procedures. Although this is an effective method of assessing in vitro diaphragm activity, it involves non-survival surgery. The application of non-invasive ultrasound imaging as an in vivo procedure is beneficial since it not only reduces the number of animals sacrificed, but is also suitable for monitoring disease progression in live mice. Thus, our ultrasound imaging method may likely assist in the development of novel therapies that …

Diaphragm and ventilatory dysfunction during cancer cachexia.

Cancer cachexia is characterized by a continuous loss of locomotor skeletal muscle mass, which causes profound muscle weakness. If this atrophy and weakness also occurs in diaphragm muscle, it could lead to respiratory failure, which is a major cause of death in patients with cancer. Thus, the purpose of the current study was to determine whether colon-26 (C-26) cancer cachexia causes diaphragm muscle fiber atrophy and weakness and compromises ventilation. All diaphragm muscle fiber types were significantly atrophied in C-26 mice compared to controls, and the atrophy-related genes, atrogin-1 a…

Robotic thoracoscopic plication for symptomatic diaphragm paralysis.

Diaphragmatic paralysis is an uncommon condition characterized by significant elevation of a hemidiaphragm, and can cause dyspnea. The goal of diaphragm plication is to improve dyspnea by correcting the dysfunctional movement of a diaphragm during inspiration. Minimally invasive thoracoscopic diaphragm plication has been widely used and has been reported to lead to significant improvements in dyspnea and postoperative pulmonary function. Advantages of thoracoscopic plication compared to open thoracotomy are less postoperative pain and shorter hospitalization, yet technical difficulties due to …
