"The Simplest Way to Go": An Exploration of Why Women Who Inject Drugs Chose Long-Acting Injectable Cabotegravir Instead of Daily Oral PrEP.

Long-acting injectable PrEP was approved for use in the US in 2021 but roll out has been slow, with few studies exploring uptake among cisgender women who inject drugs (WWID). We purposively recruited 25 WWID within 30-days of receiving a PrEP prescription from a low-barrier clinic co-located with a syringe services program to complete semi-structured interviews about PrEP product choice. We used an intersectional lens to compare decision-making between women choosing injectable PrEP versus oral PrEP and continued enrolling new participants until we reached thematic saturation (12/2022 to 2/20

Switch to long-acting cabotegravir and rilpivirine in virologically suppressed adults with HIV in Africa (CARES): week 48 results from a randomised, multicentre, open-label, non-inferiority trial.

BACKGROUND: Long-acting injectable cabotegravir and rilpivirine is licensed for individualised treatment of HIV-1 infection in resource-rich settings. Additional evidence is required to support use in African treatment programmes where demographic factors, viral subtypes, previous treatment, and delivery and monitoring approaches differ. The aim of this study was to determine whether switching to long-acting therapy with injections every 8 weeks is non-inferior to daily oral therapy in Africa. METHODS: CARES is a randomised, open-label, non-inferiority trial being conducted at eight sites in

Hepatitis B Virus Reactivation after Switch to Cabotegravir/Rilpivirine in Patient with Low Hepatitis B Surface Antibody.

A patient in Japan with HIV began antiretroviral therapy because of acute hepatitis B virus (HBV) 15 years ago, with low hepatitis B surface antibody, and experienced breakthrough HBV reactivation 4 months after switching from bictegravir/emtricitabine/tenofovir alafenamide to cabotegravir/rilpivirine. An immune escape mutation, E164V, was identified in the isolated HBV DNA.

Identification of neoechinulin B metabolites in vivo by UPLC Q/TOF MS and exploration of its anti HCV mechanism based on network pharmacology and molecular docking.

Fungal secondary metabolites, owing to their structural diversity and broad spectrum of bioactivities, have become a vital source for the discovery of microbial drugs and antiviral lead compounds. Fungi of the genus Aspergillus, which are ubiquitous filamentous microorganisms, are capable of producing structurally novel and functionally diverse secondary metabolites, thus representing a rich reservoir for drug lead discovery. Neoechinulin B (NeoB), an isoprenyl indole diketopiperazine alkaloid isolated from Aspergillus strains, exhibits potent antiviral activity. However, its poor characteriza

The cellobiose sensor CebR is the gatekeeper of Streptomyces scabies pathogenicity.

UNLABELLED: A relatively small number of species in the large genus Streptomyces are pathogenic; the best characterized of these is Streptomyces scabies. The pathogenicity of S. scabies strains is dependent on the production of the nitrated diketopiperazine thaxtomin A, which is a potent plant cellulose synthesis inhibitor. Much is known about the genetic loci associated with plant virulence; however, the molecular mechanisms by which S. scabies triggers expression of thaxtomin biosynthetic genes, beyond the pathway-specific activator TxtR, are not well understood. In this study, we demonstrat

Sequences of Landomycin E and Carotenoid Biosynthetic Gene Clusters, and Molecular Structure of Transcriptional Regulator of Streptomyces globisporus 1912.

Streptomyces globisporus 1912 and its derivatives 1912-2 and 1912-4Crt are the producers of the landomycin E, carotenoids and the regulator of antibiotics biosynthesis and morphogenesis of streptomycetes. The genome DNA of two mutant strains, 1912-2, the more effective producer of the landomycin E and the regulator, and 1912-4Crt, the producer of beta-carotene and lycopene, was sequenced by Illumina. Comparative analysis of the DNA sequences using GenBank data allowed localization of 36 landomycin E biosynthetic genes lnd of S. globisporus 1912 in one cluster. Twenty of these lnd genes have be

Brief Report: To Err Is Human, to Persist Is Diabolical: Are We Repeating the Same Cost and Insurance Coverage Mistakes Again With Injectable PrEP?

BACKGROUND: Long-acting injectable pre-exposure prophylaxis (PrEP) is the most promising biomedical HIV prevention innovation since the release of oral PrEP in 2012, but its reach may be undermined by inadequate access. The high list price of injectable PrEP has been cited as a critical driver of access issues for cabotegravir-formulated injectable PrEP, challenges that are likely to persist for the recently approved, twice-yearly injectable PrEP drug, lenacapavir. In this Viewpoint piece, we argue that now is the time to take stock of our recent progress with the rollout of injectable caboteg

Durability of Long-Acting Cabotegravir + Rilpivirine in Virologically Suppressed Adults Living With HIV: A Multicenter Observational Cohort in Tuscany (LAHIV).

To evaluate the durability of long-acting cabotegravir (CAB) plus rilpivirine (RPV), available in Italy since June 2022, for maintaining HIV-1 virological suppression. This multicentric observational study included 191 virologically suppressed adults (HIV-RNA < 50 copies/mL) from 11 centers in Tuscany, followed from first CAB + RPV injection until discontinuation, death, or last visit. Discontinuation was defined as regimen switch or two consecutive missed doses, virological failure (VF) as two consecutive HIV-RNA > 50 copies/mL or a single > 1000 copies/mL. Kaplan-Meier survival analysis asse

Cost-effectiveness of leveraging long-acting injectable cabotegravir to expand PrEP coverage among MSM in two contrasting North American cities.

INTRODUCTION: Long-acting injectable cabotegravir (CAB-LA) is superior to daily oral tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) for HIV pre-exposure prophylaxis (PrEP) and could expand PrEP usage. Given price differentials between CAB-LA and TDF/FTC, evaluating the cost-effectiveness of potential PrEP coverage scenarios is warranted. METHODS: We simulated PrEP coverage expansion among men who have sex with men (MSM) via introducing CAB-LA using two age- and risk-stratified HIV transmission models separately calibrated to local data from a high-incidence (Atlanta, USA) and a low-inc

Long-acting injectable cabotegravir and rilpivirine in observational cohort studies: A systematic review on virological failure, resistance and re-suppression outcomes in virally suppressed individuals living with HIV.

INTRODUCTION: Randomized controlled trial evidence suggests that long-acting injectable (LA-I) cabotegravir and rilpivirine (CAB+RPV) has similar virological failure (VF) rates to daily oral therapy, but clinical practice evidence is lacking. Integrase inhibitor (INI) resistance may limit future therapy. The optimal regimen is uncertain. METHODS: We synthesized evidence from PubMed, EMBASE, Cochrane and conference abstract databases through 18 November 2024, to identify observational cohort studies (OCS) that reported on VF events in virally suppressed individuals who switched to LA-I CAB+RPV

Do people with HIV really have the choice between oral and injectable ART? Evidence from a multicentric survey in the Swiss HIV cohort study.

INTRODUCTION: Injectable long-acting antiretroviral therapy (iLA-ART) offers a valuable alternative to oral ART (oART). While the efficacy of these treatment strategies is similar, adequate information on their specific characteristics is essential to enable people with HIV (PWH) to decide which option best suits their values and preferences. METHODS: We conducted a multicentric survey of PWH on oART in the Swiss HIV Cohort Study (SHCS). Using a questionnaire co-developed with expert patients, we assessed participants' (1) values and preferences on characteristics of modern oART and treatment

Experience of treatment with long-acting injectable cabotegravir/rilpivirine (LA iCAB/RPV) in viremic young patients with perinatally acquired HIV with adherence challenges: A five-case series.

BACKGROUND AND OBJECTIVES: Long-acting injectable antiretroviral therapy (ART) has transformed HIV care by reducing the burden of daily medication and improving adherence. While primarily approved for virologically suppressed individuals, emerging evidence supports its use in viremic patients with complex treatment histories. However, real-world evidence remains limited, especially in youth with perinatally acquired HIV. This case series provides further insights into the use of long-acting ART in this group. PATIENTS AND METHODS: This case series presents five young adults (aged 20-31) with

Long-acting injectable antiretroviral treatment: experiences of people with HIV and their healthcare providers in Uganda.

INTRODUCTION: Long-acting injectable antiretroviral treatment (LAI-ART) has emerged as a novel alternative to the burden of daily oral pills. The bi-monthly intramuscular injectable containing cabotegravir and rilpivirine holds the promise of improving adherence to ART. The perspectives of potential users of LAI-ART, the majority of whom reside in Eastern and Southern Africa, are still largely unexplored. We set out to understand the experiences of people with HIV (PWH) who received LAI-ART at Fort Portal Regional Referral Hospital in mid-Western Uganda for at least 12 months. METHODS: This q
