A 43-year-old woman with breast cancer underwent Tc-hydroxymethylene diphosphonate bone scintigraphy for preoperative workup. An abnormal lesion with increased uptake was found in the right humeral shaft, and F-NaF bone PET/CT was performed for clarification. There was high uptake of F-NaF without cortical bone disruption in the right humeral shaft, corresponding to the site on bone scintigraphy. Upper arm MRI was performed subsequently, and metastasis was suggested as a differential diagnosis. However, this lesion was pathologically diagnosed as fibrous dysplasia after curettage surgery.

Metatarsalgia is a general term for pain in the area of the metatarsophalangeal joints. Sesamoiditis is an uncommon cause of metatarsalgia. It is pain at the sesamoid bones beneath the head of the first metatarsal with inflammation or fracture. The diagnosis is usually clinical, with localized tenderness on palpation. Intraosseous pressure was measured in the lunate of 10 patients undergoing surgery for Kienböck's disease. Using the styloid process of the radius and the capitate as reference points of bone pressure, I found a stasis rather than arterial insufficiency in the lunate. A patient with alkaptonuria and ochronotic arthrosis was imaged twice with technetium-99m dicarboxypropane diphosphonate (99mTc-DPD)--once during a bout of arthritic knee pain and once when symptom-free. There was a marked accumulation of radioactivity in the large joints. During the episode of arthritis the knee joints had a higher uptake than when the patient was without symptoms. The intervertebral discs showed a high uptake which extended laterally from the axial vertebral column; the finding gave an impression of whiskers, and this "whisker sign" may be characteristic of ochronosis.

Daily administration of vitamin D3 (75,000 IU/kg b. wt) for 7 days accelerated Trichinella spiralis cyst calcification in rats with a 14-week-old infection. When disodium ethane-1-hydroxy-1, 1-diphosphonate (EHDP) was administered (50 mg/kg b. wt) from 2 days before until 2 days after vitamin D3 treatment, cyst calcification was inhibited. Thus, the ability to inhibit E. spiralis calcification has demonstrated for the first time.

We present a 51-year-old woman with a history of colectomy for colon cancer who was referred to our department for bone assessment. Planar image revealed a focus of increased uptake of Tc-99m methylene diphosphonate in the nose root. According to the images of SPECT combined with low-dose CT, the abnormal radiotracer uptake at the site of left frontal sinus was considered to be osteoma. We conclude that SPECT/CT provides an accurate evaluation of the localization of the lesions and supplies anatomic information that can be valuable for diagnosis of nonmalignant bone diseases.

A 52-year-old woman with nonspecific left leg pain was examined by Tc-99m methylene diphosphonate (MDP) bone scintigraphy. The patient had been a marble quarry worker for 10 years and had developed chronic congestive heart failure secondary to pneumoconiosis. Her hemoglobin analysis and hematologic findings were interpreted as being consistent with sickle cell beta+ thalassemia and also hypersplenism. Bone scintigraphy showed intense and diffuse MDP accumulation in the enlarged spleen without ultrasonographic or radiologic evidence of calcification.

One hundred thirty patients with lung cancer were studied to determine the incidence of unilateral thoracic soft-tissue accumulation (UTS) of 99mTc methylene diphosphonate (MDP). The finding was present in 60 of 130 (46%) of the patients. Of 52 patients who had received radiation therapy to the primary tumor in the chest, 46 (88%) had UTS, while six (12%) did not. Radiation therapy to lung tumors was the most significant of the factors studied in unilateral soft-tissue uptake of bone agent in the thorax of patients with lung cancer.

A 70-year-old man with a 20-year history of heart failure developed atrial fibrillation and underwent pacemaker implantation; however, his heart failure continued to worsen. Imaging with 99m Tc-PYP scintigraphy revealed significant uptake in the myocardium and the right pelvic bone. Similarly, 99m Tc-methylene diphosphonate scintigraphy demonstrated positive uptake in both regions. A myocardial biopsy confirmed cardiac amyloidosis, and mass spectrometry identified ApoA-I amyloidosis. In addition, a bone biopsy of the right iliac wing resulted in a pathologic diagnosis of Paget disease of bone.

A 4-year-old boy had had progressive central lumbar pain and hamstring spasm. He had a normal lumbar-spine x-ray except for minimal L-5, S1 spondylolysis, but gave an abnormal gallium-67 scan in the region of the low lumbar spine. Eight weeks following intensive antibiotic therapy, confirmation of the diagnosis of disc-space infection was established by roentgenographic studies that demonstrated narrowing of the L 4-5 intervertebral disc space.
