Clinical Application of 4D Flow MR Imaging for the Abdominal Aorta.

Blood vessels can be regarded as autonomous organs. The endothelial cells on the vessel surface serve as mechanosensors or mechanoreceptors for the flow velocity and turbulence of the blood flow in terms of wall shear stress (WSS), thereby monitoring changes in the flow velocity. Accordingly, the endothelial cells regulate the flow velocity by releasing numerous mediators. Such regulatory systems also trigger atherosclerosis, where the WSS decreases or fluctuates to maintain the flow velocity or local WSS. As occurrences of abdominal aortic aneurysms and aortic dissection are closely related t

Recurrent Cervical Artery Dissection Prevalence and Predictors: A Secondary Analysis of the STOP-CAD Study.

BACKGROUND: Patients with cervical artery dissection (CeAD) may experience a recurrent dissection, but its frequency, risk factors, and clinical implications are not well defined. We aimed to determine the risk, associated factors, and clinical impact of recurrent CeAD. METHODS: The STOP-CAD study (Antithrombotic Treatment for Stroke Prevention in Cervical Artery Dissection) was a multicenter international retrospective observational study of patients with CeAD treated between January 2015 and June 2022. Recurrent dissection was defined as a CeAD occurring at least 7 days after the diagnosis

Misdiagnosis of a Dissecting Aneurysm as a Cerebral Aneurysm: A Preoperative Case Report.

BACKGROUND: Intracranial Aneurysm (IA) is characterized by abnormal dilation of intracranial arterial walls, a tumor-like protrusion, often occurring in the anterior communicating artery. Intracranial Dissecting Aneurysm (IDA) refers to hemodynamic changes within intracranial arteries, leading to ruptures between blood vessel walls, disrupting normal arterial blood flow within the arterial lumen. IDA is relatively uncommon in the anterior circulation. To date, there have been no reported cases of dissecting aneurysms misdiagnosed as cerebral aneurysms before surgical intervention. This case re

A Simple Technique of Tunnel Constructing for Occluding Splenic Vessels During Laparoscopic Splenectomy in Patients with Cirrhosis.

Purpose: Dissecting and ligating the splenic artery is crucial for bleeding control during laparoscopic splenectomy (LS). However, for patients with portal hypertension from liver cirrhosis, it is difficult for identification and ligation because the splenic vessel is circuitous and dilated. The aim of this study was to describe a simple technique of constructing a tunnel behind the tail of the pancreas for occluding the splenic vessels during LS in patients with portal hypertension. Materials and Methods: The clinical data of 61 patients who underwent LS from April 2016 to January 2017 were r

Rescue technique for proximal flow diverter foreshortening leading to dislodgement into aneurysm.

Flow diverters (FDs) are an established treatment method for dissecting aneurysms.1-3 However, device shortening post-deployment and subsequent migration into the aneurysm may pose significant risks.4,5 In this technical video (Video 1), we present a case of Pipeline Flex Shield deployment for a dissecting aneurysm in the M1 segment of the left middle cerebral artery, where proximal segment shortening with subsequent device migration into the aneurysm sac was observed during the 9-month follow-up evaluation. In this procedure, we successfully advanced a microwire-guided microcatheter through t

[Aortic dissection at the acute phase].

ACUTE AORTIC DISSECTION Acute aortic dissection is the most common and dramatic of aortic emergencies. The many complications that it generates, such as rupture or malperfusion, make it a complex pathology to be managed in the context of an emergency, which requires many medical and surgical skills. Its medical management is based on the control of arterial pressure, and its surgical treatment on the replacement of the ascending aorta when damaged, or the endovascular repair of the descending thoracic aorta by aortic stentgraft. The clinical and radiological features of its prevention in the g

[Predictors of early adverse events after the frozen elephant trunk procedure].

AIM: The aim of this study was to identify predictors of adverse events after the frozen elephant trunk procedure in the early postoperative period. PATIENTS AND METHODS: Between March 2012 and March 2020, a total of 273 patients were operated on for aortic pathology. A retrospective analysis aimed at identifying probable predictors was performed on 83 patients who underwent the FET procedure. Uni- and multivariate logistic regression was used to identify predictors of such adverse events as postoperative delirium, respiratory failure, acute kidney injury, and in-hospital mortality. RESULTS:

[Attach importance to the planning and implementation of endovascular repair for ascending aorta dissection].

Since 2000 Dorros et al. reported the first case of type A aortic dissection (TAAD) treated with an endovascular repair, surgeons explored a novel treatment option for TAAD gradually. The application of endovascular repair for TAAD highlights some points below which should pay attention to because of the special anatomic location and morphological characteristics of ascending aorta: (1) customized treatment strategy based on the characteristics of patients; (2) pre-operation evaluation; (3) selection of the stent and delivery system; (4) selection of the access sites; (5) preservation of the c

Rapidly successive, symptomatic, bilateral, spontaneous vertebral artery dissections: treatment with stent reconstruction.

BACKGROUND: Vertebral artery dissection has been increasingly identified as a cause of ischemic stroke in young adults. This condition is typically secondary to trauma, with spontaneous VA dissection accounting for only a minority of cases. We report the case of a patient with bilateral symptomatic spontaneous VA dissections, presenting in rapid succession. Each ictal event was successfully managed with stent reconstruction and antiplatelet therapy. CASE DESCRIPTION: A 40-year-old gentleman, with no preceding trauma or apparent inciting event, presented with the acute onset of imbalance, dysa

[Recent advancement of surgical treatment of aneurysms of the thoracic aorta].

The recent advancement of surgical treatment for aneurysms of the thoracic aorta with special reference to the operative technique and adjunctive methods of distal aortic perfusion during aortic cross-clamping were reviewed. Between 1960 and July, 1991, 415 patients underwent operation for aneurysms of the thoracic aorta in our institution. The overall early mortality rates were 7.7% for the nonruptured aneurysms and 30.6% for the ruptured aneurysms during the last 10 years with recent establishment of mechanical adjuncts and refinement of operative technique. Composite graft replacement with

Acute and Chronic Management of Spontaneous Coronary Artery Dissection.

Spontaneous coronary artery dissection (SCAD) is an increasingly recognized, non-atherosclerotic cause of acute coronary syndrome (ACS), predominantly affecting younger women without traditional cardiovascular risk factors. SCAD results from an intramural hematoma or intimal tear that compresses the coronary lumen, leading to myocardial ischemia. Diagnosis is critical, as management differs fundamentally from atherosclerotic ACS. Conservative therapy is preferred for hemodynamically stable patients due to high rates of spontaneous vessel healing; revascularization is reserved for ongoing ische

A Novel Single-Branched Stent-Graft System for Aortic Dissection Involving the Aortic Arch.

PURPOSE: This report describes first-in-human experiences and early results of novel SureCham stent-graft system (SC) for aortic arch pathologies. DESCRIPTION: The SC system consists of a modular design comprising a pre-fenestrated main stent-graft and a designed skirt-flared bridging covered stent to enhance the structural integrity of the stent connection. The SC incorporates a preloaded catheter for precise supraaortic cannulation, along with a precurved delivery system that enables navigation and accurate alignment. Collectively, these design features facilitate efficient reconstruction o

[Craniocervical dissection].

CLINICAL ISSUE: Craniocervical dissections are among the most common causes of stroke in people aged under 50 years, which is why it is essential to clarify, diagnose, and treat them as quickly as possible. Dissections usually occur spontaneously due to bleeding into the vessel wall. The affected segments are usually the motion segments of the internal cerebral artery (C1 segment) and the vertebral artery (V3 segment). Clinically, there is head and/or neck pain and neurologic symptoms, which can vary according to the localization of the dissection. PRACTICAL RECOMMENDATIONS: Pathognomonic is

Internal Trapping of a Growing Ruptured Dissecting Aneurysm of the A1 Segment: A Case Report and Literature Review.

A 47-year-old man presented with sudden-onset headache and Fisher group 3 subarachnoid hemorrhage. The World Federation of Neurological Surgeons grade was II. Digital subtraction angiography (DSA) only showed a vessel wall irregularity in the A1 segment of the right anterior cerebral artery (ACA), but an obvious bleeding source was not detected. Repeat angiography showed a tiny aneurysmal dilatation in the A1 segment with an intimal flap. The aneurysm enlarged on subsequent angiograms. Dissecting aneurysm was diagnosed, and the patient underwent internal trapping of the A1 segment to prevent r

[Distal Stent Graft-induced New Entry on the Lesser Curvature of Distal Aortic Arch:Report of a Case].

A 62-year-old male was admitted for cerebral hemorrhage, enhanced computed tomography (CT) revealed a dissecting thoracic aortic aneurysm in the distal arch. Following cerebral hemorrhage treatment, the patient was referred to our department and underwent a total arch replacement using the frozen elephant trunk (FET) technique. The patient was followed up regularly post operatively at our hospital. Enhanced CT at 31 months post surgery revealed a distal stent graft-induced new entry (dSINE) on the lesser curvature of the distal arch. Additional thoracic endovascular aortic repair (TEVAR) was p

Concomitant aortic root replacement during frozen elephant trunk implantation does not increase perioperative risk.

OBJECTIVES: Our aim was to evaluate the risk of concomitant aortic root replacement during frozen elephant trunk (FET) total arch replacement. METHODS: Between March 2013 and February 2021, 303 patients underwent aortic arch replacement using the FET technique. Patient characteristics, intra- and postoperative data were compared between patients with (n = 50) and without (n = 253) concomitant aortic root replacement (implantation of a valved conduit or using the reimplantation valve-sparing technique) after propensity score matching. RESULTS: After propensity score matching, there were no st
