The experiences of parents and caregiver(s) whose child received an organ from a living anonymous liver donor.

BACKGROUND: Anonymous living donor transplantation is a potential strategy to address the shortage of available organs for transplant. A living anonymous donor (LAD) is a donor with no biological connection and whose identity is unknown to the recipient. This study captured the lived experiences of pediatric liver transplant recipient families whose child received an organ from a LAD. METHODS: Qualitative data collection and analysis were guided by a theoretical framework of phenomenology. Data analysis highlighted themes through an inductive process of reviewing transcript paragraphs to code

Choice architecture for young adult blood donor recruitment - a feasibility study.

Blood shortages remain an ongoing challenge, ameliorable by increasing blood donations. Choice architecture is an emerging concept in psychology dealing with the targeted presentation of options to encourage a desired decision. A pilot study was designed to test the feasibility of implementing six choice architecture strategies on a Midwest high-school blood drive. These include framing, conformity, mere measurement, availability heuristic, loss aversion and status quo bias. A pre-post interrupted time-series evaluation was performed to evaluate for an immediate impact on blood donations. All

Pediatric liver transplantation at the University of Chicago Hospitals.

The concept of graft-reduction surgery in liver transplantation has revolutionized pediatric transplantation and soon may carry over into adult transplantation. Reduced-size liver transplants have virtually eliminated patient deaths while waiting for donor organs to become available by increasing the potential donor pool. This has shifted the distribution of pediatric liver transplant recipients to a younger age which more closely matches the needs of pediatric patients with end-stage liver disease. Split-liver transplants can create new grafts without increasing the donor supply. Living-donor

Reproductive parameters of Mangalarga Marchador mares in a commercial embryo transfer programme.

The objective of this study is to evaluate the reproductive efficiency in donors and recipient Mangalarga Marchador mares in commercial programmes of embryo transfer (ET) and the effects of some reproductive characteristics and ET methodology on conception rates in the recipient mares. A total of 1140 flushing procedures were performed and 830 embryos (72.8%) were recovered. There were no differences between the rates of embryonic recovery in the different breeding seasons (p > 0.05) and 92.8% of the recovered embryos were 8-9 days old. There was no difference in the embryonic recovery regardi

Success of intrauterine insemination using cryopreserved donor sperm is related to the age of the woman and the number of preovulatory follicles.

OBJECTIVE: Our objective was to assess parameters associated with a successful outcome of intrauterine insemination (IUI) using cryopreserved donor sperm. DESIGN: We analyzed 750 consecutive donor IUI cycles undertaken by 363 women in an assisted conception clinic. The main outcome measure was clinical pregnancy. RESULTS: IUI was performed in 94.7% of the 750 IUI treatment cycles commenced and 180 clinical pregnancies occurred. The clinical pregnancy rate per cycle was 26.4%. The rate was significantly related to the patient's age (30.5% for age < or = 35 years and 18.1% for age > 35 years;

[Blood components: Are they drugs or special medicines?].

Blood transfusion and plasma derived-drugs significantly differ from other medicines in that their availability strictly depends on blood and plasma collected from healthy donors. Blood collection must comply with a double objective: to maintain donor heath safety, and to avoid any transmitted infections in recipients. This raises several ethical concerns that appear to be different from usual ethical and deontological issues linked to other pharmaceutical and industrial processes. The main concern is the non-commercialization of the human body. Words and concept are of major importance in thi

Kidney Transplants From Marginal Donors: From Brenner's Abstract to Double Kidney Transplantation in Humans.

"In 1993, based on findings from their experimental studies of kidney mass manipulation in a rat kidney transplant model, Barry Brenner and his colleagues postulated that augmenting the number of viable nephrons in kidney transplantation to meet the recipient's metabolic and excretory demands may protect against long-term graft loss. This enlightening idea set the stage for the development and worldwide implementation of dual kidney transplant programs using organs from marginal-including older-donors. This concept was then complemented by robust clinical evidence that showed that a scoring sy

Organic Molecules Containing N, S and O Heteroatoms as Sensors for the Detection of Hg(II) Ion; Coordination and Efficiency toward Detection.

Rapid detection of potentially toxic heavy metals like Hg(II) has attracted great attention in the last few decades due to the importance to maintain a safe and sustainable environment for human beings. Coordination chemistry and concepts therein, play an important role in the detection of Hg(II). Size, charge, and nature of the donor atom and the respective cation (metal ion), are crucial in selective interactions between the sensor and metal ions. The sensors designed for the purpose, coordinate to Hg(II) ion through various donor sites, coordination causes a change in the electron density i

The clinical content of preconception care: genetics and genomics.

The prevalence of paternal and maternal genetic conditions that affect pregnancy varies according to many factors that include parental age, medical history, and family history. Although some genetic conditions that affect pregnancy are identified easily early in life, other conditions are not and may require additional diagnostic testing. A complete 3-generation family medical history that includes ethnicity information about both sides of the family is arguably the single best genetic "test" that is applicable to preconception care. Assessment of genetic risk by an experienced professional h

An Ideal Choice for Closure of Large Free Radial Forearm Flap Donor Site Using a Local Full-Thickness Skin Graft.

The local full-thickness skin graft (FTSG) method based on the V-Y closure is reported to be the most convenient and reliable technique for free radial forearm flap (FRFF) donor site closure. The spindle-shaped FTSG was the most common local closure method in free osteofasciocutaneous fibula flaps but not FRFF reconstructions, despite being conveniently applicable to FRFF donor sites. The authors devised a spindle-shaped FTSG harvested from the ipsilateral radial forearm as a new concept in repairing the FRFF donor site. This procedure was performed on 4 patients and the average flap defect si

The role of extracorporeal membrane oxygenation in donation after circulatory death.

Donor scarcity and the increased need for organ transplantation has prompted the development of an alternative source of donors to the more conventional brain dead donor. While in a Beating-Heart donor, abdominal and intrathoracic organs are perfused, in a non-beating heart donor (NHBD, or DCD), perfusion should be maintained, after confirmation of death, by means of ECMO and inflation of intra-aortic balloon accordingly to the localization of the organs that should be transplanted. In this setting, ECMO allows selective perfusion of the organs which should be transplanted ("compartmental ECMO

Reactive oxygen species and their role in the andrological factor of couple fertility.

Reactive oxygen species play a significant role in male fertility and infertility. They are essential for physiological processes, but when their concentration becomes excessive, they can be a cause of various sperm pathologies. Seminal leukocytes and pathologically abnormal sperm are the primary sources of oxygen radicals in ejaculate. They negatively affect sperm quality, including DNA fragmentation and sperm motility impairment. Addressing increased concentrations of reactive oxygen species involves various appropriate lifestyle modifications and measures, including the use of antioxidants,

Hepatic IL-8 release during graft procurement is associated with impaired graft function after human liver transplantation.

In experimental models, brain death induces inflammatory cascades, leading to reduced graft survival. Thus far, factors prior to graft preservation have gained less attention in clinical setting. We studied pre-preservation inflammatory response and its effects on graft function in 30 brain dead liver donors and the respective recipients. Before donor graft perfusion, portal and hepatic venous blood samples were drawn for phagocyte adhesion molecule expression and plasma cytokine determinations. Donor intensive care unit stay correlated with donor C-reactive protein (R = 0.472, p = 0.013) and

Two decades of psychological adjustment of donor-conceived offspring of lesbian parents: examining donor contact and type.

RESEARCH QUESTION: How stable is psychological adjustment in donor-conceived offspring of lesbian parents over 21 years, considering developmental stages, donor type and impact of donor contact? DESIGN: Data were drawn from the US National Longitudinal Lesbian Family Study, a cohort of 70 donor-conceived offspring (71.43% heterosexual, 90.00% White) assessed across four developmental stages - childhood (age 10 years, Wave 4), adolescence (age 17, Wave 5), emerging adulthood (age 25, Wave 6) and established adulthood (ages 30-33, Wave 7). From parental reports, 34.29% had an anonymous donor, 3

Physiognomy, familism and consumerism: preferences among Jewish-Israeli recipients of donor insemination.

This paper examines the physiological preferences of Jewish-Israeli donor insemination (DI) recipients regarding the desirable donor. (1) By comparing recipients' own physiognomy with that of a hypothetical donor, we explore some of the social potentials of DI in its emerging democratized form. We examine prevailing notions regarding the 'natural family' and 'family resemblance' and query the relative weight of the natural vs. the consumer model as applied to one's own family through considerations regarding physical appearance. While examining the 'natural family' concept as a limit in the ma

Analysing lapsing rates among first-time blood donors at a blood centre in Zimbabwe using survival analysis.

INTRODUCTION: blood centres are often faced with the problem of donor lapsing resulting in loss of donors from the already strained donor pool. In Zimbabwe, 70% of the donated blood comes from younger donors aged 40 years and below, who at the same time, have high attrition rates. This study seeks to apply the concept of survival analysis in analysing blood donor lapsing rates. METHODS: in analysing the donor lapsing and retention rates, data on 450 first-time blood donors at the National Blood Service Zimbabwe, in Harare´s blood bank for the period 2014 to 2017 was extracted from the donors´

Population-based study of cesarean section after in vitro fertilization in Australia.

BACKGROUND: Decisions about method of birth should be evidence based. In Australia, the rising rate of cesarean section has not been limited to births after spontaneous conception. This study aimed to investigate cesarean section among women giving birth after in vitro fertilization (IVF). METHODS: Retrospective population-based study was conducted using national registry data on IVF treatment. The study included 17,019 women who underwent IVF treatment during 2003 to 2005 and a national comparison population of women who gave birth in Australia. The outcome measure was cesarean section. Simultaneous bilateral foot reconstruction using a single radial forearm flap.

The radial forearm septocutaneous flap is an excellent source of thin tissue with an anatomically consistent network of large vessels that simplify microsurgical transfer. It may be divided into multiple flaps leaving a single donor-site defect. The clinical usefulness of this concept is demonstrated in a single case in which simultaneous coverage of electrical burns of both feet was required.

Pediatric Cardiac Xenotransplantation as a Bridge to Allotransplantation: State of the Field and Preclinical Insights From a Pediatric Model.

Infants with complex congenital heart disease represent a vulnerable subset of pediatric patients who experience unacceptably high mortality while awaiting cardiac allotransplantation. The limited availability of size-matched donor organs and the unsuitability of existing mechanical circulatory support in this patient population compels the development of alternative bridging strategies. Cardiac xenotransplantation using genetically modified porcine donors represents a potential solution to this complex clinical issue. In this review, we describe the scientific and clinical landscape of pediat

The COPe program: communicational, organizational and personal factors influencing the transplant process and the number of donor reports.

In this article a supporting program for increasing donor rates is described. Beside medical knowledge and skills, communication and organisation as well as personal values and beliefs are determining factors for the success of every single step in the whole process of organ transplantation. Training for the sensibility in communication, developing concepts for the organization within the team and the work with personal attitudes and values, not only with skills and tools are necessary. In already evaluated sections of our program we markedly improved personal attitudes, self-management and st
