An Effective Surgical Method for Correcting Different Degrees of Constricted Ears.

BACKGROUND: The constricted ear, also known as cup ear or lop ear, is visually characterized by the curling and lidding of the helix. As the deformity intensifies, it also involves structural abnormalities of the antihelix, scapha, and triangular fossa, resulting in the auricle presenting a lidding, protrusion, decreased vertical height and low position appearance. Exploring a cost-effective treatment strategy for all Tanzer types of constricted ears remains a significant challenge. METHODS: Thirty-four patients with unilateral constricted ears who underwent surgical correction from January 2

Formation of tissue engineered composite construct of cartilage and skin using high density polyethylene as inner scaffold in the shape of human helix.

BACKGROUND: Formation of external ear via tissue engineering has created interest amongst surgeons as an alternative for ear reconstruction in congenital microtia. OBJECTIVE: To reconstruct a composite human construct of cartilage and skin in the shape of human ear helix in athymic mice. METHODS: Six human nasal cartilages were used and digested with Collagenase II. Chondrocytes were passaged in 175 cm(2) culture flasks at a density of 10,000 cells/cm(2). Frozen human plasma was then mixed with human chondrocytes. Six human skin samples were cut into small pieces trypsinized and resuspended.

Fabrication of a three-dimensional autogenous costal cartilage framework in auricular reconstruction: experience with 250 cases of Asian lobule-type microtia.

BACKGROUND: Ear reconstruction is the necessary choice for patients with congenital lobule microtia. The best results are still obtained by autogenous cartilage ear reconstruction. We developed a three-dimensional autogenous costal cartilage framework for auricular reconstruction with the purpose of achieving a new auricle of high fidelity. METHODS: The patient's sixth to eighth rib cartilages were harvested as auricular framework materials, which were divided into three laminates for carving fine anatomic structures. By connecting and fixing, a three-dimensional auricular framework with a co

Variant Types of Preauricular Sinuses: Classifications, Clinical Presentation and Management.

OBJECTIVE: To make surgeons aware of the differing types of preauricular sinuses (PAS), we summarize our experience with diagnosis and treatment of varying types of PAS. METHODS: We retrospectively reviewed clinical data from patients who had undergone preauricular fistulectomy between March 2015 and March 2020. These patients were categorized into two groups according to locations of congenital fistula pit. RESULTS: Twelve patients with variant PAS accounted for 6.8% (12/177) of all patients. The variant types of PAS could be classified into three types (from type 1 to type 3), based on the

Innervation of the Human Cavum Conchae and Auditory Canal: Anatomical Basis for Transcutaneous Auricular Nerve Stimulation.

The innocuous transcutaneous stimulation of nerves supplying the outer ear has been demonstrated to be as effective as the invasive direct stimulation of the vagus nerve for the treatment of some neurological and nonneurological disturbances. Thus, the precise knowledge of external ear innervation is of maximal interest for the design of transcutaneous auricular nerve stimulation devices. We analyzed eleven outer ears, and the innervation was assessed by Masson's trichrome staining, immunohistochemistry, or immunofluorescence (neurofilaments, S100 protein, and myelin-basic protein). In both th

Randomized study comparing fascia and cartilage grafts in myringoplasty.

OBJECTIVES: The study compares the medium-term outcomes of myringoplasty procedures using fascia and cartilage grafts. METHODS: Patients with chronic otitis media with perforations larger than 50% of the size of the tympanic membrane were included in a randomized, controlled, prospective clinical trial. The perforations were repaired with either temporalis fascia (20 ears) or cartilage (18 ears) grafts selected randomly. A search of the literature was performed to look for other randomized studies comparing fascia and cartilage. RESULTS: The graft take rates of fascia and cartilage grafts at

Management of the Prominent Ear.

This article incorporates the opinions and preferred surgical options in managing patients of 3 prominent facial plastic surgeons who have large otoplasty practices. Six different questions covering the management of prominent ears are answered by the 3 practitioners. Nonsurgical options for the treatment of prominent ears are discussed. The role of cartilage-cutting and cartilage-sparing techniques as well as individual preferred otoplasty techniques are thoroughly covered. Postoperative management of these patients is presented by the individual surgeons.

Candidate's thesis: Revision tympanoplasty utilizing fossa triangularis cartilage.

OBJECTIVES/HYPOTHESIS: A small percentage of fascia graft tympanoplasties fail. Cartilage tympanoplasty has a reputation for excellent graft healing but potentially sacrifices maximum hearing improvement and creates difficulty in postoperative follow-up resulting from opacity and immobility. We sought to use a tissue thicker than fascia but thinner than tragal cartilage to repair tympanic membranes that had failed previous fascia grafting. Our hypothesis was that use of the thinner cartilage would maintain the excellent healing rate and resistance to chronic negative pressure while improving h

Total auricular reconstruction without skin grafting.

Successful ear reconstruction depends on two factors: an ear framework and the skin covering the framework. However, the relative deficiency of skin for coverage of the cartilage framework remains an issue. This new method for total auricular reconstruction is a three-stage operation and involves the use of two tissue expanders. First, two skin expanders are implanted, one underneath the scalp and the other sited behind the microtic ear. At the second stage, after lobule transposition, the two expanded skin flaps (upper and lower) and mastoid fascial flap are raised. At the same time, the auto

The Pillars Concept: An Approach for Managing Hypertrophic Concha in Otoplasty.

Prominent ear deformities often result from a combination of hypertrophic conchal cartilage and an underdeveloped antihelical fold. Traditional otoplasty techniques, such as antihelical folding sutures and conchal setback maneuvers, may introduce tension and risk of relapse, leading to suboptimal aesthetic outcomes. The PILLARS CONCEPT: represents an alternative surgical approach specifically designed to address hypertrophic concha, ensuring stable and natural results. This study aimed to present the Pillars Concept as an alternative method for correcting hypertrophic conchal cartilage, highlig

Brent Technique of Repair Versus Nagata Auricular Reconstruction for Microtia Reconstruction: A Systematic Review and Meta-Analysis.

BACKGROUND: Up to 17.4 in every 10,000 births are affected by microtia, but no consensus exists on a gold standard technique for autogenous repair. In this study, the authors compare 2 common methods-the Brent and Nagata autogenous costal cartilage ear reconstruction techniques. A systematic review of the literature and a quantitative meta-analysis to compare the outcomes of these 2 approaches were performed. The outcomes analyzed included rates of infection, necrosis, cartilage exposure, cartilage resorption, hematoma, wire extrusion, and hypertrophic scar. METHODS: A MEDLINE database system

The crumpled-ear deformity.

The common characteristics of the crumpled-ear deformity, the steps for surgical correction, and the authors' experience are described in this article. Commonly, the ear has a folded-over appearance; a normal length and width when unfurled; and wrinkling of the skin and cartilage of the helical rim, scapha, antihelix, and concha. Principles of correction of this deformity include (1) superficial scoring of the concave segments of the cartilage to open the wrinkled segments, (2) creating the antihelical fold by a mattress-suture technique, (3) repositioning of the helical rim, and (4) repositio

Microvascular ear reconstruction using a free radial forearm flap after dog bite.

A 68-year old woman suffered from a subtotal amputation of the left ear due to a dog bite. The recovered ear was implanted and reconstructed using a radial forearm flap. In our opinion, the forearm is an excellent choice for implantation of recovered ear cartilage as preparation for one-stage reconstruction.

Tissue Engineering Auricular Cartilage: A Review of Auricular Cartilage Characteristics and Current Techniques for Auricular Reconstruction.

Microtia and anotia are congenital auricular anomalies that negatively impact the psychosocial development of those affected. Because auricular cartilage is a type of elastic cartilage that lacks regenerative capacity, any notable defect in its structure requires a surgical approach to reconstructing the auricle. While there are several reconstructive options available between alloplastic and prosthetic implants, autologous rib cartilage grafts remain the most commonly used treatment modality. Still, this widely used technique is accompanied by significant patient discomfort in a young child a

A new method in the treatment of ear amputation: experimental and clinical study.

There have been plenty of reconstruction methods for ear amputation, and replantation preserves its importance. In situations where replantation is not feasible, various methods were proposed. We indicate an alternative technique for the ear amputation without replantation indication. The method of replacing of a vascular structure into the tunnel formed on the posterior side of the amputated ear was used instead of replacing the ear cartilage into a vascular area that was described in the literature of ear prefabrication. The dorsal fascial flaps which were prepared from the back of 10 New Ze

Firm elevation of the auricle in reconstruction of microtia with a retroauricular fascial flap wrapping two titanium plate struts.

BACKGROUND: Reconstruction of the cephaloauricular sulcus in patients with microtia in ear reconstructions remains challenging. Costal cartilage and other support materials wedge were used for ear elevation. Each material has its disadvantages. To reconstruct a stable cephaloauricular sulcus and reduce costal cartilage harvesting, we used two titanium plate struts to support the costal cartilage framework. METHODS: A titanium plate strut was designed. The angle of the strut was 60°, the arm length was 0.8 cm, and the width was 0.5 cm. The thickness was 0.6 mm. Four small pores are formed on t

Safe and Simple Costal Cartilage Harvest.

Most rhinoplasty patients have insufficient reconstructive cartilage, which is often required for structural considerations like tip support, reconstructing internal or external nasal valves, or for dorsal augmentation or camouflage. When there is insufficient septal cartilage, the surgeon must turn to other cartilaginous sources including ear cartilage, autologous rib cartilage, or cadaveric rib cartilage. While adding rib cartilage harvest, surgeons often hesitate with autologous rib cartilage as the case may be prolonged or possible increased morbidity due to the risk of a pneumothorax. How
