Triceps-to-Biceps Tendon Transfer for Restoration of Elbow Flexion in Brachial Plexus Injury.

PURPOSE: Restoring elbow flexion is a reconstructive priority in patients with brachial plexus injuries. This study aimed to evaluate the results and assess factors contributing to outcomes of triceps-to-biceps tendon transfer in patients presenting with delayed or chronic upper brachial plexus injury. METHODS: Patients with traumatic brachial plexus injuries undergoing triceps-to-biceps tendon transfer at a single institution's multidisciplinary brachial plexus center between 2001 and 2021 were retrospectively reviewed. The entire triceps tendon was transferred around the lateral aspect of t

Paediatric lateral condyle fracture with a posterolateral elbow dislocation: an atypical injury in a preadolescent.

Paediatric lateral condyle fractures of the distal humerus are common but a concomitant elbow dislocation is rare. Typically, paediatric orthopaedic surgeons will treat lateral condyle fractures with pin or metaphyseal single-screw fixation and supplementary immobilisation for several weeks. These techniques sacrifice the early stability and mobilisation necessary to avoid stiffness after a complex elbow fracture-dislocation. We present an 11-year-old boy who sustained a traumatic posterolateral elbow dislocation with lateral condyle and coronoid fractures. Due to advanced skeletal age, both p

Prior to Repair Functional Deficits in Above- and Below-Elbow Ulnar Nerve Injury.

PURPOSE: Clinical deficits might vary, depending on whether an ulnar nerve lesion is above or below the elbow. Lack of strength and clawing … nd pinch-to-zoom decreased by 62%, 51%, and 75% compared with 59%, 61%, and 76% in below- and above-elbow injuries groups, respectively. In both groups, sensory deficits were predominantly over the little finger and ulnar side of the hand. CONCLUSIONS: Minimal differences were observed in clinical deficits after above- and below-elbow ulnar nerve injuries. Hand …

Trapezius Muscle Transfer for Restoration of Elbow Extension in a Traumatic Brachial Plexus Injury.

Voluntary elbow extension is essential for optimal upper limb positioning required for daily living activities, particularly above-shoulder maneuvers. The authors present a case of traumatic brachial plexus injury in which paralysis of the musculature selectively supplied by the posterior cord was based on magnetic resonance imaging and nerve conduction studies. An attempt at a radial nerve graft at another center was not effective. Ipsilateral hand function improved after multiple local tendon transfers were performed. Restoration of active elbow extension was not possible using the posterior

Fracture-dislocation of the elbow with inferior radioulnar dislocation: a variant of the Essex-Lopresti injury.

We describe two patients with an Essex-Lopresti fracture dislocation in association with a dislocation of the elbow. This combination of injuries has not been previously reported. The Essex-Lopresti fracture is a rare injury, and the associated distal radioulnar dislocation is often missed. Meticulous radiographic evaluation of the wrist, including dynamic stress radiographs of the forearm, can lead to an earlier diagnosis and improved care of these patients.

Nerve injury in revision total elbow arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Revision total elbow arthroplasty (TEA) is technically demanding and carries a substantial risk of postoperative neurological complications because of scarring, altered anatomy, implant removal, and repeated humeral and ulnar exposure. The incidence, nerve distribution, and recovery profile of nerve injury after revision TEA remain incompletely defined. This study aimed to systematically review the literature to define the incidence, recovery profile, and risk factors for nerve injury after revision TEA. METHODS: A systematic review of the literature was performed in accordance wi

Risk of nerve injury during elbow arthroscopy: ultrasonographic evaluation of preoperative patients.

BACKGROUND: To clarify the real risk of nerve injury during elbow arthroscopy, the distances of the radial and median nerves to the elbow joint were investigated using ultrasonography in patients who underwent surgery. METHODS: A total of 35 patients who underwent arthroscopic surgery of the elbow were investigated. The distances of the nerves to the capsule and bony landmarks were measured using ultrasonography. The radial nerve distances were measured at the capitellum, joint space, radial head, and radial neck levels. The median nerve distances were measured at the trochlear, joint space,

Pediatric anterior elbow dislocation due to a rare mechanism of injury: A case report.

INTRODUCTION & IMPORTANCE: Elbow dislocations are rare among the pediatric population as their ligaments are stronger than bones. Anterior dislocations of the elbow are even rarer entity usually caused by anteriorly directed force on the hyper-flexed elbow. We present a case of a rotational force on an extended elbow causing anterior dislocation of the shoulder in an 11-year-old boy. CASE PRESENTATION: An 11-year-old boy presented to the Accident & Trauma unit with pain & swelling of the right elbow following a twisting injury by a spinning washing machine. The child was in pain. Injury of the brachial artery accompanying simple closed elbow dislocation: a case report.

BACKGROUND: Elbow dislocation is the second common dislocation in adults, after the shoulder. The anatomical proximity to the joint of the brachial artery could lead to concomitant vascular injuries, even if their occurrence remains very rare. METHOD: It is reported the case of a right-hand-dominant  42-year-old man who sustained a simple closed  posterior elbow dislocation of  his left elbow, associated to a complete brachial artery rupture. He urgently underwent the reduction of the joint dislocation and an artery-repairing surgical procedure using a graft from ipsilateral saphenous vein.

Humeral Torsion as a Risk Factor for Shoulder and Elbow Injury in Professional Baseball Pitchers.

BACKGROUND: Numerous studies have demonstrated that humeral retrotorsion is increased in the dominant arms of throwing athletes. No study has clearly defined the relationship between humeral retrotorsion and shoulder and elbow injury. HYPOTHESIS: Uninjured professional pitchers will display more dominant humeral torsion (HT) than professional pitchers who sustain shoulder injuries but less than pitchers who sustain elbow injuries. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Pitchers from the Colorado Rockies professional baseball organization were recruited for particip…

[APPLICATION OF COMPRESSION MINI-SCREWS IN TREATMENT OF PATIENTS WITH INJURY OF ELBOW JOINT BONES].

A case report presents the experience of application of compression pileateless mini-screws (Gerbert's screws) in treatment of intra-articular fractures, which formed the elbow joint (44 cases). There were performed 32 operations concerning fracture of head of radius, 10 operations on the occasion of fractures of distal section of the humerus and 2 operations on the coronoid process. Long-term treatment results were followed-up in 31 patients during more than 6 months. On basis of analysis of treatment results the authors made a conclusion that the application of mini-screws in case of bone fr

The effects of forearm fatigue on baseball fastball pitching, with implications about elbow injury.

This study investigated the contribution of flexor muscles to the forearm through fatigue; therefore, the differences in forearm mechanisms on the pitching motion in fastball were analysed. Fifteen baseball pitchers were included in this study. Ultrasonographical examination of participants' ulnar nerve in the cubital tunnel with the elbow extended and at 45°, 90° and 120° of flexion was carried. A three-dimensional motion analysis system with 14 reflective markers attached on participants was used for motion data collection. The electromyography system was applied over the flexor carpi ulnari

Effectiveness of the lateral unilateral dynamic external fixator after elbow ligament injury.

BACKGROUND: The optimum management of ligamentous injuries of the elbow is not known. Use of dynamic external fixators has been advocated to stabilize the joint while maintaining motion, but there are no published data to corroborate their efficacy. The purpose of this study was to test the hypothesis that a laterally applied unilateral dynamic external fixator is capable of stabilizing and restoring normal kinematics to elbows with varying degrees of soft-tissue injury. METHODS: Six fresh-frozen cadaveric upper extremities, from donors who were an average of seventy-six years of age at the t

[The anatomic basis and the clinical treatment of the elbow joint stiffness after electric injury].

OBJECTIVE: To explore the optimal method for the treatment of the elbow joint stiffness accompanied with neural injury after electrical injury. METHODS: Thirty adult cadaver elbows were fixed and dissected to observe the attachments of the ligaments and the muscles around the joint, and its relationship with the major nerves. Ten patients with electric burns with stiffness of the elbow joint were treated by means of loosening the anterior and posterior fascicles of radial and ulnar collateral ligaments, at the same time down shifting the insertion of flexor muscles, replacing the ulnar nerve

Biomechanics of windmill softball pitching with implications about injury mechanisms at the shoulder and elbow.

Underhand pitching has received minimal attention in the sports medicine literature. This may be due to the perception that, compared with overhead pitching, the underhand motion creates less stress on the arm, which results in fewer injuries. The purpose of this study was to calculate kinematic and kinetic parameters for the pitching motion used in fast pitch softball. Eight female fast pitch softball pitchers were recorded with a four-camera system (200 Hz). The results indicated that high forces and torques were experienced at the shoulder and elbow during the delivery phase. Peak compressi
