Oral Health in Electroconvulsive Therapy: A Neglected Topic.

Psychiatric medications may have serious and untoward adverse effects such as blurred vision, restlessness, agranulocytosis, muscle rigidity, and tremors. When compared to medications, electroconvulsive therapy (ECT) is becoming a more acceptable treatment due to its efficacy, tolerability, and minimal adverse effect profile. Oral trauma can be an ECT-related adverse effect. We reviewed the published literature on oral health and dental protection in patients undergoing ECT, and found that there are deficits in all guidelines on dental protection during ECT. Dental assessment and treatment bef…

Microarray Analysis of Human Blood During Electroconvulsive Therapy.

INTRODUCTION: Electroconvulsive therapy (ECT) is currently regarded as a significant treatment option for intractable psychiatric disorders, such as catatonic schizophrenia or treatment-resistant depression; however, the underlying molecular mechanism for its therapeutic effect remains obscure. METHODS: Employing microarray analysis (Human Genome U133 Plus 2.0 Array; Affymetrix, United States) of cDNA derived from the peripheral blood of patients with catatonic schizophrenia (n = 5), we detected a significant change in 145 genes (0.68%) before and after modified ECT (mECT). Moreover, we perfo…

Electroconvulsive therapy for catatonia in juvenile neuropsychiatric lupus.

Neuropsychiatric manifestations are serious and frequent complications of systemic lupus erythematous (SLE). Catatonia is a neuropsychiatric … isolone, intravenous cyclophosphamide and rituximab. The patient responded to a combined therapy of electroconvulsive therapy and benzodiazepines. The present case suggests that although rarely reported, catatonia seen in the background of SLE should be promptly identified and treated to reduce the morbidity.

Electroconvulsive therapy in patients with deep brain stimulators.

The scope of functional neurosurgery is expanding, and deep brain stimulation (DBS) indications with comorbid depressive disorders that are challenging to manage, such as movement disorders, are common. There is limited research or clinical experience on the safety and efficacy of electroconvulsive therapy (ECT) in patients with DBS electrodes. We present a case illustrating the challenges when faced with ECT for treatment-resistant depression in the context of preexisting DBS electrodes, and establish a database to collect cases of ECT in DBS patients.

Electroconvulsive therapy, the placebo effect and informed consent.

Major depressive disorder is not only the most widespread mental disorder in the world, it is a disorder on the rise. In cases of particularly severe forms of depression, when all other treatment options have failed, the use of electroconvulsive therapy (ECT) is a recommended treatment option for patients. ECT has been in use in psychiatric practice for over 70 years and is now undergoing something of a restricted renaissance following a sharp decline in its use in the 1970s. Despite its success in treating severe depression there is continued debate as to the effectiveness of ECT: in some stu…

Retrograde amnesia after electroconvulsive therapy: a temporary effect?

OBJECTIVE: Although electroconvulsive therapy (ECT) is generally considered effective against depression, it remains controversial because of its association with retrograde memory loss. Here, we assessed memory after ECT in circumstances most likely to yield strong retrograde amnesia. METHOD: A cohort of patients undergoing ECT for major depression was tested before and after ECT, and again at 3-months follow-up. Included were 21 patients scheduled to undergo bilateral ECT for severe major depression and 135 controls matched for gender, age, education, and media consumption. Two memory tests…

[Maintenance electroconvulsive therapy and treatment of refractory schizophrenia].

BACKGROUND: Electroconvulsive therapy, a standard treatment in mood disorders, is sometimes also indicated in psychotic disorders, especially in the treatment of refractory schizophrenia. In this instance, maintenance electroconvulsive therapy (M-ECT) can also become a long-term treatment. This paper presents the effects of M-ECT in the treatment of refractory schizophrenia using a retrospective analysis. Previous works showed that electroconvulsive therapy is effective on catatonia, anxiety with somatisation, lack of compliance, opposition, delusions especially with hallucinations and persecu…

[Treatment of postictal (emergence) delirium after electroconvulsive therapy].

Most patients treated with electroconvulsive therapy (ECT) develop cognitive and motor aspects of delirium afterwards. These symptoms are often self-limiting, lasting 10 to 20 minutes. However, about 10% of these patients develop a state characterised by confusion, restlesness, repetitive movements, lack of response to commands and amnesia regarding the period. This state, called emergence delirium, usually has a short duration but may persist for hours. In the present case a 50-year-old woman had 18 ECT treatments for severe depression. She experienced excellent effects as regards the depress…

Diffuse T-wave inversions associated with electroconvulsive therapy.

Electroconvulsive therapy (ECT) can lead to ST depression and arrhythmias in patients with coronary artery disease, most likely secondary to increased myocardial oxygen demand. This report describes a case of ECT-associated, global T-wave inversions in a patient with chronic atrial fibrillation treated with digoxin. Laboratory evaluation and echocardiography were normal. These T-wave changes may result from increased sympathetic activity associated with ECT. Prospective studies suggest that this finding may be seen in up to 4% of patients undergoing ECT.

[Cotard's syndrome in depression and maintenance electroconvulsive therapy].

We describe the case of a 65-year-old woman with long-standing, atypical facial pain and fluctuating mental symptoms. A variety of psychiatric diagnoses were made over a period of nine years but no effective treatment was found. Ultimately, the patient was found to suffer from a psychotic depression indicating Cotard's syndrome. Two series of ECT were then given with temporary short remissions. Subsequently, continuation ECT was introduced. To our knowledge, continuation ECT in Cotard's syndrome is rarely, if ever, described in the literature.

[Electroconvulsive therapy in the Czech Republic 1981-1989].

The authors processed statistical data on the application of electroconvulsive treatment in 1981-1989 in all in-patient psychiatric departments in the Czech Republic. During this period a slight decline of shocked subjects was recorded. Among shocked patients there is a permanent, higher relative ratio of women. Approximately every 18th discharged or deceased patient underwent electroconvulsive treatment. Psychiatric hospital departments use electroconvulsive treatment to a very different extent. Most frequently patients with the diagnosis of schizophrenic psychosis (43%), affective psychosis

Medication resistance and clinical response to electroconvulsive therapy.

We examined the extent to which medication resistance during an episode of major depression was related to short-term clinical response to bilateral electroconvulsive therapy (ECT). Strength of pharmacological treatment trials was rated in 53 patients who met Research Diagnostic Criteria for major depressive disorder and were subsequently treated with ECT. Patients who had failed to respond to adequate pre-ECT pharmacotherapy were substantially less likely to respond to ECT than patients who had not received adequate pharmacological trials before ECT. Therefore, medication resistance had predi…

Electroconvulsive therapy in one child and three adolescents.

Four patients aged 12 to 15 years who received ECT are reported. Despite the presentation of signs and symptoms consistent with positive response to electroconvulsive therapy (ECT) in adults, only one of the four patients responded favorably, and three patients had seizures lasting longer than 4 minutes. The patient who did respond to ECT did not experience a prolonged seizure, and he was also the only prepubescent patient. The authors present a tentative hypothesis that correlates youth with prolonged seizures and refractoriness to ECT.

Patients' attitudes and attributions to electroconvulsive shock therapy.

An attributional analysis of ECT as a placebo was investigated by comparing the attitudes of patients on two psychiatric wards. It was hypothesized that on the ward on which ECT was administered more frequently, general attitudes toward the treatment would be more favorable and more patients would express the opinion that ECT had worked well for them personally. The results, which supported the predictions, indicate that the operation of placebo effects in connection with ECT may account for differential treatment results more adequately than explanations based on physiological models. The fin

Seasonal patterns of suicide, depression and electroconvulsive therapy.

This study examined the hypothesis that there are seasonal variations for suicide, hospital admissions for depressive illnesses and electroconvulsive therapy and that these are inter-related. It was found that the seasonal variations did coincide, with peaks occurring in the Ontario spring and autumn. The importance of depression as a cause of suicide and the effect of electroconvulsive therapy upon the suicide rate are discussed.
