Insights and Constraints: Understanding the Boundaries in Investigating Otosclerosis and Endolymphatic Hydrops Interaction.

Otosclerosis and endolymphatic hydrops (EH) are distinct inner ear pathologies that may coexist within the same temporal bone, creating diagnostic and therapeutic challenges. Their interaction remains controversial, with proposed mechanisms suggesting that otosclerotic involvement of the vestibular aqueduct or cochlear endosteum may obstruct endolymphatic flow, leading to secondary hydrops. This review summarizes current evidence on the coexistence, pathophysiology, diagnostic tools, and clinical implications of otosclerosis-associated EH. Histopathologic studies have demonstrated EH adjacent …

Improvement of vertigo symptoms and acoustic power absorbance in cases with endolymphatic hydrops.

OBJECTIVE: The pathophysiology and symptoms underlying Meniere's disease (MD) manifest as endolymphatic hydrops (EH), potentially impacting acoustic power absorbance in vestibular EH. The longitudinal effects of middle ear pressure therapy (MEPT) and conservative therapies for EH by magnetic resonance imaging (MRI) and on acoustic power absorbance on wideband acoustic immittance (WAI) were evaluated, and their changes were compared with clinical symptoms. METHODS: Eleven patients with definite MD or delayed endolymphatic hydrops (DEH), resistant to conservative therapies and who continued MEP…

Protective effect of anakinra on audiovestibular function in a murine model of endolymphatic hydrops.

INTRODUCTION: Ménière's disease (MD), a common disease in the inner ear, is characterized by an increase in endolymph in the cochlear duct and vestibular labyrinth. The pathophysiology of the condition appears to be the immune response. Studies have shown that basal levels of the IL-1β increased in some MD patients. METHODS: Here, we used a murine model of endolymphatic hydrops (EH) to study the effect of anakinra on auditory and vestibular function. Mice were intraperitoneal injected with anakinra or saline before LPS by postauricular injection. Weight and disease severity were measured, his…

Temporal changes in endolymphatic hydrops on MRI with or without intervention: A systematic review.

OBJECTIVE: The pathophysiology of Meniere's Disease (MD) involves endolymphatic hydrops (ELH) of the inner ear. Magnetic Resonance Imaging (MRI) has been shown to detect ELH, but changes in ELH have been poorly described using this modality. Our objective was to review MRI-measured changes in ELH over time and after medical and/or surgical intervention in patients with MD. We secondarily aim to associate changes in ELH with changes in MD symptomatology. DATABASES REVIEWED: Medline, Web of Science, and Embase databases. METHODS: A systematic review of articles was performed to identify studie…

MR imaging of endolymphatic hydrops in Ménière's disease: not all that glitters is gold.

Ménière's disease (MD) is a chronic condition characterised by fluctuating hearing loss, intermittent vertigo, tinnitus and aural fullness. Its anatomical and pathological counterpart is represented by endolymphatic hydrops (EH). Recent development and progress in magnetic resonance (MR) imaging techniques has enabled visualisation of EH in living human subjects using a 3 Tesla (T) scanner and gadolinium-based contrast-agent (GBCA) via intravenous (IV) or intra-tympanic (IT) administration. Data emerging from the literature about MR imaging of EH in MD patients are limited, and we therefore re…

Progression of endolymphatic hydrops in Ménière's disease as evaluated by magnetic resonance imaging.

OBJECTIVE: To evaluate the presence and the degree of endolymphatic hydrops (EHs) in patients with unilateral Ménière's disease (MD), as a function of duration of the disease, estimated using a 3-dimensional fluid-attenuated inversion recovery sequence in a 3-Tesla magnetic resonance imaging unit, after intratympanic gadolinium administration. PATIENTS: A total of 32 patients (21 male and 11 female subjects, aged 25-78 yr; median, 56 yr) participated in the investigation. The duration of the disease ranged from 2 months to 10 years (median, 3 yr), with a prevalence of vertigo spells in the la…

Measurement of peripheral vestibular blood flow in a gerbil model of endolymphatic hydrops.

Presently, many investigators believe that the dysfunction of microcirculatory mechanisms may be responsible for vestibular symptoms in Meniere's disease. This study, using intravital microscopy (IVM), a technique that provides in vivo microcirculatory measures, was designed to determine whether impaired vestibular blood flow exists in endolymphatic hydrops. Hydrops was induced in the gerbil model by obliteration of the vestibular aqueduct and was confirmed histologically after IVM. Posthydrops gerbil subjects at 8 weeks as well as control animals were prepared for IVM. With a customized intra…

Mumps labyrinthitis, endolymphatic hydrops and sudden deafness in succession in the same ear.

Acute sensorineural hearing loss, mostly unilateral and reversible, is a well-known complication to mumps. Secondary endolymphatic hydrops, Ménière's syndrome, has rarely been associated with a previous mumps infection. This paper presents the case report of a woman who experienced unilateral hearing loss, vestibular symptoms and a caloric depression on the same ear during mumps. The symptoms and findings were reversible. Twelve years later she developed Ménière's symptoms in the same ear. This continued for 2 years after which she suddenly had a sensorineural hearing loss. This was localized …

[Measurement of Ca2+ concentration and endocochlear potential in experimental endolymphatic hydrops in vivo].

This study was based on observations on 14 normal guinea pigs and 17 guinea pigs in which the left endolymphatic duct and sac had been surgically obliterated to induce endolymphatic hydrops. Three groups of hydropic animals were tested 1, 2 and 3 months after surgery. The Ca2+ concentration and endocochlear potential (EP) were recorded by means of double-barrelled ion selective micro-electrode in the third turn of the cochlea. The results indicated that a decrease in EP and an increase in Ca2+ in hydropic cochlea, a negative correlation was present. In the control ear, the values of Ca2+ and E…

Endolymphatic hydrops in animal experiments. A confirmation of mechanical and immunological methods of inducement.

The etiology underlying endolymphatic hydrops is scarcely understood. It is generally accepted, however, that the primary cause of endolymphatic hydrops is the malfunction and malabsorption of the endolymphatic sac. This has already been proven by animal experiments as well as by histopathological studies of human temporal bone. In this study, we attempted to induce endolymphatic hydrops by utilizing both mechanical and immunological methods. An obliterative procedure was performed on the endolymphatic sac and duct of 14 guinea pigs, 12 of which showed extensive endolymphatic hydrops after a v…

Magnetic resonance-based volumetric measurement of the endolymphatic space in patients with Meniere's disease and other endolymphatic hydrops-related diseases.

OBJECTIVE: To employ magnetic resonance imaging (MRI) to measure the volume of the inner ear endolymphatic space (ELS) in patients with acut … 1 for uMD vs. all other groups). CONCLUSION: The cochlear ELS volume of patients with MD and other endolymphatic hydrops-related diseases differed from that of CS. Our results suggest that ALHL may not be caused by endolymphatic hydrops. We confirmed the presence of extended ELS in patients with SD.

Magnetic resonance imaging of endolymphatic hydrops in patients with unilateral Meniere's disease: volume ratio and distribution rate of the endolymphatic space.

BACKGROUND: Endolymphatic hydrops (ELH), which is a pathological feature of Meniere's disease (MD), is characterized by an extended endolymphatic space (ELS). AIMS/OBJECTIVES: We aimed to perform a quantitative volumetric analysis of inner-ear ELH in patients with unilateral MD (uMD). MATERIAL AND METHODS: This study included 97 patients with definite uMD and 49 control subjects (CS). The ELS/total fluid space (TFS) volume ratio (%) and the distribution rate of the inner-ear components in the ELS (%) were measured using 3D magnetic resonance imaging and compared between patients with uMD and…

Compressed Sensing Technology Accelerated 3D-FLAIR MRI Sequence for Endolymphatic Hydrops at 3T.

BACKGROUND AND PURPOSE: 3D-FLAIR sequence has an important contribution to the display of endolymphatic hydrops (EH) in Meniere disease (MD), but its clinical application is limited because of the long acquisition time. We investigated whether 3D-FLAIR combined with compressed sensing (CS) technology (3D-FLAIR-CS) can shorten the scan time while maintaining the image quality and diagnostic efficiency for EH. MATERIALS AND METHODS: This prospective study included 50 patients with unilateral definite MD who underwent 3T MR imaging 4 hours after gadolinium injection using traditional 3D-FLAIR (1…

New non-contrast MRI of endolymphatic hydrops in Ménière's disease considering inversion time.

OBJECTIVES: Three-tesla MRI with gadolinium-based contrast agents is important in diagnosing Ménière's disease. However, contrast agents cannot be used in some patients. By using the compositional difference between the inner ear endolymph and perilymph, we performed basic and clinical research focused on potassium ions and protein to find the optimal parameters for visualizing endolymphatic hydrops on MRI without contrast. We then examined the relationship between severity stage and visualization rate of endolymphatic hydrops. METHODS: In phantom experiments simulating the endolymph and peri…

Three-dimensional visualization of endolymphatic hydrops after intravenous administration of single-dose gadodiamide.

Endolymphatic hydrops can be visualized with high contrast-to-noise ratio even after intravenous injection of single-dose gadolinium-based contrast material (IV-SD-GBCM) using HYDROPS-Mi2 images. We applied 3-dimensional rendering software to process HYDROPS-Mi2 images of 15 ears with and without suspected Ménière's disease and separately visualized the volumes of endo- and perilymph in patients with Ménière's disease even after IV-SD-GBCM. Such dimensional visualization will aid understanding of the pathophysiology of Ménière's disease.
