OBJECTIVES: Pancreatic steatosis or fatty pancreas refers to the fat accumulation in the pancreas, which can lead to inflammation and fibrosis, β-cell dysfunction, fibrosis, and, possibly, pancreatic cancer. Endoscopic ultrasound-guided fine needle aspiration biopsy (EUS-FNA) is currently performed on a routine basis at many endoscopic centers and it is evident that this procedure has a major impact on the therapeutic management of patients by obtaining a definite tissue diagnosis from lesions outlined by endosonography. Endoscopic ultrasonography (EUS) is a useful modality for imaging of the blood vessels of the mediastinum and abdomen. The aorta acts as an important home base during EUS imaging. The aorta and its branches are accessible by standard angiographic methods, but endosonography also provides a unique opportunity to evaluate the aorta and its branches. This article describes the techniques of imaging of different part of the aorta by EUS.

Endosonography is now an effective tool for the assessment of anorectal pathologies. We present a case of rectal prolapse in a patient with progressive systemic sclerosis, with low resting anal pressure, no rectoanal inhibitory reflex in manometry, and a thin, heterogeneous, difficult to delineate, internal sphincter on endoanal ultrasound. We also provide a review of the literature on anorectal involvement in progressive systemic sclerosis.

Gastric duplication cyst is a very rare gastrointestinal tract malformation that accounts for 2%-4% of alimentary tract duplications. Obstetric trauma is by far the commonest cause of anal incontinence in women. Denervation and reinnervation of the pelvic floor and anal sphincter following vaginal delivery has been previously demonstrated. The advent of anal endosonography, however, has enabled the identification of occult anal sphincter defects following vaginal delivery. It is now possible to identify risk factors and change obstetric practice accordingly so as to minimize anorectal morbidity.

BACKGROUNDS/AIMS: Distinguishing gastric ectopic pancreas (GEP) from malignant tumors is relatively difficult. This study evaluated the endosonography findings of pathologically proven GEP.

A patient with cystic dystrophy in heterotopic pancreatic tissue, localized in the second duodenum, is described. Diagnosis was assessed by computed tomography, endosonography and magnetic nuclear resonance (MNR). Medical treatment with octreotid during three weeks was effective but duodenal stenosis persisted. Finally, pylorus preserving pancreaticoduodenectomy was performed. Microscopy examination confirmed the diagnosis and the efficiency of medical treatment. A review of the literature about cystic dystrophy is done.

The primary staging of an oesophageal cancer can be difficult, if accompanied by sarcoidosis. In these patients endosonography, CT and PET may not be sufficient for staging purposes concerning lymph node and distant metastases. In these special cases operative biopsies of enlarged lymph nodes and unclear pulmonary nodules have to be obtained. In connection with the radiographic examinations the histopathological results of the biopsies contribute to further precise staging and help to decide on a curative versus a palliative therapy concept.

The method of choice to detect tumors of the papilla of Vater is endoscopic diagnosis. However, no definite decision on the dignity can be made through the macroscopic appearance itself. Only the combination of various diagnostic methods (side view duodenoscopy with biopsy, endosonography, ERCP, IDUS) allows a reliable statement on the local growth of tumors and the dignity. If there are no signs for local infiltration, histological indications for a carcinoma or distant metastasis, the endoscopic resection in specialized centers is recommended.

The new classification of primary lymphomas of the stomach according to the MALT concept is very promising in therapy planning and prognostic estimation. This new classification sets strict requirements for imaging diagnostics. Besides endosonography which allows the determination of the infiltration depth in stages E I1 and E I2 in an excellent manner, computed tomography and, possibly, magnetic resonance tomography are indispensible for evaluation of higher stages and of the resectability of a lymphoma. Further MRI examinations are recommended.

Endosonography offers two major advantages in the pretherapeutic examinations of a histologically verified carcinoma of the uterus. OBJECTIVE: To investigate sexual abuse as a cause of faecal incontinence.
PATIENTS: Seven patients (two men and five women) with a history of sexual abuse, including anal penetration, and faecal incontinence.
METHODS: Anorectal physiological techniques and anal endosonography were used for patient assessment.
RESULTS: All seven patients had evidence of internal sphincter disruption and three had additional external anal sphincter dysfunction.
CONCLUSION: Unwanted anal penetration can cause permanent structural anal sphincter damage.
