GRAPPA Debate: Be it Resolved That Clinical Enthesitis Indices Do Not Reflect True Enthesitis and Hence Should Be Discontinued.

Enthesitis is increasingly recognized as a key manifestation of psoriatic disease (PsD). However, how best to assess enthesitis remains a point of discussion due to limitations of the existing assessment tools, including their inability to differentiate between inflammatory and noninflammatory enthesitis as well as a high placebo response in clinical trials. At the Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) 2024 annual meeting, a debate was held to address whether traditional clinical enthesitis indices should be discontinued for PsD. Dr. Sibel Aydin advoca

Tendon Cells Root Into (Instead of Attach to) Humeral Bone Head via Fibrocartilage-Enthesis.

Large joints are composed of two closely linked cartilages: articular cartilage (AC; rich in type II collagen, a well-studied tissue) and fibrocartilaginous enthesis (FE; rich in type I collagen, common disorder sites of enthesopathy and sporting injuries, although receiving little attention). For many years, both cartilages were thought to be formed by chondrocytes, whereas tendon, which attaches to the humeral bone head, is primarily considered as a completely different connective tissue. In this study, we raised an unconventional hypothesis: tendon cells directly form FE via cell transdiffe…

The use of ultrasound to detect enthesitis as a potential guide for intervention in patients with psoriasis at risk of psoriatic arthritis: a systematic review.

INTRODUCTION: Psoriatic arthritis is estimated to develop in 2% of patients with psoriasis per year and can result in significant morbidity. Early diagnosis and treatment of psoriatic arthritis are imperative to prevent irreversible arthritic joint damage. Dermatologists play a key role in identifying patients who are at risk for or with early signs of psoriatic arthritis. Subclinical enthesopathy may be a risk factor for psoriatic arthritis or an early sign of the disease and can be detected using ultrasound. METHODS: In this systematic review, we determined the prevalence of ultrasound-diag…

Pathways driving tendinopathy and enthesitis: siblings or distant cousins in musculoskeletal medicine?

Tendinopathy and enthesitis share clinical, anatomical, and molecular parallels. However, their relationship is complex, presenting challenges in diagnosis and treatment. The biomechanics underlying these pathologies, together with relative immune and stromal contributions to pathology, are characterised by crucial comparative elements. However, methodologies used to study enthesitis and tendinopathy have been divergent, which could account for discrepancies in how these conditions are perceived and treated. In this Review, we summarise key clinical parallels between these two common presentat

Targeted systemic therapies for psoriatic arthritis: a systematic review and comparative synthesis of short-term articular, dermatological, enthesitis and dactylitis outcomes.

INTRODUCTION: Randomised controlled trials (RCTs) have compared biological and targeted systemic disease-modifying antirheumatic drugs (DMARDS) against placebo in psoriatic arthritis (PsA); few have compared them head to head. OBJECTIVES: To compare the efficacy and safety of all evaluated DMARDs for active PsA, with a special focus on biological DMARDs (bDMARDs) licensed for PsA or psoriasis. METHODS: A systematic review identified RCTs and Bayesian network meta-analysis (NMA) compared treatments on efficacy (American College of Rheumatology (ACR) response, Psoriasis Area and Severity Index

Improved diagnostic performance of CASPAR criteria with integration of ultrasound.

BACKGROUND: The difficulty in determining synovitis, tenosynovitis, or enthesitis by physical examination (PE) has limited the diagnostic capability of CASPAR for psoriatic arthritis (PsA). Therefore, we aimed to evaluate the diagnostic utility of CASPAR with the integration of ultrasound (US). METHODS: Patients with a hint of PsA were enrolled. Besides routine PE for tender or swollen joints, enthesitis, and dactylitis, US was performed to evaluate peripheral joints, entheses, and tendons. The additional value of the US to the CASPAR criteria was analyzed. RESULTS: A total of 326 consecutiv

Hidden in plain sight: Is there a crucial role for enthesitis assessment in the treatment and monitoring of axial spondyloarthritis?

OBJECTIVE: To review the evidence surrounding the pathophysiology of enthesitis in axial spondyloarthritis (axSpA), its prevalence and contribution to the overall disease burden, and response to treatment at axial and peripheral sites. METHODS: Literature searches of the Cochrane Library, PubMed, and Embase / Medline using the terms "enthesitis", "enthesopathy", "spondyloarthritis", "axial spondyloarthritis", and "ankylosing spondylitis" were conducted. Publications mentioning enthesitis or enthesopathy in the context of pathophysiology, diagnosis, or treatment were included. RESULTS: Enthes…

Focal Synovial Inflammation Versus Enthesitis Theory in Distal Psoriatic Arthritis: A Pioneer Histopathologic Study.

The aim of this analysis was to re-examine the classical concept of distal interphalangeal joint (DIP) psoriatic arthritis (PsA) as an entheseal-driven disease. Two cadaveric fingers with severe psoriatic arthritis were analyzed. Our results demonstrate that inflammation of DIP PsA is multifocal without interconnection between entheses and articular cartilage of the DIP. We found a clear association between synovitis and focal loss of articular cartilage at the head of the intermediate phalanx. By contrast, the articular cartilage adjacent to the zone of severe enthesitis did not show notable

Single-cell RNA-sequencing analysis of immune and mesenchymal cell crosstalk in the developing enthesis.

Autoimmunity underlies many painful disorders, such as enthesopathies, which localize to the enthesis. From infiltration of the synovium and axial skeleton by B cells, to disturbances in the ratio of M1/M2 enthesis macrophages, to CD8 + T cell mediated inflammation, autoimmune dysregulation is becoming increasingly well characterized in enthesopathies. Tissue resident B cells, macrophages, neutrophils, and T cells have also been localized in healthy human entheses. However, the potential developmental origins, presence, and role of immune cells (ICs) in enthesis development is not known. Here,

Clinico-radiological correlation of enthesitis in seronegative spondyloarthropathies (SNSA).

The goal of this work was to evaluate clinico-radiological correlation of enthesitis in SNSA patients, selected for presenting at least one radiological enthesopathy. Out of 50 patients with SNSA, 40 were selected for having had at least one radiological enthesitis. In a cross-sectional study, 32 males and 8 females, whose mean age was 40.4 years and mean disease duration 13 years, were evaluated. Nineteen patients had ankylosing spondylitis, 15 psoriatic arthritis and 6 Reiter's syndrome. Sites evaluated were pelvis and lower limbs. Radiological enthesopathies were identified by the presence …

Spondylodiscitis and Achilles tendonitis due to gout.

The patient, a 62-year-old man with a 3-year history of hyperuricemia, presented with severe neck pain, Achilles enthesopathy and polyarthralgia. He consumed alcohol heavily. The biochemical profile was normal except for elevated levels of CRP (3.6 mg/dl; normal < 0.3), uric acid (UA) (10.9 mg/dl; normal 2.5-7.5) and creatinine (1.7 mg/dl; normal 0.5-1.0). Bone scintigraphy showed polyarthritis at the right elbow, wrist and bilateral first MTP joints. Notably, bone scintigraphy with computed tomography also revealed spondylodiscitis of C5-C6, which was confirmed by MRI, and left Achilles tendo…

Rheumatic diseases that can be confused with work-related upper extremity disorders.

Rheumatic illnesses are a common cause for musculoskeletal complaints in the general population. All ages can be affected, including people in the prime of their working years. Secondary problems, such as entrapment neuropathies, enthesopathies, and Raynaud's syndrome, can be associated with various inflammatory arthritides. A detailed history and physical are the most important tools in screening for potential inflammatory disease in workers with upper extremity complaints.

Burosumab Treatment for Autosomal Recessive Hypophosphatemic Rickets Type 1 (ARHR1).

CONTEXT: Autosomal recessive hypophosphatemic rickets (ARHR) are rare, heritable renal phosphate-wasting disorders that arise from overexpression of the bone-derived phosphaturic hormone fibroblast growth factor 23 (FGF23) leading to impaired bone mineralization (rickets and osteomalacia). Inactivating mutations of Dentin matrix protein 1 (DMP1) give rise to ARHR type 1 (ARHR1). Short stature, prominent bowing of the legs, fractures/pseudofractures, and severe enthesopathy are prominent in this patient population. Traditionally, treatment consists of oral phosphate replacement and the addition…

[Vertebral and costal stippling. A pitfall in scintigraphy].

Bone scintigraphy is largely used for diagnosis and surveillance of osteoarticular diseases. Interpretation of fortuitous increased isotope accumulation elsewhere on the skeleton may be difficult. We report a man with a peripheral seronegative inflammatory arthropathy, whose bone scintigraphy disclosed stippled ribs and vertebral increased isotope uptake, corresponding to rib enthesopathy and vertebral hyperostosis. Better knowledge of these unusual pictures should prevent further unnecessary investigations.

Psoriatic arthritis: Ultrasound peculiarities with particular emphasis on enthesitis.

The prevalence of psoriatic arthritis among patients with psoriasis has a marked variability with ethnic and geographic variations. Inflammatory changes associated with psoriatic arthritis include bone erosion, tenosynovitis, and synovial hypertrophy, but enthesitis is considered the hallmark. Both X-ray and magnetic resonance imaging (MRI) are usefull in the diagnosis of psoriatic arthritis, but ultrasonography is the best imaging modality to assess entheses. Ultrasound findings of enthesitis include a loss of the regular fibrillar architecture, hypoechoic thickening, hypervascularization of

Enthesitis in Psoriatic Arthritis, the Sonographic Perspective.

PURPOSE OF REVIEW: To provide an overview of the ultrasound (US) studies focusing on enthesitis in psoriatic arthritis (PsA). RECENT FINDINGS: Last-generation US equipment has demonstrated the ability to detect subtle morphostructural and vascular abnormalities at entheseal level. US is able to identify pathologic changes in both "classical" (i.e., the site of attachment of tendons, ligaments, and joint capsules into the bone) and "functional" entheses (i.e., anatomical regions where tendons or ligaments wrap around bony pulleys). US has the potential to be the first-line method in the assess

Spondyloarthritis: not only enthesitis.

Spondyloarthritis represents a heterogeneous group of inflammatory diseases that share common genetic, clinical and radiological features. Those diseases are characterised by inflammation in the spine and in the peripheral joints. Enthesitis is considered a pathological hallmark of the spondyloarthritis group of conditions but there are also many other relevant clinical manifestations. The aim of this article is to present an overview on articular and extra-articular manifestations and comorbidities associated with spondyloarthritis.
