Comparative effects of adjuvant cimetidine and omeprazole during pancreatic enzyme replacement therapy.

In a double-blind, randomized crossover study, the hypotheses were tested that more powerful inhibition of gastric acid secretion by adjuvant omeprazole further improves the efficacy of pancreatic enzyme replacement therapy compared to adjuvant cimetidine and that excluding the influence of pH-related factors, by virtually complete inhibition of gastric acid secretion with 60 mg omeprazole daily, does not lead to total elimination of steatorrhea. During both adjuvant cimetidine and omeprazole treatment, fecal fat excretion was significantly lower compared to pancreatin monotherapy (P < 0.01).

MRI of multiple platyspondyly in Gaucher disease: response to enzyme replacement therapy.

Vertebra plana and multiple platyspondyly are complications of aggressive Gaucher disease. Magnetic resonance imaging readily identifies vertebra plana and secondary spinal cord compression. We present a case of a 2-year-old boy with clinically aggressive Type 1 Gaucher disease in whom MRI showed partial reconstitution of the height of a collapsed lumbar vertebral body after 16 months of enzyme replacement therapy.

Current status of the immunogenicity of enzyme replacement therapy in fabry disease.

In patients with Fabry disease (FD), treatment with enzyme replacement therapy (ERT), may trigger the formation of anti-drug antibodies (ADAs). The consequences of this immune reaction range from the transient appearance of clinically insignificant ADAs to the generation of neutralizing antibodies that negate the clinical benefit of the biotherapeutic agent, lead to side effects (such as injection site reactions), and even cause severe, life-threatening symptoms. Many factors may influence the immunogenicity of these therapeutic proteins. Currently, there are three commercially available long-

Enzymes approved for human therapy: indications, mechanisms and adverse effects.

Research and drug developments fostered under orphan drug product development programs have greatly assisted the introduction of efficient and safe enzyme-based therapies for a range of rare disorders. The introduction and regulatory approval of 20 different recombinant enzymes has enabled, often for the first time, effective enzyme-replacement therapy for some lysosomal storage disorders, including Gaucher (imiglucerase, taliglucerase, and velaglucerase), Fabry (agalsidase alfa and beta), and Pompe (alglucosidase alfa) diseases and mucopolysaccharidoses I (laronidase), II (idursulfase), IVA (

Aerobic training as an adjunctive therapy to enzyme replacement in Pompe disease.

BACKGROUND: Aerobic exercise may be used in conjunction with enzyme replacement therapy (ERT) to attenuate cardiovascular deconditioning, skeletal muscle wasting, and loss of motor function in Pompe disease (glycogen storage disease type II; GSDII), but the effects on lysosomal glycogen content and macroautophagy have not been defined to date. PURPOSE: The main objectives of this study were to determine if acute aerobic exercise enhances 24-h uptake of recombinant human enzyme (rhGAA; Myozyme® [aim 1]) and if endurance training improves disease pathology when combined with ERT [aim 2] in Pomp

[A retrospective study on enzyme replacement therapy in patients with Gaucher disease].

OBJECTIVE: Gaucher disease is the most common lysosomal storage disorder. A deficiency of beta-glucocerebrosidase causes accumulation of the glucocerebroside in macrophages throughout the body. This study summarizes the effects of enzyme replacement therapy (ERT) with Imiglucerase in children with Gaucher disease. METHODS: Data from 72 patients (46 were male and 26 female, age ranged from 16 months to 22 years, median 8 years and 8 months) who were treated in the hospital between May 1999 and October 2005 were collected and analyzed, 57 of the patients had type 1 Gaucher disease, 2 patients w

Enzyme replacement therapy--an experiment of nature in a chimeric mannosidosis calf.

Enzyme replacement therapy was studied in a chimeric mannosidosis calf which had received a natural transplacental transplant of normal lymphocytes from its co-twin. There was considerable reduction in the pathology of certain organs and in the amount of storage oligosaccharides, but the clinical course of this neurologic disease was not significantly altered. It was postulated that if this disease had been purely visceral the transplant would have been relatively effective.

AAV9-mediated GAA gene therapy following enzyme replacement therapy discontinuation in children with infantile-onset Pompe disease.

BACKGROUND: While our previous research showed that a single infusion of recombinant AAV9-mediated GAA gene therapy enabled infants (<1 year) with infantile-onset Pompe disease (IOPD) and minimal prior enzyme replacement therapy (ERT) to remain ventilator- and ERT-free for at least one year, it was unknown whether the same benefits could be sustained in older children with IOPD with prolonged, high-dose ERT exposure after treatment cessation. METHODS: In this investigator-initiated, single-arm trial (ChiCTR2200065664), six children with IOPD aged 12.5-50.8 months-all having previously receive

Anti-TNF-alpha therapy enhances the effects of enzyme replacement therapy in rats with mucopolysaccharidosis type VI.

BACKGROUND: Although enzyme replacement therapy (ERT) is available for several lysosomal storage disorders, the benefit of this treatment to the skeletal system is very limited. Our previous work has shown the importance of the Toll-like receptor 4/TNF-alpha inflammatory pathway in the skeletal pathology of the mucopolysaccharidoses (MPS), and we therefore undertook a study to examine the additive benefit of combining anti-TNF-alpha therapy with ERT in a rat model of MPS type VI. METHODOLOGY/PRINCIPAL FINDINGS: MPS VI rats were treated for 8 months with Naglazyme® (recombinant human N-acetyl-

Pancreatic enzyme replacement therapy for exocrine pancreatic insufficiency-Insights into prescribing practices.

INTRODUCTION: Exocrine pancreatic insufficiency (EPI) can affect individuals with a wide range of diseases. Despite the publication of multiple international guidelines, the prescribing practices vary across the globe. The present study was done with an aim to assess how doctors in India evaluate and manage EPI, focusing on usage of pancreatic exocrine replacement therapy (PERT) and barriers to its use. METHODS: This was a cross-sectional questionnaire-based survey using Google forms. Doctors practicing gastroenterology were invited using email, social media networks to participate voluntaril

Enzyme replacement therapy during pregnancy and breastfeeding in late-onset Pompe disease.

BACKGROUND: Pompe disease is an autosomal recessively inherited lysosomal storage disorder, caused by enzyme deficiency of acid alpha-glucosidase (GAA). This deficiency leads to the accumulation of glycogen in lysosomes and subsequent muscle dysfunction, thus affecting heart and skeletal muscle. Since 2006, enzyme replacement therapy (ERT) is available. The use of ERT during pregnancy and lactation remains a topic of concern due to limited data on the safety of ERT during these periods. Despite this, some previous case reports suggest that ERT is safe during pregnancy and lactation. CASE PRES

Nebulized and intravenous enzyme replacement therapy in mice with mucopolysaccharidosis type II.

Mucopolysaccharidosis Type II is a hereditary lysosomal storage disease characterized by deficiency in the enzyme iduronate 2-sulfatase (IDS). IDS is critical in the breakdown of sulfated glycosaminoglycans and its deficiency leads to an accumulation of these compounds across various tissue types resulting in multisystemic dysfunction. Intravenous administration of recombinant IDS (idursulfase) substantially improves patients' quality and length of life. However, recombinant IDS delivered intravenously is sequestered in the liver and respiratory failure remains as the leading cause of death fo

Targeting Neurological Aspects of Mucopolysaccharidosis Type II: Enzyme Replacement Therapy and Beyond.

Mucopolysaccharidosis type II (MPS II) is a rare, pediatric, neurometabolic disorder due to the lack of activity of the lysosomal hydrolase iduronate 2-sulfatase (IDS), normally degrading heparan sulfate and dermatan sulfate within cell lysosomes. The deficit of activity is caused by mutations affecting the IDS gene, leading to the pathological accumulation of both glycosaminoglycans in the lysosomal compartment and in the extracellular matrix of most body districts. Although a continuum of clinical phenotypes is described, two main forms are commonly recognized-attenuated and severe-the latte
