Fixation with a single screw for slipped capital femoral epiphysis.

The effectiveness of a single 6.5 or seven-millimeter-diameter screw for the promotion of premature physeal closure and the provision of stability of a slipped capital femoral epiphysis was investigated. Physeal fusion was demonstrated in forty-nine (92 per cent) of fifty-three hips after fixation with a single screw. Premature fusion of the involved physis, compared with the uninvolved, contralateral physis, was documented (p less than 0.001). Epiphyseal stability, as measured by the lateral head-shaft angle, was maintained in all except one hip. An analysis of twenty-nine hips for which ther

Wiberg's center-edge angle in patients with slipped capital femoral epiphysis.

Wiberg's center-edge (CE) angle was measured in 104 patients with slipped capital femoral epiphysis (SCFE) to determine whether it correlates with a deeper acetabulum. Initially, the radiographs of 45 pre- and mild slip patients (group I) were measured using the contour of the femoral head to determine the head center (point C) as described by Wiberg's original article and, alternatively, using the acetabular line (Visser's method). In addition, the radiographs of 59 patients with moderate and severe slip (group II) were measured in both ways. The measurements were not statistically different

Slipped capital femoral epiphysis associated with renal failure osteodystrophy.

A retrospective institutional and literature review of 31 children (20 boys and 11 girls) with 58 renal failure-associated slipped capital femoral epiphyses (RFASCFEs) was performed. The average age at diagnosis of renal failure was 8.9 years, and for RFASCFE 11.5 years, with a linear correlation between these two ages. All patients had secondary hyperparathyroidism at diagnosis of the RFASCFE. The median height and weight were below the 5th percentile. The RFASCFEs were stable and bilateral in 95% of the children. Slip magnitude was mild in 53%, moderate in 8%, and severe in 39%. Avascular ne

Pretreatment bone scan in SCFE: a predictor of ischemia and avascular necrosis.

Sixty-two consecutive patients with 73 slipped capital femoral epiphyses (SCFEs) underwent pretreatment planar technetium bone scan to identify the presence of ischemia of the femoral head. The relationship of pretreatment ischemia and the development of avascular necrosis (AVN) was evaluated after a minimum follow-up of 12 months. All patients were treated with spica casting or pinning across the physis. No forceful manipulation was performed. None of the 63 stable SCFEs demonstrated ischemia by bone scan, and none developed AVN. Of the 10 unstable SCFEs, six demonstrated ischemia by bone sca

Congenital dislocation of the knee.

Congenital dislocation of the knee is a very rare condition, the incidence in Scandinavia hitherto not being known with certainty. Thirteen patients treated during the years 1960 to 1983 with 19 affected knees were reviewed and followed up. Findings of muscular imbalances in nine of these cases and of spinal abnormalities in four cases strongly indicate neuromuscular imbalance as an aetiological factor. The minimum incidence was estimated to be 0.017 per mille or approximately 1 per cent of the incidence of congenital dislocation of the hip. Recommendations for treatment are that manipulation

Metabolic bone disease in pseudohypoparathyroidism: radiologic features.

We reviewed the radiographs of six patients with pseudohypoparathyroidism (PHP) manifested by variable renal and skeletal resistance to parathyroid hormone (PTH). Features compatible with hyperparathyroid bone disease, osteomalacia, and rickets were observed. Skeletal changes of hyperparathyroidism included (a) subperiosteal bone resorption, (b) radiolucent lesions, caused by either brown tumors or bone cysts, (c) slipped capital femoral epiphyses, (d) focal areas of osteosclerosis, (e) periosteal neostosis, and (f) osteopenia with reduced cortical and trabecular bone volume. Histologic bone f

Prophylactic pinning in slipped upper femoral epiphysis. Prevention of complications.

We report the complications of prophylactic pinning of slipped upper femoral epiphysis with Crawford Adams pins in 95 cases. Complications of pin placement were seen in 13.7%. Although seven hips had penetration of the joint, there were no cases of chondrolysis or avascular necrosis. Excavation of the lateral femoral cortex was required at pin removal in 12.5% of cases. Analysis of the growth around pins allowed recommendations to be made regarding pin protrusion. The use of improved fixation devices may reduce the need for multiple pins.

Development of Bilateral Cam Deformity After Dunn Procedure and Contralateral Prophylactic Fixation: A Periosteal Reaction?: A Case Report.

CASE: We present the case of a fourteen-year-old male athlete with a slipped capital femoral epiphysis who was managed with a unilateral Dunn procedure and contralateral prophylactic screw fixation. Even though the cartilaginous epiphyseal growth plate was removed on one side, cam deformities subsequently developed on both sides in the postoperative period. CONCLUSION: This case suggests that structures other than the growth plate are also capable of inducing cam deformity of the hip.

Slipped capital femoral epiphysis in an adult with an unidentified Kallmann syndrome. Case report and etiology literature review.

A 23-year-old male consulted for a subacute-onset left hip pain, being evident on radiographs a slipped capital femoral epiphysis (SCFE) while the contralateral proximal femur physis was still slightly open. Urgent percutaneous in-situ fixation with two cannulated screws was performed. Further investigations due to the atypical features of the case revealed that the patient had anosmia, which allowed for Kallmann syndrome (KS) diagnosis, being responsible for the delayed skeletal maturity of the patient. Very few cases of a delayed-onset SCFE in association with KS have been described in the l

A comparison of two manual catching methods of broiler considering injuries and behavior.

The aim of this study was to compare the 2 manual catching methods in terms of injuries and behavior. Throughout 12 loadings on practical farms with the same standard, 1 container each was caught using the one-legged (1LCM) and 1 using the two-legged catching method (2LCM). The animals were filmed during loading to evaluate their behavior and subsequently examined regarding injuries. Wing flapping was observed more frequently in broilers caught with the 1LCM than 2 LCM. Carrying animals with neighbors (1 neighbor: P < 0.001; 2 neighbors: P < 0.001) and a grasping position at or above the tarsa

Bilateral Unstable Severe Valgus Slipped Capital Femoral Epiphysis: A Case Report.

CASE: Bilateral valgus slipped capital femoral epiphysis (SCFE) is a rare condition with few cases reported in the literature. However, there are no reports of bilateral unstable valgus SCFE. We report a unique case of bilateral acute-on-chronic unstable valgus SCFE with severe coxa valga in a 9-year-old girl. She was treated with bilateral capital realignment procedure along with subtrochanteric osteotomy and had a good clinical outcome. CONCLUSION: We hitherto report a rare case of bilateral unstable valgus SCFE which was successfully managed. This report also highlights the importance of t

Examining delays in diagnosis for slipped capital femoral epiphysis from a health disparities perspective.

Slipped Capital Femoral Epiphysis (SCFE) is a skeletal pathology affecting adolescents which requires timely surgery to prevent progression. Delays in diagnosis and treatment of SCFE can negatively affect patient prognosis, and few studies have examined how health disparities and barriers to care may influence these delays. In particular, only a handful of studies have included a Hispanic patient sample, despite this ethnic group's increased risk for the disease and unique barriers to care. A retrospective chart review was conducted for 124 patients surgically treated for idiopathic SCFE from
