[Correction osteotomy of neglected "monteggia" lesion with an external fixator].

OBJECTIVE: Definite reduction of the dislocated radial head to prevent further damage and loss of function in case of a neglected radial head dislocation following Monteggia lesion. Three different operation methods are demonstrated. INDICATIONS: A correction must be striven for in any fresh injury. The indication has to be made according to the complaints in chronic dislocations. A correction is very difficult and not always successful. CONTRAINDICATIONS: In principle, none. SURGICAL TECHNIQUE: In principle, the surgical technique depends on the underlying disease and the kind of deformity

[External fixation in the treatment of open tibial shaft fractures].

BACKGROUND/AIM: Besides the conquasant fractures, open tibia shaft fractures belong to the group of the most severe fractures of tibia. Open tibia shaft fracture is one of the most common open fractures of long bones. They most frequently occur as a result of traffic accidents caused by the influence of a strong direct force. METHODS: Within the period from January, 2000 to December 31, 2005 at the Clinic for Orthopaedics and Traumatology, Clinical Center Nis, 107 patients with open tibial fractures were treated. We analyzed 96 patients with open tibial fracture. In the series analyzed, the m

External fixation of tibial pilon fractures and fracture healing.

Distal tibial fractures are rare and difficult to treat because the bones are subcutaneous. External fixation is commonly used, but the method often results in delayed union. The aim of the present study was to find out the factors that affect fracture union in tibial pilon fractures. For this purpose, prospective data collection of tibial pilon fractures was carried out in 1998-2004, resulting in 159 fractures, of which 83 were treated with external fixation. Additionally, 23 open tibial fractures with significant > 3 cm bone defect that were treated with a staged method in 2000-2004 were ret

Early failure of external fixation in the management of war injuries.

OBJECTIVE: To review external fixation in the management of war injuries. METHOD: We prospectively followed up 15 external fixators (14 patients) applied in the management of war injuries. All these patients were treated at 202 Field Hospital during the 2003 Gulf Conflict. RESULTS: Of the 15 fixators, 13 (86.7%) required early revision or removal due to complications of the injury or the fixator. Instability was a problem with 10 fixators (67%), pin loosening was noted with 5 fixators (33%) involving twelve pins, and a significant pin track infection developed at 14 pin sites (3 fixators - 2…

[Psychiatric symptoms in patients treated with an Ilizarov external fixator].

OBJECTIVES: The Ilizarov technique limits daily activities of patients and may cause psychiatric symptoms. We investigated psychiatric symptoms associated with the use of an Ilizarov circular external fixator (CEF) and the relationship between symptoms and duration of CEF application in adult patients. METHODS: The study included 40 adult male outpatients (mean age 26.4 years; range 20 to 40 years) who were treated with a CEF. The participants were administered a questionnaire on sociodemographic and clinical characteristics. Psychiatric symptoms were assessed using the Symptom Check List (SC

Biomechanics and biology of external fixation of distal radius fractures.

External fixation is a versatile and useful tool for management of complex fractures. There is little to choose between the various types of commercially available fixators, and it is important to use one that allows the surgeon adequate versatility and follows sound biomechanical principles. Ligamentotaxis can be used effectively to reduce the most difficult fractures; however, over-distraction and prolonged traction are harmful and should be avoided. Certain types of fractures do not respond to treatment with ligamentotaxis alone and require adjunctive treatment, such as limited internal fix

[Arthrodesis of interphalangeal joints by means of external frame fixation].

PURPOSE OF THE STUDY: The indications for arthrodesis of interphalangeal joints include pain, instability, deformity or irreparable damage to the relevant flexor or extensor tendon. The general principle of arthrodesis is to resect the affected joint ends in the flexion desired so that a highest possible surface of contact may be provided. Subsequently, retention by means of internal or external fixation is performed. The aim of this study is to evaluate our method of interphalangeal joint arthrodesis, using an external frame fixator, to present the results and to point out the advantages of t

Open subtalar dislocation treated by distractional external fixation.

Subtalar dislocation is a rare injury, constituting approximately 1% of acute dislocations, and often is the result of high-energy trauma, particularly falls from heights. We present a case of a 33-year-old man who sustained a Gustilo type IIIB open subtalar dislocation when he fell 4 m from a scaffold. After irrigation and debridement, a Mitkovic distractional external fixator was applied medially for 6 weeks while the wound healed by secondary intention. Physical therapy was initiated along with partial to full weight bearing during the next 3 months. The patient returned to work at 8 months

Type 1B external fixation of a metacarpal fracture in an alpaca.

External skeletal fixation can be used in small ruminants as a successful, economic, alternative to internal fixation. In this case report, a type 1B frame was used on an alpaca to salvage a comminuted metacarpal fracture after failed internal fixation. Positioning the frame on the caudal aspect of the limb allowed full carpal flexion for kneeling and hence normal use of the limb throughout the recovery period. Complete healing occurred without further complications.

Indications, alternatives, and complications of external fixation about the elbow.

External fixation around the elbow is a concept as complex as the pathology that it treats. A myriad of potential pitfalls await the surgeon who casually applies a frame to any extremity, let alone a portion of one as complicated as the elbow. A careful understanding of the anatomy, specific technique, applications of the various devices commercially available, and rational alternatives to each problem is crucial for the judicial use of external fixation around the elbow. The potential benefits of the external fixator, however, are worthy of the surgeon's time to understand and pass on to his

The use of upper limb external fixation in paediatric trauma.

External fixation is an alternative method of treatment for paediatric fractures of the upper extremity. We report our experience of the management of 23 children with an average age of 10 years 1 month and review the literature. The method is indicated for second and third degree open fractures, open multiply injured patients and severely comminuted fractures. External fixation has proved unequalled for correction of limb deformities and lengthening procedures.

Ilizarov external fixation in the treatment of comminuted leg fractures.

Fifty consecutive comminuted leg fractures were treated by Ilizarov external fixation. Eleven were closed fractures, 37 were Grade II, and 2 were Grade III open fractures. Forty-five fractures healed within 6 months. In 5 fractures the Ilizarov device was removed and healing occurred by intramedullary nailing (2 cases) and decortication (3 cases). The causes of failure were: rapid peroneal union or bone fragment necrosis. Subjective tolerance and fracture stability are limited by septic loosening of the wires requiring frame removal after 6 to 8 months. Excellent results may be obtained (no le

Effect of pin location on stability of pelvic external fixation.

Pelvic external fixators allow two locations of pin purchase: anterosuperior (into the iliac crest) and anteroinferior (into the supraacetabular dense bone, between the anterior superior and anterior inferior iliac spine). The purpose of this study was to compare the stability of these two methods of fixation on Tile Type B1 (open book) and C (unstable) pelvic injuries. Five unembalmed cadaveric pelves (mean age, 68 years; four males and one female) were loaded vertically in a servohydraulic testing machine in a standing posture. The AO tubular system and Orthofix were used. On each pelvis, a …

[Treatment of war injuries of the trochanter area by external fixation].

In a specific series war injuries in the trochanteric region were characterized; a limited methodological approach magnified the problem. Our experience and the treatment results (debridement, drainage and external fixation) are presented. The methodology, complications and final results are also given for 15 patients. Consolidation was achieved in primary external fixation without additional procedures such as spongioplasty or internal fixation conversion methods. The time to consolidation without complications was 28.6 weeks (24-36) and with complications (deep infections, chronic osteomyeli

Pilon fractures: treatment with combined internal and external fixation.

The purpose of this study was to prospectively evaluate the use of limited internal fixation and the application of a hybrid external fixator (tensioned wires distally and 5.0 mm half pins proximally attached to a semicircular frame without crossing the ankle joint) in the treatment of severe distal tibia fractures. This technique involves accurate reduction and fixation of the intraarticular component through an incision based over a fracture site followed by stabilization of the metaphysis with the hybrid external fixator. We studied 26 patients 15-55 years of age who were followed for 8-36

External fixation of distal radial fractures: results and complications.

External fixation of unstable fractures of the distal radius yields satisfactory results but has a high complication rate. We studied thirty-five fractures in thirty-four patients to determine whether the results obtained with external fixation warranted it use. At a mean follow-up period of 31 months, the results of treatment were assessed by interviews and clinical and radiographic examination of both wrists. Twelve fractures had an excellent result, twelve had a good result, ten had a fair result, and one had a poor result. Radiographic results were graded excellent in ten fractures, good i
