BACKGROUND: Tranexamic acid (TXA) and recombinant human activated factor (F)VII (rhFVIIa) both control severe traumatic bleeding, with mortality benefit only demonstrated for TXA.
OBJECTIVES: To compare the effects of rhFVIIa and TXA on bleeding, trauma-induced coagulopathy (TIC), inflammation, and survival in a murine trauma model.
METHODS: Liver laceration (LL) was employed to induce TIC. C57BL/6J mice were pretreated intravenously with saline, rhFVIIa (3 mg/kg), or TXA (10 mg/kg). Blood loss, coagulopathy (activated partial thromboplastin time [aPTT], FII, FV, FVIII, FX, thrombin-antithro

BACKGROUND: Perioperative bleeding is a frequent high-risk complication, especially during Left Ventricular Assist Device (LVAD) implantation, and frequently necessitates reoperation. Bleeding control required many therapeutic options, and in refractory cases, recombinant activated Factor VII (rFVIIa) was considered one of the additional off-label options used. We report our experience with rFVIIa administration during LVAD implantation, particularly with the HeartMate III device, to assess safety outcomes, including pump thrombosis, stroke, and thromboembolic events.
METHODS: This is a retro…

BACKGROUND: Perioperative management for severe Factor VII (FVII) deficiency in thoracic surgery typically aims to reduce bleeding risk by correcting coagulation parameters with factor replacement—a strategy limited by cost, thrombotic risk, and availability. This report describes a phenotype-driven strategy, guided by a prior uneventful invasive procedure, that enabled safe surgery without routine factor replacement, offering a viable approach despite profound laboratory abnormalities. CASE PRESENTATION: A 58-year-old male with severe congenital FVII deficiency (FVII: C < 8%) and symptomatic …

Hemophilia A is a bleeding disorder caused by deficiency or low activity of circulating factor VIII characterized by prolonged blood coagulation time and often spontaneous bleeding. Patients with the severe form of the disease may present considerable heterogeneity in the occurrence of bleeding episodes and some of them have a mild hemophilia A phenotype. This study aimed to evaluate the association of biomarkers and coagulation parameters to the differential hemorrhagic profile of severe hemophilia A patients. Polymorphisms in the genes of proteins C and S, factors V and VII and prothrombin w…

BACKGROUND: Congenital coagulation factor VII (FVII) deficiency is a rare, autosomal-recessive haemorrhagic disorder with an estimated incidence of 1:500,000. This disorder is caused by mutations in the F7 gene.
CASE DESCRIPTION: Here, we report a pedigree of congenital FVII deficiency. The proband was a 30-year-old female with severely low FVII activity and a history of menorrhagia and epistaxis since her childhood who was subsequently diagnosed with congenital compound heterozygous FVII deficiency. A genetic study revealed a novel combination of compound heterozygous mutations (c.64G 〉 A, p…

Activated factor (F) VII is a vitamin K-dependent glycoprotein that initiates blood coagulation upon interaction with tissue factor. FVII deficiency is the most common of the rare congenital bleeding disorders. While the mutational pattern has been extensively characterized, the pathogenic molecular mechanisms of mutations, particularly at the intracellular level, have been poorly defined. Here, we aimed at elucidating the mechanisms underlying altered FVII biosynthesis in the presence of three mutation types in the catalytic domain: a missense change, a microdeletion and a frameshift/elongati

BACKGROUND: Inherited factor (F)VII deficiency is the commonest of the rare bleeding disorders, with a wide set of hemorrhagic features. Other than for the severe clinical forms (for which treatment guidelines are well defined), consistent recommendations regarding perioperative replacement management do not exist for mild and asymptomatic FVII-deficient patients.
OBJECTIVES: The present study aimed to evaluate the influence of bleeding history, FVII procoagulant activity levels (FVII:C) and the type of surgical procedure on the management of inherited FVII-deficient patients before surgery.

Pediatric critical care medicine : a journal of the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies2009eng
OBJECTIVES: Postoperative hemorrhage is a recognized complication of pediatric cardiac surgery. Both the immature coagulation system and increased susceptibility to hemodilution increase the likelihood of pediatric patients developing coagulopathy when compared with adult counterparts. Treatment options remain limited. Recombinant factor VII (rFVIIa) is a hemostatic agent increasingly used to reduce hemorrhage in other surgical settings, the role of which is unclear in this population. This article systematically reviews the published literature on the use of rFVIIa in pediatric cardiac surger…

Vitreous haemorrhage poses a serious threat to vision if untreated. Therapeutic options remain scarce and surgical intervention to resolve persistent bleeding is associated with risks that may further compromise vision. We report the use of recombinant activated factor VII (rFVIIa) in seven patients (six men, one woman; age, 30-65 years) with vitreous haemorrhage and severe reduction in visual acuity caused by trauma (n = 4) or proliferative diabetic retinopathy (n = 3). Initial doses ranged from 60 to 140 microg/kg; most patients received maintenance therapy with 20-60 microg/kg for at least …

Hermansky-Pudlak syndrome is a rare autosomal recessive disorder characterized by the absence of platelet dense bodies in association with albinism. We present the use of recombinant activated factor VII (rFVIIa) in a patient with Hermansky-Pudlak syndrome who underwent total thyroidectomy because of a large richly vascularized nodule (10 cm) compressing the trachea. The patient had a prolonged bleeding time (> 20 min) that remained unchanged after platelet transfusions. However, after infusion of platelets plus rFVIIa, it diminished to 5 min. The platelet aggregation response to adenosine dip…

Congenital factor VII (FVII) deficiency is an uncommon autosomal recessive bleeding disorder with highly variable clinical expression. Although most patients present with mild to moderate bleeding symptoms, severe cases may manifest with life-threatening hemorrhage, particularly intracranial bleeding. The emergence of inhibitors against exogenous FVII is exceedingly rare but poses a significant therapeutic challenge because of treatment refractoriness and an increased risk of major bleeding. We describe a five-year-old Saudi girl with severe congenital FVII deficiency diagnosed in the neonatal…

BACKGROUND: Congenital factor VII (FVII) deficiency is a rare inherited autosomal recessive disorder characterized by prolongation of prothrombin time and low FVII coagulation activity, which may increase the risk of bleeding.
CASE PRESENTATION: A 66-year-old man with acute postoperative intracranial hemorrhage was transferred to our hospital owing to coagulation dysfunction. In coagulation tests, the FVII coagulation activity was less than 2%. Genetic analysis of the gene encoding FVII identified compound heterozygous mutations: c. 681+1 G>T and c. C1286T (p. Ala429Val).
CONCLUSIONS: To our…

OBJECTIVE: To explore the molecular basis for a Chinese pedigree affected with hereditary coagulation factor VII (FVII) deficiency.
METHODS: The coding regions of F7 gene were amplified by PCR and sequenced. Suspected variants were confirmed by reverse sequencing and validated in other members from the pedigree. Pathogenicity of the variants was analyzed with multiple bioinformatic tools.
RESULTS: Genetic analysis revealed that the proband has carried compound heterozygous c.985T>C (p.Ser329Pro) and c.1091G>A (p.Arg364Gln) variants in exon 8 of the F7 gene. Her mother, brother and son were h…

BACKGROUND: In congenital Factor (F) VII deficiency bleeding phenotype and intrinsic FVII activity levels don't always correlate. Patients with FVII activity levels <30% appear to have a higher bleeding propensity, but bleeding can also occur at higher FVII activity levels. Reasons for bleeding at higher FVII activity levels are unknown, and it remains challenging to manage such patients clinically.
CASE: A 19year old male with spontaneous intracranial hemorrhage and FVII activity levels of 44%, requiring emergent surgical intervention and a strategy for FVII replacement. Genotyping showed th
