Clinical Characteristics and Pathophysiology of Fecal Incontinence Mixed With Constipation: An Underrecognized Problem.

INTRODUCTION: Fecal incontinence (FI) and chronic constipation (CC) are diagnosed as distinct problems, although many suffer with both problems. The phenotypical features and underlying mechanisms of FI mixed with CC (FI-MC) is unclear. We investigated the clinical and pathophysiological characteristics of FI-MC and compared with FI alone and healthy controls. METHODS: In a retrospective study, patients with FI were categorized as having FI-MC or FI alone. For comparison, we recruited healthy controls. All subjects completed bowel symptom questionnaire, anorectal manometry, balloon expulsion,…

Impact of social determinants of health on fecal incontinence treatment in older women.

BACKGROUND: Many women with fecal incontinence do not seek care despite the availability of effective treatments. Factors influencing care-seeking for fecal incontinence are not well elucidated, and the role of social determinants of health in fecal incontinence treatment utilization is unknown. OBJECTIVE: Our primary aim was to determine the association between social determinants of health and treatment utilization among Medicare beneficiaries with fecal incontinence. We secondarily aimed to determine the baseline treatment utilization rate and to determine factors associated with fecal inc…

Translation and validation of the Finnish version of the Fecal Incontinence Quality of Life Scale.

BACKGROUND AND AIMS: The Fecal Incontinence Quality of Life Scale (FIQL) is a health-related questionnaire that measures the quality of life among patients with fecal incontinence. This questionnaire comprises 29 items divided into four subscales: Lifestyle, Coping/Behavior, Depression/Self-Perception, and Embarrassment. The aim of the study was to validate the FIQL questionnaire in the Finnish language. METHODS: A multiphase validation method was used consisting of a two-panel translation process followed by a psychometric evaluation process. The validation process was used to ensure the con…

Biofeedback Therapy in Managing Functional Fecal Incontinence in Children: A Literature Review.

This literature review explores the role of biofeedback therapy (BFT) in managing functional fecal incontinence (FFI) in children - a common condition with a substantial impact on the quality of life. FFI diagnosis relies primarily on medical history and thorough physical examination and is categorized by the Rome IV criteria into functional constipation (FC) and functional nonretentive fecal incontinence (FNRFI). Treatment options for FFI remain limited, particularly for FNRFI. BFT employs electronic or mechanical devices, such as rectal probes or surface electrodes, to provide real-time feed…

[Progress in pathophysiology research and update on diagnosis and treatment strategies for fecal incontinence].

The traditional barrier theory believes that fecal incontinence is related to an imbalance of the recto-anal barrier and the characteristics of stool. However, in clinical practice this theory proves unable to explain all types of fecal incontinence. In recent years, research on the mechanisms related to fecal incontinence has shifted to a new integrative concept with the rectum and anus as functional units, and the central-peripheral nervous system and internal and external anal sphincters as a control loop. The diagnosis and treatment strategy of fecal incontinence, which is replaced by sacr…

The Artificial Bowel Sphincter in the Treatment of Fecal Incontinence, Long-term Complications.

BACKGROUND: Fecal incontinence is a common and debilitating condition, of which the prevalence increases with age. Several medical and minimally invasive treatment modalities are available. However, for patients with greater sphincter defects, these treatments are often not sufficient. For these patients, the artificial bowel sphincter could be an alternative to colostomy. The artificial bowel sphincter has proven to be effective in the short term. Less is known whether the benefits sustain over time. OBJECTIVE: The aim of this study was to assess the long-term outcome of the artificial bowel…

Central Nervous System Processing of Emotions in Children with Fecal Incontinence and Constipation.

OBJECTIVE: Fecal incontinence and constipation are common disorders in childhood. The enteric nervous system and the central nervous system are highly interactive along the brain-gut axis. The interaction is mainly afferent. These afferent pathways include centers that are involved in the central nervous processing of emotions as the mid/posterior insula and the anterior cingulate cortex. A previous study revealed altered processing of emotions in children with fecal incontinence. The present study replicates these results. METHODS: In order to analyze the processing of emotions, we compared …

Use of Sacral Nerve Stimulation for the Treatment of Overlapping Constipation and Fecal Incontinence.

BACKGROUND Fecal incontinence and constipation are common gastrointestinal complaints, but rarely occur concurrently. Management of these seemingly paradoxical processes is challenging, as treatment of one symptom may exacerbate the other. CASE REPORT A 51-year-old female with lifelong neurogenic bladder secondary to spina bifida occulta presented with progressive symptoms of daily urge fecal incontinence as well as hard bowel movements associated with straining and a sensation of incomplete evacuation requiring manual disimpaction. Pelvic floor testing showed poor ability to squeeze the anal …

Impact of the Bowel Management Program on the quality of life in children with fecal incontinence.

PURPOSE: Evaluate the impact of implementing Bowel Management Program (BMP) on quality of life (QoL) in children with fecal incontinence (FI). METHODS: Children 3-12 years referred to the Children's Treatment Center of FI were included and categorized accordingto the tendency to constipation (dilated colon; group 1) or diarrhea (non-dilated colon; group 2). Evaluations were performed before and after 3 months of BMP implementation. Perceptions from parents and children about QoL were assessed by the instrument PedsQL4.0 and the severity of FI by the fecal incontinence index (FII) of Wexner sc…

[Treatment for fecal incontinence in patients with anorectal malformations. Introduction of a therapeutic approach].

UNLABELLED: Fecal incontinence is a serious problem that may lead to social segregation and psychological problems. Patients with anorectal malformations frequently suffer fecal incontinence even with an excellent anatomic repair. In these patients an effective management program with enemas can improve their quality of life. We want to present our experience with bowel management and anorectal irrigation as treatment for stool incontinence. MATERIAL AND METHODS: Patients who presented with soiling regardless of the type of anomaly were included in the study. The diagnostic program comprised …

The magnetic anal sphincter: a new device in the management of severe fecal incontinence.

The authors aim to report the concept and technique of implantation and the first results of the clinical use of the magnetic anal sphincter (MAS) in the management of fecal incontinence (FI). The MAS device is designed to augment the native anal sphincter. The implant is a series of titanium beads with magnetic cores linked together with independent titanium wires. To defecate, the force generated by straining separates the beads to open up the anal canal. The technique of implantation is simple with no requirement of adjustments. The MAS has a role in the management of severe FI. The device …

[Fecal incontinence: therapy of congenital sphincter dysplasia in adulthood--a case report].

The dynamic graciloplasty has gained acceptance in the therapy of intractable fecal incontinence. With a success-rate of 60 to 80%, the dynamic graciloplasty is a good alternative towards a permanent colostomy for individual cases. Furthermore, adults suffering from congenital anal atresia may be well treated by this therapy as described in this case. Following surgery, an accurate follow-up is inevitable in these patients, including training of neosphincter control. After 8 to 12 weeks the training-process of the neosphincter-control should be finished. At this point of time the patient will …
