Deformities, gonarthrosis and function after distal femoral fractures.

Sixty-two patients treated for distal femoral fractures from 1969-1976 were re-examined, after a mean follow-up period of 5 years, to study deformities, gonarthrosis and function. These fractures occur mainly in elderly persons with bone fragility due to age and disease. The deformities were analysed from precisely defined radiographic projections. An anatomical classification into supracondylar, unicondylar and bicondylar fractures, with subdivisions for undisplaced and displaced fractures was used. A special group for transcondylar fractures was included. Displaced bicondylar fractures mostl…

Pipkin Type-II fractures of the femoral head.

Ten patients with a Pipkin Type-II fracture of the femoral head associated with dislocation of the same hip were treated and followed for two to five years. Five of these ten fractures, which were reduced anatomically after closed reduction of the dislocation, were treated by traction. All five healed and no evidence of avascular necrosis was identified radiographically or clinically at follow-up. Of the remaining five patients who had an unsatisfactory closed reduction, four were treated by open reduction and internal fixation with two screws and one, by excision of the comminuted fragment. [Causes and dynamics of incapacity in femoral fractures].

The causes and dynamics of invalidism following femur fractures were analysed in 549 patients. Serious fractures of the femur were most frequently associated with traffic accidents. Treatment defects are emphasized to be greatly responsible for the appearance of invalidism. The most effective treatment and use of dynamic observations of such patients is shown to be necessary.

Traumatic hip dislocation with ipsilateral femoral shaft fractures.

A traumatic dislocation of the hip joint associated with an ipsilateral femoral shaft fracture is a rare injury resulting from severe trauma. Initially the dislocated hip joint is often unrecognized and consequently reduction is delayed. Early diagnosis and reduction of the hip joint improves the functional prognosis. The maximum functional recovery is possible only with greater awareness of this combined injury. Examination of the hip joints by clinical roentgenography in the critically injured patient, especially those with head injuries, or physical signs masked by extremity swelling, shoul

Changes in serum alkaline phosphatase after femoral fractures.

Osteomalacia may be a contributory factor in some patients in the development of fractures of the femoral neck and complicate the subsequent management. The level of serum alkaline phosphatase is often valuable in the diagnosis of metabolic bone disease but rises after any uncomplicated fracture, and since such a rise may limit the diagnostic usefulness of this measurement in detecting osteomalacia its extent was assessed in 106 patients. In the majority serum levels were normal on admission, rising after seven to nine days to reach a maximum within a month after fracture. Elevated levels on a

Biological activity is not suppressed in mid-shaft stress fracture of the bowed femoral shaft unlike in "typical" atypical subtrochanteric femoral fracture: A proposed theory of atypical femoral fracture subtypes.

BACKGROUND: We have investigated mid-shaft stress fractures of the bowed femoral shaft (SBFs), well before the first report of an association between suppression of bone turnover and atypical femoral fractures (AFFs). Although all cases of SBF meet the criteria for AFF, SBFs can also occur in patients with no exposure to bone turnover suppression-related drugs (e.g., bisphosphonates). Using bone morphometry and biomechanical analyses, we devised a theory of AFF subtypes, dividing AFFs into fragility SBFs in the mid-shaft and "typical" subtrochanteric AFFs caused by suppressed bone turnover. Th…

Age as a Risk Factor for Intraoperative Periprosthetic Femoral Fractures in Cementless Hip Hemiarthroplasty for Femoral Neck Fractures: A Retrospective Analysis.

BACKGROUND: Understanding the risk factors and outcomes of intraoperative periprosthetic femoral fractures (IPFF) during hip arthroplasty is crucial for appropriate perioperative management. Previous studies have identified risk factors for IPFF in total hip arthroplasty patients, but data for hip hemiarthroplasty (HA) is lacking. The aim of this study was to determine the age associated with increased rates of IPFF in patients undergoing HA. METHODS: We retrospectively reviewed patients aged 65 years and above who underwent a cementless HA for a displaced femoral neck fracture and had a mini…

Comparison of bone turnover suppression in atypical femoral fractures and osteoporotic hip fractures.

We aimed to compare the extent of bone turnover suppression between patients with atypical femoral fractures (AFFs) and osteoporotic hip fractures (typical femur fractures, TFFs) using a one-to-one matching strategy. A single-center retrospective comparison of females aged ≥ 60 years who underwent operative treatment for AFFs and TFFs between January 2010 and March 2021 was conducted. Demographic characteristics and clinical data including fracture site, past medical history, bone mineral density (BMD), bisphosphonate (BP) medication history, and serum bone turnover marker (BTM) levels were ex…

Surgical treatment and prognosis of acetabular fractures associated with ipsilateral femoral neck fractures.

Combined ipsilateral acetabular and femoral neck fractures are the result of high-energy trauma. Satisfactory treatment for this injury pattern remains a challenge, since traditional open reduction and internal fixation (ORIF) is always accompanied by a high prevalence of posttraumatic arthritis and avascular necrosis of the femoral head. Eight of 502 acetabular fractures from 1990 to 2008 were diagnosed with combined ipsilateral femoral neck fracture, in which 5 patients' fractures were associated with hip dislocation. These patients were injured from falls, traffic accidents, or crushing acc

Femoral Neck Fracture With Anterior Tilt.

OBJECTIVE: Posteriorly tilted femoral neck fractures have been extensively studied, whereas anteriorly tilted fractures remain largely unreported. This study aims to characterize the clinical features, radiographic patterns, and postoperative outcomes of femoral neck fractures with anterior tilt. METHODS: This retrospective cohort study included adult patients with OTA/AO type 31B femoral neck fractures who underwent internal fixation within 36 h at a single orthopedic center from 2018 to 2022 and had ≥ 24 months of follow-up. Tilt angle was assessed using preoperative radiographs and CT. Ass

Dislocation fracture of the femoral head in adult.

The smaller the head fragment, the more likely it is located at the bottom and in front of the femoral head, which then determines the most suitable approach. A CT scan is essential after reduction and before resumption of weight-bearing to verify the size and congruence of the fragments and whether there are foreign bodies and/or a fracture of the posterior wall. Classifications should include the size of the fragment and whether or not there is an associated fracture of the acetabulum or femoral neck (historical 'Pipkin', modernised 'Chiron'). In an emergency, the dislocation should be recti

Management of Subtrochanteric Femoral Fracture Nonunions.

Compared with other proximal femoral fractures, subtrochanteric fractures are at a higher risk for nonunion because of the high deforming forces in this region, the associated increased risk of malreduction, and the risk of poor bone healing secondary to bisphosphonate use frequently associated with these fractures. Further understanding of nonunion of subtrochanteric fractures is of increasing importance given the rise in incidence of subtrochanteric hip fractures. Surgeons should be aware of risk factors for nonunion and techniques for prevention as well as surgical management and complicati…

Management of Pediatric Femoral Neck Fracture.

In the pediatric population, femoral neck fracture is a relatively uncommon injury with a high complication rate, despite appropriate diagnosis and management. The anatomy and blood supply of the proximal femur in the skeletally immature patient differs from that in the adult patient. Generally, these fractures result from high-energy trauma and are categorized using the Delbet classification system. This system both guides management and aids the clinician in determining the risk of osteonecrosis after these fractures. Other complications include physeal arrest, coxa vara, and nonunion. Multi

[Femoral neck fracture in a patient with osteopetrosis].

Osteopetrosis is a rare hereditary bone disease characterized by increased bone density. Patients with adult osteopetrosis are often diagnosed incidentally by X-ray or on the presence of recurrent fractures. Internal fixations of fractures in patients with osteopetrosis are challenging due to the increased bone density, and special surgical equipment is often necessary. Non-union and malunion occur frequently making close post-operative monitoring important. We present a case of adult osteopetrosis with a femoral neck fracture.

[Stress fracture following femoral epiphysiodesis].

Growth guiding is currently widely practiced for deformity correction in the growing child. Different methods have been developed. In 2007 Stevens described a tension band device (8-plates) as a simple growth control method. Since then it has been widely used, e.g. in hemiepiphysiodesis to correct varus or valgus deformities or extension defect in the knee. Temporary epiphysiodesis can also be used to correct leg length discrepancy more accurately. Complications after treatment with 8-plates are well-described. Stress fracture after the usage of this device has to our knowledge not yet been de

Insufficiency subchondral fracture of the femoral head.

The authors recently encountered a 65-year-old osteoporotic woman who had had intractable pain in the hip joint that was diagnosed clinically as osteonecrosis. She was treated by total hip replacement. Histopathologically, the most striking finding was the presence of a subchondral fracture with associated callus formation and granulation tissue along both sides of the fracture line. There was no evidence of antecedent osteonecrosis. This case was diagnosed histopathologically as insufficiency subchondral fracture of the femoral head. This is the first case report to substantiate the presence
