Brachial plexus compressive neuropathy following the use of axillary crutches (crutch palsy) is a rare but well-recognized entity. Most reported cases involve the posterior cord of the brachial plexus in children and have resolved spontaneously within 8-12 weeks. We recently treated a 36-year-old man who was using axillary crutches for mobilization after a supracondylar femoral fracture. Bilateral posterior cord (predominantly radial nerve) compressive neuropathy subsequently developed, with lesser involvement of the ulnar and median nerves. The patient had little to no improvement clinically

We report the clinical features of five patients with lumbosacral radiculoplexopathy (LSRP) and one patient with a femoral neuropathy, all of whom had serologic evidence of a recent Epstein-Barr virus (EBV) infection. After a thorough investigation, no other etiology was apparent. Pain was a prominent feature in all cases, and the prognosis was generally good, with recovery in weeks to months. We conclude that LSRP may occur as a postinfectious process following recent EBV infection.

We present the case of a 32-year-old woman with type III von Willebrand disease who, after a chiropractic manipulation, developed a large iliopsoas muscle hematoma with femoral neuropathy. Given the lack of recombinant factor VIII at our institution, we opted for a minimally invasive, interdisciplinary approach. The patient was clinically stable and fully adherent to the proposed interdisciplinary management plan. A percutaneous drainage was placed and supplemented with intralesional alteplase irrigations, which allowed for a progressive reduction of the hematoma, resolution of the neuropathy,…

Entrapment neuropathies of the lower extremity are often underdiagnosed due to limitations in clinical examination and electrophysiological testing. Advanced imaging techniques, particularly MR neurography and high-resolution ultrasonography (US), have significantly improved the evaluation and diagnosis of these conditions by enabling precise visualization of nerves and their surrounding anatomical structures. This review focuses on the imaging features of compressive neuropathies affecting the lumbosacral plexus and its branches, including the femoral, obturator, sciatic, common peroneal, and

Charcot-Marie-Tooth disease type 1A (CMT1A) is a demyelinating peripheral neuropathy caused by the duplication of peripheral myelin protein 22 (PMP22), leading to muscle weakness and loss of sensation in the hands and feet. A recent case-only genome-wide association study of CMT1A patients conducted by the Inherited Neuropathy Consortium identified a strong association between strength of foot dorsiflexion and variants in signal induced proliferation associated 1 like 2 (SIPA1L2), indicating that it may be a genetic modifier of disease. To validate SIPA1L2 as a candidate modifier and to assess

OBJECTIVE: To estimate the incidence, etiology, and outcome of neuropathies after major gynecologic surgery and to recommend management and prevention strategies for these complications.
METHODS: The medical records of women who suffered neuropathy after major pelvic surgery between July 1995 and June 2001 were reviewed. Mechanism of injury, treatment, and outcome were determined from the patient charts.
RESULTS: Twenty-three of 1210 patients undergoing major pelvic surgery during the defined period suffered a postoperative neuropathy for an incidence of 1.9%. Neurologic injury involved the

Few data exist pertaining to the validity of isokinetic muscle function tests, particularly in describing their interrelationships with other common clinical assessments. The purpose of this study was to critically analyze the maximal voluntary knee torque, motor unit activity, range of motion, and gait sequelae in a series of 95 patients who had tourniquet-aided meniscectomy or intra-articular loose-body removal. Prior to arthrotomy, affected and contralateral knee data differed minimally. In the first days following arthrotomy, the patients invariably demonstrated severe gait and muscle mech

INTRODUCTION: Chemotherapy-induced peripheral neuropathy (CIPN) is a common and disabling complication of several chemotherapeutic agents. Peripheral nerve stimulation (PNS) has emerged as a potential neuromodulatory treatment, but its effectiveness in CIPN remains unclear. This study evaluates whether optimizing PNS lead placement improves clinical outcomes.
METHODS: Four adults with moderate to severe CIPN (Numerical Rating Scale [NRS] ≥ 4) underwent PNS implantation. Lead placements were categorized as proximal (L5/S1), intermediate (femoral or sciatic at the subgluteal region), or distal

BACKGROUND AND OBJECTIVE: Lateral femoral cutaneous nerve (LFCN) decompression and transposition are surgical treatment options for meralgia paresthetica. Identifying the LFCN during surgery may be challenging, and preoperative localization is a valuable adjunct in this case. The objective of this study was to explore a new technique using preoperative ultrasound-guided clip localization (USCL) of the LFCN.
METHODS: After Institutional Review Board approval, data were collected on patients who underwent both preoperative ultrasound-guided wire localization (USWL) and USCL over the past 13 yea

OBJECTIVE: The treatment of some pelvic injuries has evolved recently to include the use of a subcutaneous anterior pelvic fixator (INFIX). We present 8 cases of femoral nerve palsy in 6 patients after application of an INFIX to highlight this potentially devastating complication to pelvic surgeons using this technique and discuss how it might be avoided in the future.
DESIGN: Retrospective chart review. Case series.
SETTING: Five level 1 and 2 trauma centers, tertiary referral hospitals.
PATIENTS/PARTICIPANTS: Six patients with anterior pelvic ring injury treated with an INFIX who experien

We present a rare case of avulsion-fracture of the anterior superior iliac spine with meralgia paresthetica in a 16-year-old male basketball player. He had sensory disturbance affecting his left lateral thigh 10 days after the injury. Tinel's sign was elicited on percussing the avulsed bony fragment of the anterior superior iliac spine. He underwent open reduction and internal fixation. The lateral femoral cutaneous nerve was noted to be entrapped by one third of the avulsed bony fragment. That fragment was removed, and the remaining portion was reduced and fixed with 2 screws. At week 6, the

Over-the-counter drugs are emerging contaminants in the environment. These contaminants often affect aquatic communities, including freshwater invertebrates. Nonsteroidal anti-inflammatory drugs (NSAIDs) are commonly found in aquatic ecosystems worldwide and have documented negative effects on aquatic organisms. Naproxen is often suggested as a more environmentally friendly alternative due to less bioconcentration in fish. However, studies have yet to test the effects of naproxen on freshwater molluscs. This study exposed pond snails, Physa spp., to three nominal levels of naproxen sodium, 100

We report on a 45-year-old man who presented with pain in the left leg, which he had had for 2 months. A contrast-enhanced CT scan displayed a 25-mm tumor in the lateral side of the left psoas muscle. For this suspected retroperitoneal tumor, we performed laparoscopic resection of the tumor, which was encapsulated by the funicular femoral nerve, using surgical scissors rather than a surgical energy device and preserved the normal fascicles wherever possible. Pathological examination indicated a benign ancient schwannoma arising from the femoral nerve. The patient was able to return to his job

BACKGROUND: Transient femoral nerve palsy (TFNP) is a well-known complication associated with ilioinguinal-iliohypogastric nerve block (IINB). We compared the incidence of TFNP after ultrasound-guided IINB and that after anatomical landmark-based IINB.
METHODS: We reviewed medical records of adult patients (ASA-PS 1-3, Age 21-87) who had received inguinal hernia surgery under general anesthesia and LINB retrospectively. IINB was performed using 0.5% ropivacaine either by ultrasound-guidance (US group, n = 16) or by landmark-based technique (LM group, n =17). TFNP was defined as sensory loss i

This is a case report of a 68-year-old male with stage III right lower extremity lymphedema following right inguinal lymph node dissection and adjuvant chemoradiotherapy for Hodgkin's lymphoma. He developed peripheral neuropathy and radiation-induced right femoral artery thrombosis, treated with saphenous vein graft. He underwent three vascularized lymph node transfers (VLNTs) to the upper medial thigh, posterior calf, and ankle with placement of nanofibrillar collagen scaffolds. Three months after surgery, he had volume reduction, less neuropathic pain, and improved ambulation.
