Emergence of behavioural states in fetuses of type-1-diabetic women.

The aim of this study was to investigate the effects of tightly controlled maternal (type-1-)diabetes mellitus on the development of fetal behavioural states. Seventeen diabetic women, who required insulin (White's class C predominantly) and were treated with continuous subcutaneous insulin infusion (CSII) therapy, participated in the study. Adjustment to an insulin-pump occurred before conception or during early pregnancy. In all diabetic women (near-)normoglycemia was achieved during pregnancy, with glycosylated hemoglobin-values within the normal range (6-8.5%). Fifty-three 2-h recordings o

Stimulation of fetal breathing activity by beta-adrenergic mechanisms.

Experiments were done on chronically prepared fetal lambs, 125-135 days gestation, to test the effects of various catecholamines on fetal breathing (FB) as well as the influence of isoproterenol on the fetal respiratory response to hypoxemia. Bolus injections of epinephrine, norepinephrine, and isoproterenol (5-20 micrograms) were administered via the lingual artery or femoral or jugular vein during periods of FB activity or apnea. The effects of epinephrine and norepinephrine on FB were variable and not statistically significant. Isoproterenol produced a significant increase in FB, frequency

Assessment of thoracoamniotic shunt catheter patency using pulsed-wave Doppler and color flow mapping: A case report.

Thoracoamniotic shunting is an effective treatment for fetal pleural effusion. Catheter obstruction is a serious and frequently occurring phenomenon that often requires re-shunting; however, accurate diagnosis remains challenging. We present a case in which pulsed-wave Doppler and color flow mapping were used to evaluate fluid flow through a thoracoamniotic shunt catheter. The Doppler assessment revealed alternating fast inflow (median: 69.2 cm/s) and slow outflow (median: 19.0 cm/s) synchronized with fetal breathing movements. Following the cessation of fetal breathing movements, a continuous

Parental Management Choices and Discordant Ultrasound Findings in Referrals for Fetal Spina Bifida.

OBJECTIVE: The severity of spina bifida aperta can be assessed prenatally by ultrasound. Morphological findings assist parents in choosing between management options. We aimed to document those management choices since the introduction of fetal surgery, and compare initial ultrasound findings prior to referral to findings in a fetal surgery center. METHOD: Single center cohort study of 245 consecutive fetuses with a second-trimester diagnosis of SBA. Data included nature of referral (for assessment or for surgery), condition-specific findings on ultrasound, and further management. We compared

Ultrasound diagnosis of the Pena Shokeir phenotype at 14 weeks of pregnancy.

This report describes the early prenatal diagnosis of the Pena Shokeir phenotype in an at-risk patient at 14 weeks' gestation. The diagnosis was based on an abnormal fetal movement profile, in association with an abnormal position of the fetal limbs. Pena Shokeir phenotype describes an inherited condition characterized by arthrogryposis and dysmorphic features as a result of fetal akinesia. It is a lethal abnormality and early diagnosis allows safer surgical methods of termination.

Postoperative Ultrasound as a Predictor of Newborn Function and Ambulation after Open Fetal Myelomeningocele Repair.

OBJECTIVE: Function of the lower extremities after prenatal myelomeningocele (MMC) repair is best assessed with ambulatory function at 30-36 months of age, but parents often ask about function before this milestone. Lower extremity movement can be assessed by ultrasound (US) and at the newborn exam (NE), but correlation between US, NE, and ambulation is not firmly established. METHODS: This was a retrospective correlation study of fetuses that underwent open prenatal MMC repair at SSM Cardinal Glennon Fetal Care Institute, St. Louis, MO, between January 2011 and June 2017. Movement at the ank

[A process audit of stillbirths in a rural family medical practice].

A process audit was undertaken of 5 stillbirths recorded in a rural family medical practice between 1985-1987. The stillbirth rate for this practice was 81/1000, as opposed to a national rate of only 4.5/1000 (p less than 0.015). The clinic records of the women, both before and during the affected pregnancies were reviewed in detail. In each case the family and hospital physicians involved were interviewed, as were 3 of the 5 patients. In 2 cases fetal death was recorded when the women were admitted to hospital after fetal movements had already been absent for more than 48 hours. The other 3 w…

Complex umbilical cord entanglement.

We describe three cases of complex umbilical cord entanglement seen in the past 5 years at our Placenta Clinic. The cases were associated with intermittent fetal bradycardia, reduced fetal movements, and were all delivered prematurely with multiple umbilical cord loops around the neck, limbs, or body. One intrauterine fetal death occurred. Placental pathology showed downstream effects of cord occlusion and fetal thrombotic vasculopathy.

MR imaging of pregnancy in bicornuate uterus.

We report a case of incarcerated bicornuate pregnancy diagnosed by magnetic resonance. In this unusual case both CT and sonography suggested an abdominal pregnancy. Magnetic resonance may be a valuable adjunct to sonography in the management of the complicated pregnancy in spite of nonvisualization of the fetal parts due to fetal movement.

Brief hypoxia in late gestation sheep causes prolonged disruption of fetal electrographic, breathing behaviours and can result in early labour.

KEY POINTS: Brief episodes of severe fetal hypoxia can arise in late gestation as a result of interruption of normal umbilical blood flow Systemic parameters and blood chemistry indicate complete recovery within 1-2 hours, although the long-term effects on fetal brain functions are unknown Fetal sheep were subjected to umbilical cord occlusion (UCO) for 10 min at 131 days of gestation, and then monitored intensively until onset of labour or delivery (<145 days of gestation) Normal patterns of fetal behaviour, including breathing movements, episodes of high and low voltage electorcortical activ

Intrauterine fetal volvulus presenting as fetal distress on cardiotocographic monitoring: a case report.

A rare case of intrauterine fetal midgut volvulus is described. The condition was diagnosed post delivery after the patient presented with markedly decreased fetal movements and fetal distress on cardiotocographic monitoring at 35 weeks' gestation. This baby was delivered and resuscitated before proceeding to laparotomy and bowel resection. The infant has subsequently made a good recovery. Intrauterine volvulus is rare, and presentation in this case was with fetal distress which has not previously been described as a presenting feature.

Pulmonary hypoplasia and deformations related to premature rupture of membranes.

In summary, fetal pulmonary development occurs in three phases. Hypoplasia results when an interruption occurs in or before the canalicular stage. Factors necessary for normal lung growth include sustained adequate amniotic fluid volume and normal fetal breathing movements. Pulmonary hypoplasia is most likely to occur with a very early rupture of membranes (less than 22 weeks) and with persistent oligohydramnios. Lack of sustained breathing movements and failure of modulation of ductal flow are strongly associated. Ultrasound measurement of fetal lung length may eventually be useful in monitor

Oligohydramnios in the second trimester of pregnancy, fetal breathing and normal lung growth.

Two patients with severe oligohydramnios in the second trimester, one due to spontaneous membrane rupture at 18 weeks and the other, uteroplacental insufficiency from 22 weeks were prospectively followed. In both cases despite severe oligohydramnios fetal breathing movements persisted and although one infant delivered at 28 weeks developed respiratory distress syndrome and the second meconium aspiration syndrome neither had evidence of pulmonary hypoplasia. We conclude the presence of fetal breathing movements, despite oligohydramnios, may preserve lung growth, and may act as a predictor of fa

Revisiting KANET in the context of fetal connectomics: defining optimal gestational windows for neurodevelopmental screening.

OBJECTIVES: To introduce the KANET-connectome matrix (KANET-Con) as a conceptual framework linking fetal behaviors observed on four-dimensional (4D) ultrasound to underlying neural network maturation, and to evaluate optimal gestational timing for functional neurodevelopmental screening. METHODS: A narrative review was conducted using a PRISMA-guided literature identification and screening process. PubMed, Scopus, and Web of Science were searched (January 2000-March 2025) for studies addressing fetal connectomics, fetal neurobehavior, KANET scoring, and developmental neuroimaging. Forty-two p

Fetal breathing during chronic lung liquid loss leading to pulmonary hypoplasia.

The mechanism of oligohydramnios-related pulmonary hypoplasia (PH) is not understood, but is known to involve chronic loss of lung liquid. In order to investigate the recent suggestion that fetal breathing movements are absent or diminished in oligohydramnios-related PH, we made continuous recordings of fetal breathing movements (FBM) during chronic lung drainage in tracheostomized fetal sheep. FBM remained present throughout the drainage period. There was no significant difference in the number or duration of FBM epochs in tracheostomized fetuses compared to controls, although there was a sli
