Selective occlusion of placental vessels using non-invasive focused ultrasound delivered by a toroidal transducer: Preclinical application in a pregnant monkey model for the treatment of twin-twin transfusion syndrome.

Twin-to-twin transfusion syndrome (TTTS) arises in 15% of monochorionic twin pregnancies due to unbalanced vascular anastomoses and remains a major cause of perinatal morbidity and mortality. Current treatment with laser fetoscopy is invasive, limited to superficial vessels, and technically challenging with anterior placentas. This study evaluated the feasibility, safety, and efficacy of non-invasive vascular occlusion using a hand-held high-intensity focused ultrasound (HIFU) device in a pregnant primate model. Seven pregnant monkeys with singleton fetuses were treated under general anesthesi

Prenatal Fetal Neurocutaneous Melanosis: A Case Report and Literature Review.

BACKGROUND: Neurocutaneous melanosis (NCM) is a rare congenital syndrome characterized by congenital melanocytic nevus of the skin with melanocytic deposits in the central nervous system. Patients with neurological symptoms have a poor prognosis and may die within years of symptom onset. CASE REPORT: A fetus was found to have diffuse enhancement of cerebellar hemispheric echo at 23 weeks of gestation and amniocentesis was performed at 25 weeks of gestation. Fetal DNA was extracted from amniotic fluid for copy number variation sequencing (CNV-seq) and Trio-total whole-exome sequencing (Trio-WE

The Changing Trends in Prenatal Diagnosis of Hemoglobinopathies in India: The Quest of a Single Center to Reduce the Burden of Disease over Three Decades.

The β-thalassemias and sickle cell disorders pose a considerable health burden in India. Of the more than 10,000 annual births of children with a severe hemoglobinopathy, only around 10.0% are managed optimally. Thus, genetic counseling and prenatal diagnosis (PND) is a valid option for a large and diverse country. Our center was one of the first to initiate PND and we present our experience over 30 years to evaluate the impact of awareness in changing the trends of PND of hemoglobinopathies. Both second and first-trimester diagnoses were undertaken by fetoscopy/cordocentesis and globin biosyn

[Diagnosis, evolution and prognosis of prenatally diagnosed suprasellar cysts].

Suprasellar arachnoid cysts (SAC) in children are considered rare, but the incidence is increasing due to the improvement of prenatal diagnosis. We present 15 cases of SAC diagnosed during the antenatal period between 2005 and 2015. The records were reviewed retrospectively by specifying the radiological characteristics, treatment modalities, outcomes, and long-term monitoring. Mean follow-up was 71 months. The forms (SAC-1) accounted for 2 cases (13%) with hydrocephalus. We observed 8 (53%) lower forms (SAC-2) with interpeduncular cistern expansion without hydrocephalus. The 5 (33.5%) remaini

Partial amniotic carbon dioxide insufflation (PACI) during minimally invasive fetoscopic surgery: early clinical experience in humans.

BACKGROUND: The technical performance of minimally invasive fetoscopic surgery may be severely hindered by poor visualization of intra-amniotic contents. Partial amniotic carbon dioxide insufflation (PACI) allows the visual limitations of operating within the fluid environment to be overcome. PATIENTS AND METHODS: When amniotic fluid exchange failed to improve fetoscopic visualization, PACI was attempted during 37 fetoscopic procedures between 17 + 5 and 33 + 2 weeks of gestation. PACI was attempted with filtered carbon dioxide using a commercially available insufflator via one to three troca

Is there a therapeutic role for fetoscopic surgery in the prenatal treatment of gastroschisis? A feasibility study in sheep.

BACKGROUND/PURPOSE: Gastroschisis is a malformation of the anterior abdominal wall that consists of a right paraumbilical defect with bowel loops bathed in the amniotic fluid. The prognosis relies mainly on morbidity attributable to intrauterine bowel loss or postpartum bowel dysfunction. The purpose of this study was to evaluate a potential role for fetoscopic surgery in severely affected fetuses with this condition. METHODS: Employing fetoscopy, we created a laparoschisis model in eight fetal sheep between 74-92 days of gestation; median 86.5 days). Twenty to 31 days after fetoscopic creati

Fetoscopic gene therapy for congenital lung disease.

Fetal gene therapy offers the promise of cure for certain genetic diseases, like cystic fibrosis and surfactant protein B deficiency. The authors hypothesized that a fetoscopic approach could attain a high level of organ-specific gene transfer to the fetal lung late in gestation. To test this hypothesis the authors examined the efficacy, specificity, and toxicity of recombinant adenovirus-mediated transfer of the beta-galactosidase marker gene to the lung of late gestation fetal sheep using a fetoscopic technique. Twelve fetal sheep of 125 to 135 days' gestation (term, 145 days) underwent feto

[Congenital anomalies of the central nervous system--from the pediatric standpoint].

Congenital anomalies of the central nervous system frequently cause severe emotional, economic, and physical hardship for both the child and the family. Craniofacial and developmental abnormalities may cause social unacceptability. Congenital hydrocephalus may result in neonatal death or mental and motor disabilities. Frequent hospitalization of prolonged institutionalization may be necessary for the many surviving patients. With recent advances in the diagnostic techniques of amniocentesis, fetoscopy, ultrasound, computerized tomography (CT) and magnetic resonance imaging (MRI), an improved a

Prenatal diagnosis of thalassemias and hemoglobinopathies.

Thalassemia syndromes and hemoglobinopathies are of clinical genetic significance because of the severity of the sequelae associated with particular genetic constitutions in these conditions, their occurrence at high frequencies in certain populations of Mediterranean, African, and Asian origin, and the high frequency of recurrence in pregnancies of at risk families. Application of recently developed techniques of molecular genetics to the antenatal diagnosis of the most deleterious of these conditions (homozygous beta-thalassemia [Cooley's anemia], homozygous alpha-thalassemia [Barts hydrops

Radial Expansion of Chorioamniotic Membranes After Perforation Causes Less Damage Than Fixed Diameter Instruments.

OBJECTIVE: Iatrogenic preterm premature rupture of membranes (iPPROM) complicates up to 40% of fetoscopic surgeries, driven by the limited healing potential of the chorioamniotic membrane after perforation. While radial expansion reduces tissue damage in other surgical fields, its application in fetal surgery remains unexplored. METHODS: We compared perforations in fresh chorioamniotic membranes created by three devices: standard 2 mm (6Fr) and 4 mm (12Fr) sheathed trocars, and an angioplasty balloon that radially expands from 2 to 4 mm. Perforation dimensions (height, width, area) and shape

Does the order of ablation of vein-to-vein placental connections during fetoscopic laser photocoagulation affect outcomes in twin-twin transfusion syndrome?

OBJECTIVE: To assess whether the sequence of laser occlusion of vein-to-vein placental anastomoses is associated with fetal survival after laser photocoagulation for twin-twin transfusion syndrome. STUDY DESIGN: This post hoc analysis used data from the Sequential Trial, a randomized controlled trial comparing sequential vs selective laser photocoagulation techniques. Within this trial, a nested randomization assigned patients with superficial anastomoses (ie, artery-to-artery or vein-to-vein) to ablation of these anastomoses either at the beginning or at the end of surgery. For this analysis

In-utero Therapy for Fetal Aqueductal Stenosis.

Advances in prenatal diagnosis, bioengineering, and minimally invasive surgical techniques have led investigators to reconsider in-utero therapy for fetal aqueductal stenosis, a form of congenital hydrocephalus that has been held under a moratorium since the 1980s. This article will review the definition, history, prenatal diagnosis, contemporary management, and future directions in clinical and basic science research for in-utero intervention for fetal aqueductal stenosis.

Validation of an instrument prototype for the minimally invasive fetal surgery of gastroschisis in an inanimate model.

INTRODUCTION: Complex gastroschisis requires timely intervention to protect the fetal intestine from inflammation and strangulation and avoid viscero-abdominal disproportion (VAD). Earlier results in ovine models for the fetoscopic management of gastroschisis highlight the benefits of minimally invasive coverage; yet specialized instruments appear to be needed for better procedural execution. The aim of this study was to create and validate a first prototype instrument for the prenatal covering of the protruded intestines in gastroschisis. METHODS: A 7-mm diameter fetoscopic instrument was de

Feasibility of continuous intra-uterine pressure measurements during amnioreduction in twin-to-twin transfusion syndrome therapy.

INTRODUCTION: This work explores the feasibility of simultaneous and continuous intra-abdominal, intra-uterine, and arterial blood pressure measurements to examine the hemodynamic perturbation expected during therapeutic amnioreduction and to better understand the protective role of the placenta during that treatment. METHODS: Patients with twin-to-twin transfusion syndrome were treated with fetoscopic laser ablation followed by amnioreduction. Intra-abdominal, intra-uterine, and mean arterial pressures were simultaneously recorded during amnioreduction performed in steps of 200 mL. Placental
