Minimally invasive disc preserving surgery in cervical radiculopathies: the posterior microscopic and endoscopic approach.

The aim of this paper is the report of the long-term results of a prospective study on spinal cervical lamino-foraminotomy via posterior route for the surgical treatment of cervical radiculopathies due to spondylodiscoarthrosis. The goal of the described surgical procedure is the bony decompression of the involved root leaving the intervertebral protruded or herniated discs intact. Indication, surgical technique, outcome and complications are discussed. Although anterior spinal cervical approach is the standard for centrally-located disc herniations with myelopathy, posterior foraminotomy appe

Transpedicular approaches to cervical uncovertebral osteophytes causing radiculopathy.

OBJECT: The anterolateral and posterior transpedicular foraminotomies were developed to enhance the treatment of cervical spondylotic radiculopathy secondary to uncovertebral osteophytes. These novel surgical approaches may be advantageous because they allow direct decompression of the nerve root while preserving normal movement and stability of the motion segment. The use of the pedicle to obtain access to compressive uncovertebral osteophytes, without causing damage to the motion segment, has not been previously reported. METHODS: The authors present the early outcome data obtained in 27 pa

Surgical management of cervical soft disc herniation. A comparison between the anterior and posterior approach.

Anterior cervical fusion was initially described in the 1950s for cervical spondylotic radiculopathy. The indications for this procedure in the management of soft disc herniation have not been clearly defined. In addition, controversy exists as to whether a cervical soft herniation should be managed by an anterior approach or a posterior cervical laminotomy-foraminotomy. The authors report the results of a prospective study comparing anterior discectomy and fusion to posterior laminotomy-foraminotomy for the management of soft cervical disc herniation. Twenty-eight patients underwent anterior

Successful Rectification of Cervical Nerve Root Irritation with Sequential Surgical Maneuvers during Posterior Cervical Instrumentation.

Posterior cervical instrumentation is used to treat a variety of cervical pathologies, including cervical spondylotic myelopathy, severe canal stenosis, and degenerative diseases. A 55-year-old man with severe cervical canal stenosis underwent a C3-C6 laminectomy and lateral mass screw fixation under general anesthesia. After lateral mass screw fixation and rod placement on the right side, raw electromyography (EMG) revealed irritative discharges in the right biceps brachii muscle. Following a suspicion of irritation/mechanical stretching of the C5-C6 nerve root, a posterior foraminotomy was p

Minimally invasive approaches to the cervical spine.

Minimally invasive approaches and operative techniques are becoming increasingly popular for the treatment of cervical spine disorders. Minimally invasive spine surgery attempts to decrease iatrogenic muscle injury, decrease pain, and speed postoperative recovery with the use of smaller incisions and specialized instruments. This article explains in detail minimally invasive approaches to the posterior spine, the techniques for posterior cervical foraminotomy and arthrodesis via lateral mass screw placement, and anterior cervical foraminotomy. Complications are also discussed. Additionally, il

Surgical management of first caudal nerve root foraminal compression secondary to intervertebral disc disease in a Cocker Spaniel.

A nine-year-old spayed female Cocker Spaniel was investigated for an eight week history of licking and rubbing at the tail base, dullness, and signs of pain on manipulation of the tail. Left-sided intraforaminal compression of the first caudal nerve root due to intervertebral disc disease was diagnosed by radiographic, computed tomography, and magnetic resonance imaging examinations. The dog was nonresponsive to conservative medical therapy. A decompressive left-sided first-second caudal (Cd1-Cd2) foraminotomy was performed. Postoperative computed tomography confirmed surgical decompression of

Modified transcorporeal anterior cervical microforaminotomy for cervical radiculopathy: a technical note and early results.

A prospective analysis of the first twenty patients operated for cervical radiculopathy by a new modification of transcorporeal anterior cervical foraminotomy technique. To evaluate early results of a functional disc surgery in which decompression for the cervical radiculopathy is done by drilling a hole in the upper vertebral body and most of the disc tissue is preserved. Earlier approaches to cervical disc surgery either advocated simple discectomy or discectomy with fusion, ultimately leading to loss of motion segment. Posterior foraminotomy does not address the more common anterior lesion.

Early Outcomes of Endoscopic Contralateral Foraminal and Lateral Recess Decompression via an Interlaminar Approach in Patients with Unilateral Radiculopathy from Unilateral Foraminal Stenosis.

OBJECTIVE: Percutaneous endoscopic contralateral interlaminar lumbar foraminotomy (PECILF) for lumbar degenerative spinal stenosis is an established procedure. Better preservation of contralateral facet joint compared with that of the approach side has been shown with uniportal bilateral decompression. The aim of this retrospective case series was to analyze the early clinical and radiologic outcomes of stand-alone contralateral foraminotomy and lateral recess decompression using PECILF. METHODS: Twenty-six consecutive patients with unilateral lower limb radiculopathy underwent contralateral

Cervical radiculopathy due to intraforaminal vertebral artery loop - posterior microvascular decompression with graft interposition technique.

BACKGROUND: Vertebral artery (VA) kinking is an uncommon cause of cervical radiculopathy. Here, a 57-year-old patient with unilateral C6 radiculopathy due to an aberrant VA loop documented on both magnetic resonance (MR)/computed tomography angiography (CTA) studies, successfully underwent left-sided C56/C67 foraminotomies resulting in symptom resolution. CASE DESCRIPTION: A 57-year-old presented with progressive left C6 and C7 radiculopathy and C6 weakness. Imaging revealed kinking of the VA at the C5-6 level, with the VA arterial loop compressing the left C6 nerve root within the foramen. I

The prevalence of Trichomonas gallinae in budgerigars (Melopsittacus undulatus) in a Veterinary Clinic in Vienna between 2000-2019.

Currently the posterior approach undertaken to perform cervical hemilaminectomy and foraminotomy provides sufficient exposure to treat the majority of lateral soft-disc herniations or osteophytes causing radiculopathy. Limitations imposed by the surgical field, however, often necessitate excessive retraction of the nerve root and epidural venous plexus, which may potentially exacerbate a preexisting radiculopathy or increase intraoperative blood loss. Partial resection of the inferior pedicle augments exposure and enlarges the neural foramen, thus facilitating decompression while minimizing ma

Safety and efficacy of direct nerve root decompression via anterior cervical discectomy and fusion with uncinectomy for cervical radiculopathy.

Cervical radiculopathy is a common spinal condition associated with pain, sensory disturbances, and motor weakness. Symptoms often can be attributable to either disc herniation and/or bony foraminal stenosis due to uncinate hypertrophy. Posterior cervical foraminotomy and conventional anterior cervical discectomy and fusion (ACDF) represent the mainstay of treatment. In patients with severe bony foraminal stenosis, posterior foraminotomy and standard ACDF without complete resection of uncinate process may result in incomplete decompression. ACDF with uncinectomy allows for complete and direct

[Surgical treatment of cervical intervertebral disk displacement].

Between the years 1976 und 1989 147 patients were operated on cervical disc herniation at the Neurosurgical Clinic of Innsbruck. 122 of these cases underwent a posterior approach according to Frykholm, 25 patients were treated by an anterior approach according to Cloward and Smith and Robinson, respectively. All patients received a descending myelography followed by a CAT-Scan of the questionable cervical segments the latter was available. In patients with disc herniation and only mild degenerative changes of the cervical spine a dorsal foraminotomy was chosen whereas the cases with nerve root

Real-Time Intraoperative Ultrasound Using a Minimally Invasive Transducer During Anterior Cervical Spine Surgery.

BACKGROUND AND OBJECTIVES: Intraoperative ultrasound (IOUS) during anterior cervical surgery is hindered by large transducer size and small operative corridor. We hypothesized that a linear (minimally invasive) transducer designed for transsphenoidal surgery can visualize the spinal cord, nerve roots, and surrounding structures during anterior cervical approaches, facilitating intraoperative assessment of central and foraminal decompression. METHODS: IOUS was used to evaluate 26 levels in 17 patients (15 anterior cervical discectomy and fusion, 1 corpectomy, 1 arthroplasty) with a linear prob

Full-Endoscopic Foraminal Decompression for Foraminal Stenosis Following Osteoporotic Vertebral Fracture in an Elderly Woman Under Local Anesthesia:A Case Report.

Osteoporotic vertebral fracture (OVF) is common in the elderly population. In this report, we describe a case with radiculopathy due to foraminal stenosis caused by OVF in a very elderly patient that was treated successfully by full-endoscopic foraminotomy under local anesthesia. The patient was an 89-year-old woman who presented with a chief complaint of left leg pain for 5 years. She visited a couple of hospitals and finally consulted us to determine the exact cause of the pain. Computed tomography scans were obtained and selective nerve root block at L3 was performed. The diagnosis was radi

Microsurgical excision of ganglioneuroma arising from the C8 nerve root within the neuroforamen.

Ganglioneuromas are benign, slow-growing tumors originating from sympathetic nerves or peripheral nerves, often associated with multiple tumor syndromes. They occasionally occur as spinal lesions and grow within the spinal canal or as paraspinal lesions. In this report, we describe a rare solitary ganglioneuroma arising from the cervical nerve root (C8) within the intervertebral foramen in adults. The tumor could be detected as a mass limited to the neuroforamen at an early stage by MR images. Unilateral microsurgical foraminotomy and EN BLOC resection of the tumor resulted in disappearance of

Clinical outcomes of microscopic decompression for degenerative lumbar foraminal stenosis: a comparison between patients with and without degenerative lumbar scoliosis.

We performed microscopic lumbar foraminotomy in all the patients diagnosed with degenerative lumbar foraminal stenosis (DLFS) and retrospectively reviewed the clinical outcomes and the factors influencing them. The preoperative Japanese Orthopaedic Association (JOA) score of 13.8 significantly improved to 21.9 postoperatively. Although leg pain reduced in 44 patients (95.7%) immediately after surgery, it recurred in 9 patients (19.6%). The recurrence frequency was significantly higher and the JOA score improvement ratios significantly lower in patients with degenerative lumbar scoliosis (DLS)

Microsurgical nerve root canal widening without fusion for lumbosacral intervertebral foraminal stenosis: technical notes and early results.

We describe a technique for microsurgical widening of the nerve root canal in the lumbosacral spine. We also report our early results in 31 patients (19 men and 12 women; average follow-up, 3.2 years) with such foraminal stenosis but without osseous defects in the pars interarticularis and/or spondylolisthesis. The affected nerve root was decompressed by interlaminar medial foraminotomy followed by lateral laminotomy and foraminotomy using a microsurgical technique. The procedure aimed at preserving bony continuity of the pars interarticularis and segmental motion. Neurological results showed

Optic Nerve Mobilization as an Alternative to Anterior Clinoidectomy for Superior Carotid-Ophthalmic Aneurysms: Operative Technique.

BACKGROUND: Carotid-ophthalmic aneurysms arise from the internal carotid artery between the distal dural ring and the origin of the posterior communicating artery. The surgical treatment of these aneurysms usually requires anterior clinoidectomy. However, this procedure is not without complications. In the present report, we have described optic nerve mobilization after optic foraminotomy as an alternative to anterior clinoidectomy to clip superior carotid-ophthalmic aneurysms. METHODS: We have reported the cases of 3 patients with superior carotid-ophthalmic aneurysms who had undergone surgi
