Biomechanical and clinical comparison of single lateral plate and double plating of comminuted supracondylar femoral fractures.

This study was to compare the relative strength of fixation and clinical outcomes of single lateral plate and double plating of comminuted supracondylar femoral fractures. Eight matched pairs of embalmed cadaveric femurs were selected. A gap osteotomy was created to stimulate an AO/OTA A3 comminuted distal femoral fracture. One femur of each pair was fixed with a locking plate; the other, with a locking plate and a medial plate. Nondestructive axial compression and maximum load to failure test were performed. A total of 32 patients with comminuted supracondylar femoral fractures were identifie

Open reduction and internal fixation with bone grafts for comminuted mason type II radial head fractures.

BACKGROUND: The use of bone graft for the radial head fractures has been previously described and occasionally used by other authors. This is the first paper, to my knowledge, dealing with the relevant issue about the importance that the use of an autologous bone graft can have on the radial head fractures. METHODS: From July 2010 to July 2014, 20 consecutive patients who underwent open reduction and internal fixation for a closed Mason type II radial head fracture were retrospectively reviewed. Patients with Mason type I, III, simple type II, and comminuted type II fractures treated without b

Open reduction and internal fixation of comminuted patellar fractures with headless compression screws and wiring technique.

BACKGROUND: Management of a displaced comminuted patellar fractures is challenging, and various surgical fixation methods have been suggested. However, issues of loss of reduction and breakage of fixatives have not yet been resolved. In the current study, we describe a new technique for exposure and stabilization of comminuted patellar fractures and evaluate the clinical and radiologic outcomes of this new treatment. MATERIALS AND METHODS: Thirteen patellar fractures with articular comminution, which were treated by headless compression screws with additional separate vertical wiring were enr

Torsional Failure of Carbon Fiber Composite Plates Versus Stainless Steel Plates for Comminuted Distal Fibula Fractures.

BACKGROUND: Carbon fiber composite implants are gaining popularity in orthopedics, but with few independent studies of their failure characteristics under supra-physiologic loads. The objective of this cadaveric study was to compare torsional failure properties of bridge plating a comminuted distal fibula fracture with carbon fiber polyetheretherketone (PEEK) composite and stainless steel one-third tubular plates. METHODS: Comminuted fractures were simulated in 12 matched pairs of fresh-frozen human fibulas with 2-mm osteotomies located 3 cm proximal to the tibiotalar joint. Each fibula pair

Biomechanical evaluation of two innovative locking implants for comminuted olecranon fractures under high-cycle loading conditions.

INTRODUCTION: The relatively high complication rate after fixation of olecranon fractures has led to an increasing application of anatomically pre-contoured locking plate systems. The purpose of the present study was to conduct a biomechanical comparison of olecranon osteosyntheses by applying the Olecranon VA-LCP and the 3.5mm LCP Hook Plate (LCP, locking compression plate) to an unstable fracture model under high-cycle loading conditions. METHODS: After creating an unstable fracture (Schatzker type B), osteosynthesis was performed on eight pairs of fresh-frozen cadaveric ulnae by applicatio

Fixation of comminuted distal radius fractures with a mixture of calcium phosphate and calcium sulfate cement.

Distal radius fracture alignment and stabilization can be a surgical challenge in the face of severe comminution and bone loss. We describe a technique using a calcium phosphate/sulfate bone cement, as an adjunct to internal fixation. This bone cement is biocompatible, osteoconductive, and sets quickly with an isothermic reaction. The use of bone cement eliminates the need for primary autologous bone grafting and allows for easier reduction and retention of reduction at the time of surgery. Bone cement is employed for the following purposes in comminuted fractures: (1) to fill a void due to lo

Application of circular external fixator under arthroscopic control in comminuted patella fractures: technique and early results.

BACKGROUND: Comminuted fractures of the patella are traditionally treated by internal fixation. The modified tension-band technique is widely accepted for internal fixation. The management of comminuted fractures often requires additional K-wire and/or circlage material and wide surgical exposure. Therefore, symptoms and complications related to the stainless steel wire are not uncommon in these cases. We present a new alternative treatment technique for comminuted patellar fractures. METHODS: Five comminuted patellar fractures of four patients were treated by circular external fixator (CEF)

Use of a type-III trilateral external skeletal fixation device in three deer with comminuted fractures.

A type-III trilateral external skeletal fixation device was used to repair comminuted fractures of metacarpal and tibial bones in 3 deer. Mean weight of the deer was 25 kg, and mean age was 4 months. Mean interval from application until removal of the device was 6.5 weeks. All deer had complete function of their previously fractured limbs and had minimal complications during healing. Several factors should be considered when choosing a method of repair for wildlife with fractured limbs. Economic constraints and demeanor of the animal may limit the number of options. Analysis of results of our

Outcomes of Autologous and Synthetic Bone Grafts in Locking Plate Fixation of Comminuted Distal Radius Fractures.

Background The introduction of locking plate technology has improved the feasibility of distal radius fracture fixation without the need for bone grafting, yet challenges persist in cases of severely comminuted fractures and small, unstable intra-articular fragments. This study aimed to assess the outcomes of bone grafting in severely comminuted distal radius fractures treated with locking plates. Methods We performed a retrospective analysis involving 450 patients who underwent distal radius fracture fixations. We evaluated wrist motion, grip strength, and radiographic parameters, including r

The use of beta-tricalcium phosphate bone graft substitute in dorsally plated, comminuted distal radius fractures.

BACKGROUND: Intraarticular distal radius fractures can be treated with many methods. While internal fixation with angle stable implants has become increasingly popular, the use of bone graft substitutes has also been recommended to address comminution zones and thus increase stability. Whether a combination of both methods will improve clinical outcomes was the purpose of the study METHODS: The study was thus conducted as a prospective randomized clinical trial. 39 patients with unilateral, intraarticular fractures of the distal radius were included and randomized to 2 groups, one being treat
