Design and implementation of modular laparoscopic general surgery models for surgical education.

Appendectomy, cholecystectomy, and inguinal herniorrhaphy are fundamental procedures in general surgery. These surgeries help trainees develop essential surgical skills, including technical proficiency and surgical planning. In this study, we aimed to design and produce modular laparoscopic surgical training models tailored to the needs of surgical education. Modular laparoscopic models for appendectomy, cholecystectomy, and inguinal herniorrhaphy were developed. The cholecystectomy and appendectomy models consisted of two components: a frame and a module, whereas the herniorrhaphy model inclu…

Evaluation of interfacility transfer patterns among patients with emergency general surgery conditions.

BACKGROUND: Interfacility transfer for emergency general surgery is a key strategy for improving access to care. However, lower-intensity transfers are linked to increased costs, poor patient and family experience, and inefficient resource allocation. Clinical and health system characteristics associated with lower-intensity transfers among patients with emergency general surgery conditions remain unclear. METHODS: We performed a cohort study among adults with emergency general surgery conditions using claims data from Florida and California. Lower-intensity transfers were defined as admissio…

Cardiac arrest during alveolar recruitment manoeuvre under general anaesthesia for laparoscopic surgery.

A 67-year-old woman with no history of cardiovascular disease, undergoing an elective laparoscopic cholecystectomy, experienced severe bradycardia and cardiac arrest immediately following an alveolar recruitment manoeuvre under general anaesthesia. Prompt cardiopulmonary resuscitation restored cardiac output within 2-3 min. Postoperatively, she remained stable and was discharged following 24 h of monitoring. The cardiac arrest was likely triggered by vagal nerve stimulation and activation of intrinsic cardiac reflexes by the alveolar recruitment manoeuvre. The event emphasises a rare, but sign

[Evidence, Availability and Future Visions in Simulation in General and Visceral Surgery].

Practice makes perfect - a saying that everyone has certainly heard. Surgeons of all levels of training can demonstrably practice to some extent on simulators. This training outside the operating theatre and independent of patients makes sense, both ethically and financially. Although the effectiveness of simulation in surgery has been proven several times, simulation training is not a mandatory part of surgical specialist training in Germany. Simulation covers a very wide range in terms of application, effort and costs. This review is intended to give an overview of the systems and their area

Artificial intelligence versus surgeon gestalt in predicting risk of emergency general surgery.

BACKGROUND: Artificial intelligence (AI) risk prediction algorithms such as the smartphone-available Predictive OpTimal Trees in Emergency Surgery Risk (POTTER) for emergency general surgery (EGS) are superior to traditional risk calculators because they account for complex nonlinear interactions between variables, but how they compare to surgeons' gestalt remains unknown. Herein, we sought to: (1) compare POTTER to surgeons' surgical risk estimation and (2) assess how POTTER influences surgeons' risk estimation. STUDY DESIGN: A total of 150 patients who underwent EGS at a large quaternary ca…

Clinical and financial outcomes of transplant recipients following emergency general surgery operations.

INTRODUCTION: Due to immunosuppression and underlying comorbidities, transplant recipients represent a vulnerable population following emergency general surgery (EGS) operations. The present study sought to evaluate clinical and financial outcomes of transplant patients undergoing EGS. METHODS: The 2010-2020 Nationwide Readmissions Database was queried for adults (≥18 years) with non-elective EGS. Operations included bowel resection, perforated ulcer repair, cholecystectomy, appendectomy and lysis of adhesions. Patients were classified by transplant history (Non-transplant, Kidney/Pancreas, L…

Antibiotic prescribing practices in general surgery: a mixed methods quality improvement project.

BACKGROUND: A single pre-operative antibiotic dose provides optimal prophylaxis against surgical site infection (SSI), but significant variability persists in adherence to prophylaxis guidelines. We describe a quality improvement project aiming to improve guideline-driven antibiotic prescribing within surgical teams at a tertiary hospital. METHODS: Face-to-face interviews with surgical teams and anonymous surveys of senior surgeons and anaesthetists were used to collect qualitative data on the perceptions and attitudes of prescribers. This informed intervention development, including a daily

Psychiatric Comorbidity in Bariatric Surgery: A Retrospective Study in a General Hospital.

BACKGROUND: Candidates for bariatric surgery undergo a multidisciplinary evaluation in the pre-operative phase, including a psychiatric visit aimed at the screening for psychiatric comorbidities, including feeding and eating disorders (FEDs), which are shortcomings to the intervention or predictors of worse prognosis. The presence of FEDs, such as Binge Eating Disorder (BED) and Bulimia Nervosa (BN), is associated with higher rates of other psychiatric disorders. Furthermore, there is evidence of the association between obesity and Depressive Disorders, as well as B and C Cluster Personality D

Morbidity and mortality after general surgery in heart and lung transplant patients.

BACKGROUND: Heart and lung transplant patients can develop conditions necessitating general surgery procedures. Their postoperative morbidity and mortality remain poorly characterized and limited to case series from select centers. METHODS: The National Inpatient Sample (1998-2015) was used to identify 6433 heart and 3015 lung transplant patient admissions for general surgery procedures. For a comparator group, we identified 23,764,164 nontransplant patient admissions for the same procedures. Patient morbidity and mortality after general surgery were compared between transplant patients and n…

Recommendations for Trauma and Emergency General Surgery Practice During COVID-19 Pandemic.

COVID-19 is a new disease, based on currently available limited information, older adults and people of any age who have severe underlying medical conditions may be at higher risk for severe illness from COVID-19. People of all age groups are also at risk. Healthcare providers have always been the professionals most exposed to the risk of contracting to any kind of infection due to the nature of their profession. Elective interventions have been postponed to give care of patients with COVID-19. However, some interventions cannot be delayed, such as trauma surgery, acute abdomen, and emergency

Assessment of emergency general surgery care based on formally developed quality indicators.

BACKGROUND: Emergency general surgery outcomes vary widely across the United States. The utilization of quality indicators can reduce variation and assist providers in administering care aligned with established recommendations. Previous quality indicators have not focused on emergency general surgery patients. We identified indicators of high-quality emergency general surgery care and assessed patient- and hospital-level compliance with these indicators. METHODS: We utilized a modified Delphi technique (RAND Appropriateness Methodology) to develop quality indicators. Through 2 rankings, an e…

Postoperative nausea and vomiting after general anesthesia for oral and maxillofacial surgery.

BACKGROUND: Postoperative nausea and vomiting (PONV) is one of the most common postoperative complications. Despite this, few papers have reported the incidence and independent risk factors associated with PONV in the context of oral and maxillofacial surgery (OMFS). Therefore, we sought to determine the incidence of PONV, as well as to identify risk factors for the condition in patients who had undergone OMFS under general anesthesia. METHODS: A total of 372 patients' charts were reviewed, and the following potential risk factors for PONV were analyzed: age, sex, body mass index, smoking sta
