[Seip Berardinelli: the effect of the partial tongue resection].

The Seip Berardinelli or undiagnosed endocrine metabolic syndrome is a rare autosomal recessive pathology mainly described in families of Portuguese origins. This syndrome presents various metabolic disturbances responsible of various dysmorphies. We report the case of two brothers seen during their childhood for respiratory, speaking and feeding problems related to a class 3 of Angle and macroglossia. The object was to study the interaction between the size of the tongue, the prognathism and the disturbances presented in order to organize early surgery (partial glossectomy).

[A case of hypopigmentation triggered by S-1].

Right partial glossectomy and supraomohyoid neck dissection were performed on a 65-year-old man with tongue squamous cell carcinoma. Postoperatively, S-1(80-120 mg/day)was administered as adjuvant chemotherapy. Six months later, an eruption accompanied by abdominal itching was recognized. Then leukoderma appeared on his face and both hands, continued to spread and it was diagnosed by dermatology as hypopigmentation. He has been doing well without any recurrence for 4 years, but the hypopigmentation remains unchanged.

[Surgical treatment of chronic snoring. Description and indications].

Chronic rhonchopathy groups all clinical forms of a disease having a common functional sign: snoring. Surgical treatments form part of a therapeutic strategy which includes medical treatment, essentially reduction in body weight, and nocturnal respiratory prosthetic treatments, essentially continuous positive pressure respiration. First intention surgery is an uvulopalatopharyngoplasty (UPP) with or without tonsillectomy. In the severe forms of chronic rhonchopathy, after failure of UPP, major surgery:protrusion mandibular osteotomy or reduction glossectomy can be associated with the other tre…

Functional oromandibular reconstruction with the microvascular composite groin flap.

Ten patients were reconstructed with the microvascular osteocutaneous groin flap for oromandibular defects with the objective of improving function. The flap was based on the superficial and deep circumflex iliac vessels for optimal positioning of the bone and contouring of the skin. Patients with major glossectomies and arch resections had intelligible speech and were able to eat a soft diet without aspirating. Cineradiographic studies to evaluate swallowing in selected patients showed that the shape of the intraoral flap and the location of the bone graft played an important role in swallowi

Tongue Reduction for Pediatric Obstructive Sleep Apnea: A Systematic Review.

OBJECTIVE: Obstructive sleep apnea (OSA) is a prevalent pediatric disorder that, if untreated, can result in substantial developmental and health consequences. Tongue reduction procedures, such as midline posterior glossectomy (MPG), are increasingly used to address tongue base obstruction. This systematic review evaluates the effectiveness and safety of tongue reduction procedures for pediatric OSA. DATA SOURCES: Embase, PubMed, and Web of Science databases were queried for studies from inception to September 20, 2023. REVIEW METHODS: Studies reporting on outcomes of pediatric patients (≤21…

CT volumetric analysis permits comparison of tongue size and tongue fat in different canine brachycephalic and mesaticephalic breeds.

While macroglossia is a newly accepted component of brachycephalic obstructive airway syndrome (BOAS) in dogs, macroglossia with increased tongue fat is a well-known cause for obstructive sleep apnea (OSA) in people, and targeted reduction procedures such as midline glossectomy are used to treat people with OSA. While midline glossectomy has been described in dogs, tissue contributions to macroglossia have not been characterized. The purpose of this retrospective, descriptive, case-control study was to describe and compare volumetric dimensions of the tongue and tongue fat in brachycephalic (B…

Assessing the suitability of medial sural artery perforator flaps in tongue reconstruction - An outcome study.

INTRODUCTION: Oncological resection of the tongue can be reconstructed using a multitude of free flaps. The medial sural artery perforator (MSAP) flap has been well described in the literature in terms of its anatomy and harvest. However, functional outcome studies of post-reconstruction tongue defects using the MSAP flap have not been reported. This study represents the largest outcome study of patients with tongue reconstructions using MSAP flaps and a comprehensive review of its use. MATERIALS AND METHODS: From December of 2010 to October of 2015, 579 patients with subtotal glossectomy and…

Effects of smoking cessation on taste function in heavy smokers undergoing hemiglossectomy for tongue squamous cell carcinoma.

PURPOSE: Smokers with squamous cell carcinoma (SCC) of the tongue may be particularly prone to experience multifactorial gustatory dysfunction. This study investigates the effects of heavy smoking on taste perception in patients undergoing glossectomy type IIIa (i.e. non-compartment hemiglossectomy) for SCC of the tongue. METHODS: Gustatory function was assessed in 30 heavy-smoking patients with SCC of the tongue using a validated taste strips test. Psychophysical evaluations were conducted at baseline and at 3, 6, 9, and 12 months post-surgery. RESULTS: At baseline, mean taste strip scores …

Updated Hypopharyngeal Surgery for Sleep Apnea.

The failure of traditional upper airway surgery such as uvulopalatopharyngoplasty has been attributed to lower airway obstruction related to base of tongue collapse. Multiple procedures, including glossectomy, tongue base radiofrequency, genioglossus advancement, and tongue suspension techniques, have been advocated to improve success rates. No consensus exists on which subsets of patients are best treated by individual approaches and direct comparative data are lacking. The selection of procedures must be based on individual patient needs and the relative potential benefits and risks.

Surgical management of postradiation residual/recurrent cancer of the base of the tongue.

A study of 78 cases of base of the tongue cancer treated surgically for control of disease at the primary site is presented. Seventy-four of these cases were radiation failures, surgery being resorted to as a salvage procedure. The choice of the various surgical procedures is discussed and the cure rates analysed. Thirty-five cases required a total glossectomy, of which twenty-six did not have removal of the larynx. The teasibility of preserving the larynx at least in younger individuals and when good nursing care is available is stressed.

Lingual pedicle intraoral anastomosis for tongue reconstruction after partial or hemiglossectomy: technical note and case report.

OBJECTIVES: The intraoral anastomosis technique (IAT) is an approach that consists of performing microvascular anastomoses within the oral cavity. It avoids cervical incisions and oro-cervical communication. Although widely described using the facial pedicle, its application to the lingual pedicle remains poorly documented. This study aims to describe the surgical anatomy, technical feasibility, and clinical relevance of IAT using the lingual vascular network for tongue reconstruction following partial glossectomies and hemiglossectomies. MATERIALS AND METHODS: A detailed review of tongue ana

Evaluation of Facial Artery Musculomucosal Flap for Reconstruction of Small Tongue Defects.

BACKGROUND: Partial glossectomy defects can be managed by different methods, ranging from primary closure to pedicled or free flaps. The facial artery musculomucosal (FAMM) flap provides an excellent match to lingual tissue and provides an inconspicuous donor site. This study aims to compare functional outcomes, especially speech, of patients with partial glossectomy defects (≤1/3rd of tongue) reconstructed by FAMM flap with those of similar defects closed primarily or healed by secondary intention. It also offers to resolve the question of whether defects of this size should be reconstructed …

Tumor histologic grade as a risk factor for neck recurrence in patients with T1-2N0 early tongue cancer.

OBJECTIVES: In current guidelines, early tongue cancer status post partial glossectomy without adverse risk features do not require adjuvant treatment. However, many of these patients developed recurrence with neck metastases soon. The objectives of this study were to investigate the potential risk factors in early tongue cancer that prophylactic management of neck may be considered. MATERIALS AND METHODS: From January 2010 to September 2015, this retrospective study enrolled 102 patients with T1-2N0 primary oral tongue squamous cell carcinoma according to AJCC 8th edition Cancer Staging Syst…

The sternocleidomastoid perforator flap.

The conventional pedicled sternocleidomastoid (SCM) flap has a poor arc of rotation, limited volume and precarious vascularity. This report describes a new technique for raising a SCM flap based on the perforating vessels of the superior thyroid vascular pedicle. The upper and lower attachments of the sternocleidomastoid muscle are divided. Four medically and/or surgically compromised patients have successfully undergone reconstruction of hemiglossectomy (1), partial glossectomy (1) and rim of mandible (2) defects for malignancy. The arc of rotation of the SCM flap is greatly increased and the…
