Heart transplantation has become a more and more effective therapeutic strategy in severe heart failure patients. An opportune management of the several medical, immunological and psychological complications, that may occur during heart transplant recipients' life, is mandatory to succeed in this therapeutic approach. Indeed, thanks to heart transplantation, recipients may recover from a lethal cardiovascular disease, but on the other hand, they may encounter several co-morbidities. An optimized management has to involve not only the referring Transplant Center, but also the single patient's p

As several authors stated within the last few years and according to our experience as well cadaveric kidneys of young children are suitable for transplantation. Considering the shortage of transplantable organs en-bloc-transplantation should not be performed, even if this provides the opportunity to remove one kidney in case of surgical complications. On the other hand by transplanting a single organ one donor may help two recipients. Since it is known that kidneys of babies under 18 months do not guarantee sufficient function in adults, it should be a rule that kidneys of children under 18 m

The use of deceased extended criteria donors has shortened wait times for potential transplant recipients. Their use has been quite helpful in allowing elderly recipients access to transplant before the onset of intractable complications. Resorting to living extended criteria donation on the other hand represents a total disregard of the interests of the living donor and should be actively discouraged.

There are very few observations on the occurrence of stroke after general surgery; the incidence lies between 0.2 and 0.7%. On the other hand, cerebrovascular accidents are mainly seen after cardiac and carotid surgery, where the incidence is high (5%). Strokes are also an important cause of morbidity and mortality following orthotopic transplantations, particularly heart, heart/lung and liver transplantations.

Dystrophic epidermolysis bullosa (Hallopeau-Siemens, recessive dystrophic epidermolysis bullosa) is a rare inherited disorder of the skin and mucosa, characterized by blistering in response to the slightest mechanical trauma. Healing is associated with scarring and the formation of contractures and milia. Following repetitive trauma (friction), this process leads to severe hand deformities with digits contracted in flexion, the thumb contracted in adduction and pseudo-syndactyly. In advanced cases (as described here), the hands show a mitten-like deformity and digits are encased in an epiderma

Today, kidney transplantation is the treatment of choice for most patients with end-stage renal failure. However, because of the organ shortage, the limiting supply and increasing demand, needing transplants do not receive them. Chronic renal disease should be efficiently screened and prevented. On the other hand, all ways of using available organs, from living and deceased donors, including those with extended criteria are needed. Finally, strategies targeting factors involved in to chronic allograft nephropathy must be pursued in the effort to improve long-term outcomes after renal transplan

BACKGROUND/OBJECTIVE: We sought to evaluate if autologous hematopoietic stem cell transplantation (AHSCT) influences the functional status of systemic sclerosis (SSc) patients.
METHODS: From 2014 to 2018, a cohort of 27 SSc patients was assessed before, and at 6 and 12 months after AHSCT for modified Rodnan's skin score (mRSS), mouth opening, hand grip strength, range of motion (ROM), functional ability of upper limbs (DASH questionnaire and Cochin hand function scale-CHFS), 6-minute walk test (6MWT), and quality of life (SF-36 questionnaire). Linear regression models with random effects and

Deafferentation is accompanied by large-scale functional reorganization of maps in the primary sensory and motor areas of the hemisphere contralateral to injury. Animal models of deafferentation suggest a variety of cellular-level changes including depression of neuronal metabolism and even neuronal death. Whether similar neuronal changes contribute to patterns of reorganization within the contralateral sensorimotor cortex of chronic human amputees is uncertain. We used functional MRI-guided proton magnetic resonance spectroscopy to test the hypothesis that unilateral deafferentation is associ

The need to treat an abdominal aortic aneurysm (AAA) in kidney transplanted patient is a rare event. To date, no method to protect the kidney during the aneurysm treatment has been identified as undeniably relevant. On the other hand, the advantage of endovascular treatment of the aneurysm (EVAR) is to avoid transplanted kidney injury. Unfortunately, EVAR is not always available leading to open repair and then aortic cross clamping. We report here 3 cases of AAA open repair in kidney transplanted patients using a temporary axillofemoral bypass to protect the renal function.

Even though the hand comprises only 1% of our body weight, about 30% of our central nervous systems (CNS) capacity is related to its control … have functional capabilities that in some respects even outperform biological alternatives, such as hand transplantation. As with replantation and transplantations, the challenge of bionic replacement is connecting the target with the CNS to achieve natural and intuitive control. In recent years, we have developed a number of strategies to improve neural interfacing, …

Multistructural reconstructive procedures are often required to achieve a sufficient reconstruction of the forearm and/or hand after radical tumor resection. Clear margin (R0) resection is the main therapeutic goal for the treatment of sarcomas. Plastic reconstructive procedures with the possibility of microvascular transplantations play a key role in coverage of complex defects. In our department, 20 patients with soft tissue sarcomas of the hand/forearm were treated between 1995 and 2005. Eleven patients were male, nine female. The average follow-up time was 42 months. The most common tumor

At present, there are mainly 5 kinds of free upper arm flaps selected for clinical application: superior-lateral, middle-lateral, inferior-lateral and medial and posterior upper arm flaps. We have performed 17 transplantations using the 5 kinds of the free flaps since 1983, 16 cases with satisfied results. Among the 17 transplantations, 8 flaps were superior-lateral, 4 middle-lateral, 1 inferior-lateral, 2 medial and 2 posterior up arm flaps. The flaps were used for repairing 12 defects on hands, 4 on faces, and 1 on elbow. The maximum size of the flaps is 19 x 11 cm, the minimum 6 x 9 cm. We

INTRODUCTION: Laparoscopic donor nephrectomy is widely used to retrieve a kidney for transplantation. Preoperative evaluation of the donor is of crucial importance to the recipient. In particular, vascular anatomy should be assessed with the help of modern imaging modalities. We present a hand-assisted laparoscopic nephrectomy of a kidney donor with a complete duplex vena cava.
CASE REPORT: A 40-year-old male patient was admitted to our clinic as a kidney donor for his 20-year-old son. After the preliminary tests, further imaging with the use of computerized tomographic angiography showed a c

A patient who had suffered amputations at different levels of his nondominant left hand, and who had initially been treated with reattachment of his index and long fingers, presented with a minimally functional left hand. He was then managed by transplantation of his reattached index finger stump to the shortened ring finger. This significantly improved hand function and contour and alleviated a painful neuroma at the former ring finger amputation site. However, compared with the long finger, which was reattached once, the twice-reattached finger had less sensibility recovery, more pronounced
