Written action plans for asthma: an evidence-based review of the key components.

BACKGROUND: Written action plans for asthma facilitate the early detection and treatment of an asthma exacerbation. Several versions of action plans have been published but the key components have not been determined. A study was undertaken to determine the impact of individual components of written action plans on asthma health outcomes. METHODS: Randomised controlled trials (n=26) that evaluated asthma action plans as part of asthma self-management education were identified. Action plans were classified as being individualised and complete if they specified when and how to increase treatmen

Substance Abuse Screening and Treatment.

One of the more prevalent and often undiagnosed problems seen by primary care clinicians is substance misuse. Resulting in increased morbidity and mortality, loss of productivity, and increased health care costs, substance misuse in our society remains a significant public health issue. Primary care physicians are on the front lines of medical care, and as such, are in a distinctive position to recognize potential problems in this area and assist. This article outlines office-based screening approaches and strategies for managing and treating this complex issue confronting primary care.

U.S. Ebola Treatment Center Clinical Laboratory Support.

Fifty-five hospitals in the United States have been designated Ebola treatment centers (ETCs) by their state and local health authorities. Designated ETCs must have appropriate plans to manage a patient with confirmed Ebola virus disease (EVD) for the full duration of illness and must have these plans assessed through a CDC site visit conducted by an interdisciplinary team of subject matter experts. This study determined the clinical laboratory capabilities of these ETCs. ETCs were electronically surveyed on clinical laboratory characteristics. Survey responses were returned from 47 ETCs (85%)

Evidence and determinants of post-abortion family planning utilization among women of reproductive age in Africa: an umbrella review.

BACKGROUND: Post-abortion family planning (PAFP), ideally initiated within 48 h, is crucial for preventing unplanned pregnancies. Repeat-induced abortion remains a significant challenge to the sexual and reproductive health of women. Despite numerous systematic reviews and meta-analyses, inconsistent findings still hinder effective policy formulation and clinical decision-making. In Africa, high rates of unsafe abortion and unintended pregnancy persist, exacerbated by socioeconomic and health system barriers. This umbrella review consolidates global evidence on the prevalence, determinants, an

Postnatal experiences and perspectives on future pregnancies among women with type 1 diabetes- a qualitative study.

OBJECTIVE: Women with type 1 diabetes (T1D) tend to have fewer children and the postnatal period can be particularly challenging, as the rapid decline in pregnancy-induced insulin resistance often complicates diabetes management. The aim of this study was to explore postnatal experiences of women with T1D and their perspectives regarding future pregnancies. METHODS: This qualitative study is part of the "Reproductive health in diabetes and prediabetes-study". Individual interviews using a semi-structured interview guide were conducted with 17 women with T1D, with or without pregnancy experien

Point of care testing of biochemical markers for monitoring astronauts during long duration missions in deep space.

BACKGROUND: Investigations into human performance and health status during long-duration spaceflights are ongoing aboard the International Space Station (ISS) and are critical for planning missions beyond Low Earth Orbit (LEO). This review evaluates the current evidence on point-of-care-testing (POCT) in space, discussing requirements for POCT to support astronaut health during extended deep space missions and the potential for technology transfer to terrestrial healthcare. CONTENTS: Microgravity disrupts biochemical and hormonal regulation, leading to reversible homeostatic dysregulation acr

Towards resilient and agile health systems: lessons from abrupt donor withdrawal in Jordan.

BACKGROUND: Donor-funded health programmes strengthen national quality systems in low-income and middle-income countries, but abrupt withdrawal of external support can create institutional gaps and threaten service sustainability. AIM: To analyse the impact of sudden donor funding cessation, using the experience of the Health Care Accreditation Council (HCAC) as an illustrative case, and to explore implications for quality infrastructure and health system resilience. METHODS: This structured case-based policy analysis draws on institutional documentation, administrative records and publicly

Unmet Mental Health and Social Service Needs of Formerly Incarcerated Women Living with HIV in the Deep South.

Due to the disproportionate burden of HIV among incarcerated women in the United States, jails and prisons have been identified as key sites for health service delivery. Recidivism remains high, potentially reflecting unmet mental health and social service needs of incarcerated women, especially during the postrelease adjustment period. However, little published research has investigated this possibility directly. We conducted semi-structured, in-depth interviews with previously incarcerated women living with HIV, and other key informants, and completed service-availability mapping in two Alab

Pandemic influenza preparedness in the WHO South-East Asia Region: a model for planning regional preparedness for other priority high-threat pathogens.

Pandemic influenza preparedness has contributed significantly to building, strengthening and maintaining countries' core capacities to prepare for health emergencies. The Pandemic influenza preparedness framework for the sharing of influenza viruses and access to vaccines and other benefits (the PIP framework) was adopted by the World Health Assembly in 2011. The experiences and lessons learnt from the implementation of the PIP framework have provided insights that can be used to strengthen preparedness for epidemics of other priority high-threat pathogens in the World Health Organization (WHO

Hearing loss and executive functions - results from a population-based cohort study.

INTRODUCTION: Hearing loss is linked to cognitive decline, yet its relation to specific executive functions remains unclear. This study examines associations between planning ability, pure-tone audiometry and speech-in-noise performance. METHODS: Data were drawn from the Gutenberg Health Study (GHS), a large population-based cohort at the University Medical Center Mainz. Participants with complete pure-tone audiometry and planning ability data were included (N = 4,458; 2,314 males (51.9%), 2,144 (48.1%) females; mean age 58.0 ± 10.1 years, range 40-80). Audiologic testing included air- and bo

Implementing the leadership development plans of faculty education fellows: a structured approach.

The literature about medical education faculty fellowship programs, which have grown in popularity, quantifies program characteristics, provides exemplars, and reports on delivery strategies. Evaluation is generally limited to satisfaction measures, with a few longitudinal studies of postprogram achievements, but none on the process of making these changes. The authors describe the development of faculty members' postfellowship leadership plans and a structured process to support plan implementation. They also compare the implementation of initiatives specified in individual leadership developm

Mobile solutions to Empower reproductive life planning for women living with HIV in Kenya (MWACh EMPOWER): Protocol for a cluster randomized controlled trial.

BACKGROUND: Women living with HIV (WLWH) face unique reproductive health (RH) barriers which increase their risks of unmet need for contraception, contraceptive failure, unintended pregnancy, and pregnancy-related morbidity and mortality and may prevent them from achieving their reproductive goals. Patient-centered counseling interventions that support health care workers (HCWs) in providing high-quality RH counseling, tailored to the needs of WLWH, may improve reproductive health outcomes. METHODS AND DESIGN: We are conducting a non-blinded cluster randomized controlled trial (cRCT) of a dig

UNICEF supports birth control despite Holy See.

The Vatican has failed in its attempt to prevent the UN Children's Fund (UNICEF) from spreading the message of family planning in developing countries. Over the strong objections of the Holy See, the 41-member Executive Board of UNICEF has asked the agency to cooperate with the UN Population Fund (UNFPA) and the WHO "to support family planning in the context of sustainable national healthcare systems." A member of the Executive Board said the Vatican held the view that UNICEF had no legitimate right to be involved in family planning. "But the Holy See was in a minority of one," he said, adding

The worldwide use of steroidal contraception.

Historically, concerns about rapidly growing populations and resources that could not support them persuaded governments of developing countries to initiate family planning programs. Between the early 1960s and the early 1980s, the number of governments supporting these programs increased from 7 to approximately 120. Today, 52 governments in developing countries provide support to family planning programs because of the demographic rationale, and 65 governments do so because of human rights and health considerations: The effective use of family planning significantly diminishes infant, child,

Let's Talk Birth Control: Study protocol for a cluster randomized trial of a clinic-based contraceptive intervention for adolescents and young adults.

BACKGROUND: Adolescents and young adults (AYA) often lack sufficient information to make informed contraceptive choices, and many are not using their preferred method. Clinical decision aids (CDAs) improve contraceptive knowledge, self-efficacy, and decision-making, but few have been designed for or evaluated with AYA. Let's Talk Birth Control (Let's Talk) is a clinic-based intervention that combines a printed CDA and the digital Bedsider.org Method Explorer with a provider training on person-centered contraceptive counseling. Participant feedback on a health care transition conference for youth with medical complexity.

INTRODUCTION: Each year in the United States, an estimated 1.2 million youth with chronic conditions or functional limitations reach adulthood and must transition from pediatric to adult health care, a process often fragmented by service loss, limited adult providers, and complex waiver and financial systems. The University of Alabama at Birmingham developed the Staging Transition for Every Patient (STEP) Program which includes an annual Healthcare Transition Conference. Here we describe the conference and analyze two years of attendee feedback. METHODS: Conference planning followed an iterat

Exploring stakeholder perceptions of peer support initiatives in the management of diabetes in low- and middle-income countries: An online survey study.

Peer support is an effective strategy to promote self-management behaviors and improve well-being in those with cardiometabolic disease, including type 2 diabetes mellitus (T2DM). There is limited knowledge about stakeholder perceptions regarding peer support programs in low- and middle-income countries (LMICs). The study assessed stakeholders' awareness and understanding of peer support initiatives for T2DM, and explored their perceived barriers and readiness for implementation. A cross-sectional, self-administered online survey with branching logic was distributed to stakeholders across macr
