The use of the Doppler pulmonary artery velocity time integral to optimize placement of a ventricular pacing lead in a patient with Ebstein's anomaly.

Ebstein's anomaly is a rare congenital heart defect. Patients with severe symptomatic tricuspid regurgitation requiring surgical correction often have conduction system disease. We present a case of a 14 year-old girl with Ebstein's malformation and bioprosthetic tricuspid valve who required permanent pacing for symptomatic bradycardia. The placement of the right ventricular pacing lead was facilitated by the use of the Doppler pulmonary artery velocity time integral as a surrogate for stroke volume. This case demonstrates the importance of site-specific pacing and the utility of Doppler echoc

Comparison of His Bundle and Left Bundle Area Pacing in Pediatric and Adult Congenital Heart Disease Patients: A Single-Center Experience.

Conduction system pacing has been shown to offer advantages over traditional RV pacing for maintenance of ventricular function in adult patients requiring chronic ventricular pacing. Tools and implant techniques to target the His bundle (HBP) and left bundle area (LBAP) are designed for adult-size patients with structurally normal hearts. Data regarding procedural characteristics and pacing system performance in pediatric and young adult patients with congenital heart disease are limited. Review the implant characteristics and short-to-intermediate-term pacing parameters of pediatric and young

Cardiac resynchronization therapy in patients with chronic heart failure: pathophysiology and current experience.

Congestive heart failure afflicts 2 to 4 million people in the US and nearly 15 million people worldwide. Accepted goals of heart failure treatment include: improvement of symptoms;prevention of disease progression; and reduction in morbidity and mortality. Complex pharmacological therapies achieve these goals, but not in all patients with heart failure. Cardiac resynchronization therapy (CRT) represents a new therapeutic approach in patients with chronic heart failure. CRT is only applicable to a subgroup of patients with ventricular conduction system delay, characterized by prolonged QRS dur

Relationship of the T-wave Tpeak-Tend interval with conduction system disorders in arterial hypertension.

PURPOSE: The Tpeak-Tend interval of the T wave has emerged as a new electrocardiographic marker of increased transmural dispersion of ventricular repolarization. We aimed to determine the presence of cardiac conduction system disorders in patients with systemic arterial hypertension (SAH) who have altered Tpeak-Tend interval of the T wave. METHODS: The 67 patients with SAH were divided into two groups. Those with prolonged (≥ 77 ms) Tpeak-Tend intervals, 21 (31%) patients were in the study group. Those with normal (< 77 ms) Tpeak-Tend intervals, 46 (69%) patients were in the control group. Al

Interesting phenomena during threshold testing of conduction system pacing: What is the mechanism?

BACKGROUND: Although the interest in conduction system pacing is increasing, the data on longitudinal dissociation in the literature is still limited. METHODS AND RESULTS: We performed left bundle branch (LBB) pacing on a patient with sick sinus syndrome and atrioventricular block. The transition of QRS morphology can be observed during the threshold testing. The main findings were different left ventricular activation times when selective LBB capture was performed at different outputs. CONCLUSIONS: This phenomenon may be due to the anatomical basis of longitudinal dissociation of the His-Pur

A theoretical right atrial pressure feedback heart rate control system to restore physiologic control to the rate-limited heart.

Physiologic control of heart rate may be limited by sinus node injury or congenital conduction defects. Control theory may be applied to restore physiologic rate control. Mean right atrial pressure is a likely candidate for incorporation into an artificial control system because of its: (1) stability at rest; (2) tendency to increase during exertion in the rate-limited heart; (3) ability to decrease with increasing heart rate in the rate-limited heart; and (4) accessibility for measurement via the transvenous route. A theoretical right atrial pressure feedback heart rate control system is pres

[Accessory pathways and supraventricular tachycardia].

Accessory pathways represent abnormalities of atrioventricular conduction. According to the course and the site of insertion they can be divided into atrioventricular (AV), atrio-His, atriofascicular, nodoventricular, nodofascicular and fasciculoventricular. AV accessory pathways are the most common form. An anterior, lateral and posterior localisation of AV accessory pathways can be distinguished in the left and right free wall. Atriofascicular accessory pathways usually exhibit antegrade conduction with AV node-like decremental properties. Right atriofascicular accessory pathways are involve
