Four hundred eight-five fetal monitor patterns were reviewed to correlate the incidence of postparacervical fetal bradycardia with the preceding fetal heart rate pattern. Three hundred and one patients received a total of 408 paracervical blocks, with an overall incidence of bradycardia of 11.3%. Patients with reassuring patterns had an incidence of 8.4%, those with concerning patterns 20%, and those with very concerning patterns 39.1%. Restricting the use of paracervical anesthesia to patients with reassuring fetal heart rate patterns should minimize this complication of obstetric anesthesia.

Periodic breathing is common in normal infants, but may be associated with prolonged apnea leading to crib death. The mechanisms of periodic breathing and its relation to normal breathing patterns are unclear. Fetal heart rate (FHR) decelerations resembling post-paracervical block (PCB) bradycardia were observed immediately after the inadvertent intrauterine injection of lidocaine hydrochloride. A review of the pathophysiology of post-PCB bradycardia is presented. The administration of a local anesthetic, either into the paracervical tissues or directly into the uterine cavity, may precipitate FHR decelerations principally, through arterial vasoconstriction with subsequent reflex responses and/or hypoxia in the fetus.

One hundred and one electrocardiographs (ECGs) were performed on 38 babies, before, during and after gavage feeds. Forty ECGs taken from 22 babies showed a significant bradycardia occurring after tube passage. The risk of this occurring appeared higher with preterms and with babies below 2.5 kg. It is recommended that gavage feeding be restricted to babies above 2.5kg.

Heart rate variability on 24-hour electrocardiographic recording was assessed in 23 patients without structural heart disease before and after 2 months of oral treatment with verapamil prescribed for paroxysmal atrioventricular nodal reentrant tachycardia. Verapamil had no significant effect on overall heart rate variability in the frequency domain, but it increased ultra low frequency power and decreased the low-frequency/high-frequency ratio, deemed to be a marker of sympathetic activity.

The aim of the present study was to test the relationships between hemorheology and autonomic nervous system (ANS) activity in 23 sickle cell trait (SCT) carriers and 17 control subjects.
