Absence of association between length variation of an intronic region in the NFATC1 gene and congenital heart defects in a Han Chinese population.

Congenital heart defects are complicated birth defects due to the interaction of genetic and environmental factors. Previous research indicated the importance of transcription factors in heart development, which suggested that mutations of transcription factor genes could be genetic determinants of congenital heart defects. Recently, the length variation of an intronic region in the NFATC1 gene was linked to ventricular septal defect (VSD). In this study, we detected the length variation of the region in a Han Chinese population of patients with nonsyndromic VSD, atrial septal defect, patent d

Transcatheter Closure of Atrial Septal Defect in Children: Single-Center Experience, Mid-Term Follow-up Results.

OBJECTIVE: Atrial septal defect is a congenital heart disease usually diagnosed in childhood. This study aimed to evaluate the mid-term follow-up results of patients who underwent trans- catheter closure of atrial septal defect by comparing the devices and methods used in the procedure and investigating the complications of this procedure in children. MATERIALS AND METHODS: This study evaluated 232 patient files retrospectively. Of the 232 patients, 24 were excluded from the study due to missing files or data. Also, patients with multi-fenestrated atrial septal defect and aneurismatic septal

Optimal timing of surgery in common left to right shunts.

Left to right shunts like atrial septal defect, ventricular septal defect and patent ducts arteriosus are commonly encountered congenital malformations of the heart. With advances in diagnostic and therapeutic technology over the last three decades the recognition and management of these lesions has markedly improved. Currently, the surgical procedures are being performed in infants and neonates with the hope that long term results would be better. Children with atrial septal defects are usually asymptomatic or mildly symptomatic and closure of the defect is best performed at 3-4 years of age.

[Effective long-term noninvasive ventilation of a woman with kyphoscoliosis and sinus venosus atrial septal defect].

Atrial septal defects comprise one of the most common congenital cardiac malformations in adults and adolescents. However, such septal defects are rarely associated with chest wall diseases. When they are, respiratory failure soon develops and is severe. Cases in which the long-term course of disease is favorable with noninvasive ventilation have not been reported in the literature. We present the case of a 71-year-old woman with kyphoscoliosis, sinus venosus atrial septal defect, severe pulmonary hypertension, respiratory failure, and heart failure. We describe her response to treatment with

A case of mixed drainage total anomalous pulmonary venous return in an adult patient.

Total anomalous pulmonary venous return is observed in 1.5 to 3% of congenital heart diseases. Urgent surgical intervention is required following the diagnosis, since severe heart failure is accompanied by cyanosis from the first days of life in most of the patients. This rare congenital heart disease is often managed in adulthood with surgery. Patients who survive until adulthood without a surgical intervention have a large atrial septal defect. A 36-year-old female patient was admitted with complaints of fatigability, shortness of breath, and cyanosis of the lips. Total anomalous pulmonary v

Chronotropic impairment after surgical or percutaneous closure of atrial septal defect.

BACKGROUND: In previous studies, an attenuated heart rate response to exercise has been noted in patients after surgical closure of atrial septal defect. The aim of this study was to compare the prevalence of chronotropic impairment after surgical and percutaneous closure of atrial septal defect. METHODS: Thirty-eight pediatric patients who underwent a surgical (group A, n = 18) or transcatheter closure (group B, n = 20) of atrial septal defect in our institution were prospectively included in the study. Treadmill exercise testing was performed using the Bruce walking treadmill protocol to vo

Surgical treatment of amplatzer embolus in a secundum atrial septal defect patient.

A secundum atrial septal defect is the most common congenital heart defect. Transcatheter treatment of secundum atrial septal defects is a popular and less invasive alternative to surgery. Procedural complications may occur in a wide spectrum, particularly device embolus as the most emergent one, but luckily they do not commonly occur in the clinical setting. Mortality from adverse events related to transcatheter treatment strategies is twentyfold higher than that of primary elective surgical closure. Here, we report an Amplatzer device embolus in a secundum atrial septal defect patient. The d

Distinguishing Septal Heart Defects from the Valvular Regurgitation Using Intelligent Phonocardiography.

This paper presents an original machine learning method for extracting diagnostic medical information from heart sound recordings. The method is proposed to be integrated with an intelligent phonocardiography in order to enhance diagnostic value of this technology. The method is tailored to diagnose children with heart septal defects, the pathological condition which can bring irreversible and sometimes fatal consequences to the children. The study includes 115 children referrals to an university hospital, consisting of 6 groups of the individuals: atrial septal defects (10), healthy children

Double outlet right ventricle with severe left ventricular outflow tract obstruction due to small ventricular septal defect and anomalous adherence of the mitral valve to the ventricular septum.

A 1-month-old male infant with a double outlet right ventricle complicated by left ventricular outflow tract obstruction due to a very small ventricular septal defect is described. The atrial septum was intact, but mitral stenosis was present. The characteristic finding in this patient was that the inherently small ventricular septal defect was further narrowed by abnormal adherence of the anterior mitral leaflet to the left ventricular septal surface below the defect. Intracardiac anatomical features other than the adherent mitral valve could be evaluated accurately by two-dimensional echocar

Patent foramen ovale: assessment, clinical significance and therapeutic options.

Foramen ovale plays an important function in the fetus but is of no physiologic significance after birth and closes in most individuals. In about one fourth of the population, however, foramen ovale remains open for life and has been associated with cerebrovascular accidents, especially in younger patients, presumably through paradoxical embolism. Patent foramen ovale (PFO) has also been associated with hypoxia, migraine headaches and neurologic findings of decompression illness in scuba divers. Availability of transesophageal echocardiography and its frequent use in the management of patients

Atrioventricular septal defect in prenatal life.

In a series of more than 3500 pregnancies referred for fetal echocardiography, 29 cases of atrioventricular septal defect were detected in the fetus. There was a chromosomal anomaly in 14 of these cases, left atrial isomerism in 12, and right atrial isomerism in two. Complete heart block was found in 11 of the cases with left atrial isomerism. Many associated cardiac abnormalities were found, particularly in the fetuses with atrial isomerism; the most common were double outlet right ventricle or aortic arch anomalies. The prognosis was poor in all patients with atrioventricular septal defect d

The role of abnormal subaortic morphometry as a substrate for left ventricular outflow tract obstruction following atrioventricular septal defect repair.

OBJECTIVES: Although left ventricular outflow tract (LVOT) obstruction is a recognized risk after atrioventricular (AV) septal defect (AVSD) repair, quantitative assessments to define the substrate of the obstruction are lacking. METHODS: Morphometric analyses were based on measurements from early 2-dimensional echocardiographic scans (within 3 months postoperatively) for 117 patients (82 CAVVO = common AV valve; 35 SAVVO = separate AV valve orifices), which were compared to 50 age/weight matched controls (atrial septal defect/ventricular septal defect). Late echocardiographic analyses were p

Hemodynamics in congenital heart disease.

This paper introduces a very general and flexible model for the study of hemodynamic changes in congenital heart disease. The generality of the model makes it possible to use the same computer program (which is included in an Appendix) to study both the fetal circulation and the adult circulation, as well as such diverse disease states as patent ductus arteriosus, ventricular septal defect, atrial septal defect, tetralogy of Fallot and transposition of the great arteries. In this paper, only patent ductus and ventricular or atrial septal defect are studied, with special emphasis on the influen

[Surgical treatment of secondary atrial septal defect associated with mitral valve disease].

From March 1965 through August 1991, 703 patients with secondary atrial septal defect were treated surgically in our hospital. Among them, 32 cases (4.6%) were associated with mitral valve disease. The mitral valve lesion was repaired at the same time of closure of the atrial septal defect. The operative mortality was 6.3% and there was no late death. Follow-up study showed that heart function of the patients was improved obviously and most of them have resumed work or study. The clinical features, incidence, etiology, pathology and surgical techniques of secondary atrial septal defect associa

A case report of percutaneous successful closure of multiple atrial septal defect.

A 50-year-old male patient with a secundum-type atrial septal defect (ASD) suffering from respiratory distress for 7 days was referred to our hospital. Transthoracic echocardiography demonstrated the defects of 9 mm and 4 mm in the middle atrial septum in the parasternal four-chamber view. However, we found not 2, but 6 atrial defects by using real time 3-dimensional transesophageal echocardiography. Open heart surgery was not indicated, because his pulmonary function was poor. After fully informed consent, we decided to treat ASD with Amplatzer septal occluders.
