It keeps on turning: Effects of prolonged long-term left ventricular assist device support as a bridge to heart transplantation.

OBJECTIVES:: Increasing incidence of end-stage heart failure has moved the therapy with left ventricular assist devices to the forefront of surgical treatment. Moreover, continuous sophistication in this technology has resulted in increasing proportion of patients on prolonged support. Early and late complications after left ventricular assist device as a bridge to transplantation and present factors associated with long-term support and long-term outcomes of patients supported for at least 1 year were compared. METHODS:: A total of 163 consecutive patients who underwent left ventricular assi

Ultrasonic sensor concept to fit a ventricular assist device cannula evaluated using geometrically accurate heart phantoms.

Future left ventricular assist devices (LVADs) are expected to respond to the physiologic need of patients; however, they still lack reliable pressure or volume sensors for feedback control. In the clinic, echocardiography systems are routinely used to measure left ventricular (LV) volume. Until now, echocardiography in this form was never integrated in LVADs due to its computational complexity. The aim of this study was to demonstrate the applicability of a simplified ultrasonic sensor to fit an LVAD cannula and to show the achievable accuracy in vitro. Our approach requires only two ultrason

A case of radiotherapy for an advanced bronchial carcinoma patient with implanted cardiac rhythm machines as well as heart assist device.

We present a case of radiotherapy for a 66-year-old patient with squamous cell carcinoma on the left main bronchus undergoing implantation of pacemaker, implantable cardioverter defibrillator (ICD) as well as cardiopulmonary support (CPS) device. The radiation area was determined according to 4D List Mode positron emission tomography-computed tomography (PET-CT) data. Planning Target Volume (PTV) included a part of the active ICD. For the optimal tumor coverage and sparing of both the implantable cardiac devices and organs at risk, we combined the conformal radiotherapy with stereotactic body

Survival on the heart transplant waiting list: impact of continuous flow left ventricular assist device as bridge to transplant.

BACKGROUND: Continued donor organ shortage and improved outcomes with current left ventricular assist device (LVAD) technology have increased the number of patients supported with bridge-to-transplantation (BTT) therapy. Using the United Network of Organ Sharing (UNOS) database, we assessed the impact on survival in patients supported with BTT while on the heart transplant waiting list. METHODS: The UNOS database was queried from January 2005 to June 2012 to identify patients listed for heart transplantation as UNOS status 1A or 1B. Patients implanted with a pulsatile-flow device or an LVAD o

Mechanical ventricular assist device as a bridge to recovery post-ABO-incompatible heart transplantation for failed Fontan circulation.

A girl received an ABO-incompatible heart transplantation (ABOiHTx) at the age of 3.5 years for failed univentricular palliation with protein-losing enteropathy (PLE). She was born with a hypoplastic left heart syndrome and underwent multistage palliation to a Fontan circulation at 2½ years of age. After the Fontan surgery, she developed PLE, necessitating a Fontan revision, followed by a Fontan takedown and eventually HTx, which was performed with a blood group B heart into an O recipient. Right ventricular (RV) failure secondary to increased pulmonary vascular resistance (PVR) evolved immedi

Electrophysiologic characteristics and catheter ablation of ventricular tachyarrhythmias among patients with heart failure on ventricular assist device support.

BACKGROUND: Ventricular tachyarrhythmias (VT) are common among ventricular assist device (VAD) recipients, yet electrophysiologic (EP) characteristics and catheter ablation outcomes remain uncharacterized. OBJECTIVE: To evaluate the EP characteristics and catheter ablation outcomes for VTs among heart failure patients on VAD support. METHODS: The Cleveland Clinic registry of consecutive patients undergoing VAD placement in 1991-2010 with medically refractory, symptomatic VT referred for EP study and catheter ablation. RESULTS: Among 611 recipients of VAD (mean age 53.3 ± 12.4 years, 80% men

Case of fulminant giant-cell myocarditis associated with polymyositis, treated with a biventricular assist device and subsequent heart transplantation.

Giant-cell myocarditis is an autoimmune myocarditis that rapidly progresses to heart failure, and is often associated ventricular tachycardia. We describe an otherwise healthy patient who was acutely ill with decompensated heart failure and ventricular tachycardia associated with rash and polymyositis, who then developed cardiogenic shock and multiorgan failure due to giant-cell myocarditis.

Multiple splenic infarctions due to septic embolization in a patient supported with Berlin Heart-EXCOR biventricular assist device.

Patients awaiting cardiac transplantation often require ventricular assist devices (VADs) due to instable circulatory condition. Device-related infections remain the 'Achilles heel' of mechanical circulatory support and wound complications after VAD placement are a formidable challenge to surgeons. Up to 25% of deaths in VAD patients are due to systemic sepsis, which occurs in 11-26% of patients and bacteria that are common pathogens including Staphylococcus, Pseudomonas, Enterococcus, and Candida. Sepsis, infection of implanted pump components, and infections of percutaneous drivelines contin

[Rare case of enterovirus myocarditis with consecutive acute heart failure and subsequent "rescue therapy" with biventricular assist device].

UNLABELLED: PATIENT HISTORY AND FINDINGS: A 37-year-old woman was admitted in cardiogenic shock and multiorgan failure. On echocardiography, left ventricular function was reduced, at that time, to 35%. The patient had been suffering from a cold for 1 week prior to admission. Within 24 h, left ventricular function dramatically decreased to 7%. Examinations included left-and right-heart catheter evaluation with removal of right ventricular biopsies. Histology and molecular pathology revealed the diagnosis of an enteroviral myocarditis. THERAPY: Imminently, a biventricular assist device (BVAD -B

Role of assist device implantation and heart transplantation in the long-term outcome of patients with structural heart disease after catheter ablation of ventricular tachycardia.

INTRODUCTION: Ablation of ventricular tachycardias (VTs) in patients with structural heart disease (SHD) has been associated with advanced heart failure and poor survival. METHODS AND RESULTS: This matched case-control study sought to assess the difference in survival after left ventricular assist device (LVAD) implantation and/or heart transplantation (HTX) in SHD patients undergoing VT ablation. From the initial cohort of 309 SHD patients undergoing VT ablation (187 ischemic cardiomyopathy, mean age 64 ± 12 years, ejection fraction of 34 ± 13%), 15 patients received an LVAD and nine patient

Outcome for children following admission to hospital with a first episode of heart failure, due to heart muscle disease, in the ventricular assist device (VAD) era.

AIMS: Most reports on the outcome of children who present with heart failure, due to heart muscle disease, are from an era when ventricular assist devices were not available. This study provides outcome data for the current era where prolonged circulatory support can be considered for most children. METHODS & RESULTS: Data was retrieved on 100 consecutive children, who presented between 2010 - 2016, with a first diagnosis of unexplained heart failure. Hospital outcome was classified as either death, transplantation, recovery of function or persistent heart failure. Median age at presentation

Refractory heart failure dependent on short-term mechanical circulatory support: what next? Heart transplant or long-term ventricular assist device.

Chronic heart failure is a progressive and eventually fatal illness. Although the disease cannot be cured and treatment is symptom oriented, most of the patients benefit from optimum medical treatment. Patients with rapid deterioration in chronic advanced heart failure refractory to medical treatment need inotropic support and may need intra-aortic balloon pump to maintain circulatory support, which of course cannot be prolonged beyond a certain limit. The outcome of heart transplant and long-term ventricular assist device (VAD) in such patients is poor. The short-term mechanical circulatory s

Impact of Heart Allocation Change on Selection and Adverse Events of Bridged-To-Transplant Durable Ventricular Assist Device Recipients.

BACKGROUND: The 2018 UNOS heart allocation reform significantly altered the waiting list priority for recipients of durable left ventricular assist devices (LVADs). We sought to compare the characteristics of patients who were bridge-to-transplant (BTT) or bridge-to-candidacy (BTC) with a durable LVAD before and after the UNOS allocation change. METHODS: Data was retrieved from the Society of Thoracic Surgeons Interagency Registry for Mechanically Assisted Circulatory Support. Adult patients implanted with a continuous-flow, fully magnetically levitated (CF-ML) LVAD as a BTT or BTC from Augus

A case report: corheart 6 biventricular assist device therapy for end-stage heart failure in dilated cardiomyopathy.

Approximately one-third of patients with end-stage heart failure (ESHF) experience biventricular failure. Left ventricular assist devices (LVADs) are employed as a salvage therapy for individuals with advanced heart failure. The onset of right ventricular failure complicates the postoperative management of patients receiving LVAD support. Currently, no right ventricular assist device is specifically designed for isolated right heart failure support, necessitating cardiac surgeons worldwide to adopt various improvised methods using LVADs for right ventricular assistance. This report details the

Vericiguat Use in Right Heart Failure Management Following Left Ventricular Assist Device Implantation: A Case Report.

Right heart failure (RHF) is a major cause of mortality in patients using left ventricular assist devices (LVADs). Vericiguat, an oral soluble guanylate cyclase stimulator, has recently been used to treat chronic heart failure (HF) with a reduced ejection fraction (EF) and is expected to improve RHF owing to its pulmonary vasodilatory and inotropic properties in patients with an LVAD. A 62 year old woman with advanced HF due to cardiac sarcoidosis was transferred to our center with HF recurrence and left ventricular (LV) thrombosis. Since her status became inotropic-dependent post-thrombectomy

"The heart in a bag": The lived experience of patient-caregiver dyads with left ventricular assist device during cardiac rehabilitation.

OBJECTIVE: The Left Ventricular Assist Device (LVAD) has increasingly become a primary therapeutic option for longer-waiting heart transplant lists. Although survival rates are growing, the device requires complex home care. Therefore, the presence of a caregiver trained in the LVAD management is important for the success of the therapy. The LVAD leads both patients and their caregivers to experience new challenges and adapt to new lifestyle changes and limitations - but their subjective beliefs before home management remained little explored. DESIGN: This study identified, using a phenomenol

Left Ventricular Assist Device Evaluation for Congestive Heart Failure With Hypereosinophilic Syndrome in a 76-Year-Old Male.

Hypereosinophilic syndrome, a rare condition, is characterized by eosinophilia with associated organ infiltration and organ failure. The most commonly involved sites include the cardiovascular, gastrointestinal, and central nervous systems. Infiltration of eosinophils into the myocytes may result in congestive heart failure warranting the use of a left ventricular assist device (LVAD). The authors describe a case of a 76-year-old male with dyspnea and a consistently elevated eosinophil count. Initially diagnosed with adult-onset asthma, he failed to improve despite his adherence to recommended
