The Anglo-Scandinavian Cardiac Outcomes Trial lipid lowering arm: extended observations 2 years after trial closure.

AIMS: To determine the cardiovascular benefits in those originally assigned atorvastatin in the Anglo-Scandinavian Cardiac Outcomes Trial-2.2 years after closure of the lipid-lowering arm of the trial (ASCOT-LLA). METHODS AND RESULTS: The Blood Pressure Lowering Arm of the ASCOT trial (ASCOT-BPLA) compared two different antihypertensive treatment strategies on cardiovascular outcomes. ASCOT-LLA was a double-blind placebo-controlled trial of atorvastatin in those enrolled into ASCOT-BPLA with total cholesterol concentrations at baseline of < or =6.5 mmol/L. A total of 19 342 hypertensive patie

[Stroke due to spontaneous calcified cerebral embolus as presenting feature of calcified aortic stenosis].

We report a case of calcified aortic stenosis revealed by an ischemic stroke. An 81-year-old man with hyperlipidemia, hypertension and renal function impairment presented with acute onset of right-sided hemiparesis and aphasia. Head CT scan revealed a rounded calcified high-density mass within the distal segment of the left anterior cerebral artery, consistent with a calcified cerebral embolus, and an infarct in the left paracentral lobule. Transesophageal echocardiography demonstrated a sclerotic aortic valve. The patient was discharged from hospital on aspirin and atorvastatin, and the outco

[Difficulties in detecting the causative agent of an adverse drug reaction in a complicated case].

Due to the great variety of drug-induced adverse side-effects, the causative agent could not be easily determined. Improvement of the clinical picture after cessation of the causative agent, the clinical observations, as well as data given in medical literature might provide useful information for the imputability of a drug. The diagnosis of drug hypersensitivity is difficult, and must be carefully considered. It is needed in cases when the causative agent cannot be replaced by another in the same pharmacological family of the drug and when the patient is in absolute need of the medicine. The

LC and LC-MS methods for the investigation of polypills for the treatment of cardiovascular diseases. Part 1. Separation of active components and classification of their interaction/degradation products.

"Polypill" is a fixed-dose combination (FDC) containing three or more drugs in a single pill. The same is under development for the treatment and prevention of cardiovascular diseases. In the present study, gradient LC methods were developed for simultaneous determination of the possible components of a polypill, i.e., lisinopril, aspirin and one each among atenolol/hydrochlorothiazide and atorvastatin/simvastatin/pravastatin, in the presence of a total of 13 major interaction/degradation products. The drugs and the products were well separated using a reversed-phase (C-8) column and a mobile

Effects of a potent and selective PPAR-alpha agonist in patients with atherogenic dyslipidemia or hypercholesterolemia: two randomized controlled trials.

CONTEXT: Fibrates are weak agonists of peroxisome proliferator-activated receptor alpha (PPAR-alpha). No trials have reported effects of more potent and selective agents. OBJECTIVES: To examine the safety and efficacy of LY518674, a PPAR-alpha agonist. DESIGN, SETTING, AND PARTICIPANTS: Two multicenter, randomized, double-blind, placebo-controlled trials: 1 in patients with elevated triglycerides and low HDL-C (atherogenic dyslipidemia), the other in patients with elevated LDL-C (hypercholesterolemia). Between August 2005 and August 2006, the dyslipidemia study randomized 309 patients at US

Effects of early administration of atorvastatin treatment on thrombotic process in normocholesterolemic patients with unstable angina.

BACKGROUND: Although statin-treatment during the acute phase of unstable coronary syndromes improve the outcome their effects on thrombosis/fibrinolysis system in normocholesterolemic patients admitted with unstable angina remain obscure. We assessed the effects of short-term atorvastatin treatment on thrombotic/fibrinolysis markers in normocholesterolemic in patients with unstable angina. METHODS: Forty-five patients with unstable angina were allocated into two groups to receive atorvastatin 10 mg/day (n = 24) or no statin (n = 21) for 6 weeks. Treating to hypertension targets.

Control of blood pressure levels in hypertensive patients often fails to meet the targets suggested by the British Hypertension Society (BHS) guidelines. The optimal treatment target for non-diabetic patients is < or = 140/85 mm Hg. Data from the health survey for England show that in 1998 only 8% of men and 11% of women had blood pressure controlled to < 140/90 mm Hg. The Anglo-Scandinavian cardiac outcomes trial (ASCOT) data show that 87% of non-diabetic patients in the St Mary's Centre are currently treated to below 140/90 mm Hg. These differences can be explained by strict protocol adheren

Absorption of short and medium chain fatty acids in the jejunum of the rat.

The uptake of the shortest six fatty acids (acetic to octanoic) was studied in vitro, using everted segments of rat jejunum. The marked influence of medium-pH and fatty acid chain-length suggests that non-ionic diffusion through the lipoid membrane is quantitatively the most important way of transport, but ionic diffusion through the membrane as well as transport through hydrophilic pores also seem to play a role. Though fatt acids evidently are accumulated in the tissue-fluid, and saturation kinetics, competitive inhibition and sodium- as well as energy-dependence apparently are observed, the

BML-111 inhibit H2O2-induced pyroptosis and osteogenic dysfunction of human periodontal ligament fibroblasts by activating the Nrf2/HO-1 pathway.

BACKGROUND: Periodontitis is a common and harmful chronic inflammatory oral disease, characterized by the destruction of periodontal soft and hard tissues. The NLRP3 inflammasome-related pyroptosis and human periodontal ligament fibroblasts (hPDLFs) osteogenic dysfunction are involved in its pathogenesis. Studies have shown that lipoxin A4 is an endogenous anti-inflammatory mediator and BML-111 is a lipoxin A4 analog, which was found to have potent and durable anti-inflammatory effects in inflammatory diseases, but the mechanism remains unclear. The purpose of this study was to investigate whe

Effect of intensive versus moderate lipid-lowering therapy on epicardial adipose tissue in hyperlipidemic post-menopausal women: a substudy of the BELLES trial (Beyond Endorsed Lipid Lowering with EBT Scanning).

OBJECTIVES: This study sought to evaluate the effect of intensive and moderate statin therapy on epicardial adipose tissue (EAT). BACKGROUND: EAT has been associated with coronary artery disease severity and outcome. It is currently unknown whether EAT volume changes over time when patients are exposed to statin therapy. METHODS: Subanalysis of a randomized study of atorvastatin 80 mg/day versus pravastatin 40 mg/day for 1 year in a clinical trial designed to assess the progression of coronary artery calcium (CAC) in hyperlipidemic post-menopausal women. Patients underwent cardiac computed t

Statin-associated polymyositis following omeprazole treatment.

Statins are an extensively used class of drugs, and myopathy is an uncommon, but well-described side effect of statin therapy. Inflammatory myopathies, including polymyositis, dermatomyositis, and necrotizing autoimmune myopathy, are even more rare, but debilitating, side effects of statin therapy that are characterized by the persistence of symptoms even after discontinuation of the drug. It is important to differentiate statin-associated inflammatory myopathies from other self-limited myopathies, as the disease often requires multiple immunosuppressive therapies. Drug interactions increase t

Effects of statins on the recovery of olfactory function in a 3-methylindole-induced anosmia mouse model.

BACKGROUND: Despite the importance of olfactory function, no effective medications have been identified to treat olfactory disorders. This study was performed to evaluate the functional recovery of olfaction damaged by 3-methylindole (3MI) in a mouse model with hydroxymethylglutaryl-coenzyme A reductase inhibitors (statins). METHODS: In a randomized placebo-controlled trial, 24 healthy female BALB/c mice (aged 9-10 weeks and weighing 18-20 g each) were randomly allocated to statin-treated or control groups. Olfactory loss was induced by i.p. injections of 3MI. Atorvastatin (10 mg/kg) or norma

Study of Atorvastatin in experimental allergic airway inflammation in mice.

BACKGROUND: Inflammation and oxidative stress are associated with airway diseases. There is growing evidence that Atorvastatin could be used as a therapy for these conditions. OBJECTIVE: On these bases, we evaluated Atorvastatin as a protective and reversal treatment for the allergic airway diseases in mice model. We also looked at the possible interaction with the currently used effective medication. METHODS: Mice were sensitized and challenged with ovalbumin (OVA) to develop features of allergic airway diseases mainly of bronchial inflammation. Atorvastatin was injected during or after the
