OBJECTIVE: To discuss the main steps of renographic technique and underline some pitfalls related to the acquisition, processing and interpretation of diuretic renography, an important method for guiding the management of asymptomatic congenital hydronephrosis.

We report a case of appendiceal abscess complicated with right hydronephrosis in a 67-year-old woman. She visited our department complaining of right flank pain. Between 1986 and 1995, 24 newborn and young infants underwent to surgical repair of 32 renal units affected by congenital hydronephrosis in spite of their levels of renal function and dilation. 22 renal units were pelviureteral junction obstruction and 10 were vesico-ureteral obstruction. Hereditary hydronephrosis is a rare condition but several families are described in the literature. The inheritance pattern is autosomal dominant (McKusick number 143400) but the exact aetiology of the hydronephrosis is not clear. However, linkage with the HLA region on chromosome six has been shown previously. We report a family not showing linkage to this region, giving further evidence of genetic heterogeneity in this condition.

PURPOSE: We evaluated clinical outcomes after attempted prenatal intervention in fetuses with hydronephrosis.
MATERIALS AND METHODS: We retrospectively reviewed the histories of 10 fetuses considered for prenatal intervention between 1984 and 1993. One female and 8 male fetuses had bilateral hydroureteronephrosis and profound or progressive oligohydramnios, and 1 male fetus had massive progressive unilateral hydronephrosis.

The incidence, cause and significance of hydronephrosis in patients with renal ectopia are unknown. Therefore, we conducted a retrospective review of 77 patients with 82 ectopic kidneys. Of 82 kidneys 46 (56%) had hydronephrosis. Dilatation of the renal pelvis was the result of primary ureteropelvic or ureterovesical obstruction in 52%, grade 3, 4 or 5 vesicoureteral reflux in 26% and extrarenal collecting systems with malrotation that produced apparent ureteropelvic junction obstruction in 22%. One hundred patients with hydronephrosis were treated for 10 years. In 88 patients operations confirmed the causes of obstruction including pyelo-ureteral junction obstruction (pujo, 46 cases), anomalous (15), retroperitoneal fibrosis (15), ureteral polyps and valves and others (12). Six adult patients underwent renal ablation with absolute ethanol for nonfunctioning hydronephrosis due to various ureteral lesions. Twelve fetuses with renal pelvis dilatation < 15 mm have been followed up until a year after birth. Dilatation cleared up in 59% of the cases within the last weeks of pregnancy and in 33% in the first months of life, while was asymptomatically detected in one case only a year after birth. A grade I and II vesico-ureteral reflux was diagnosed in two babies. The results of operative treatment of 110 children with congenital hydronephrosis are presented. Conventional surgical treatment was ineffective in 38.5% of the patients. Complex investigation of a function of the kidney involved was performed with the use of computed renangiography. Development and use of segmental-selective resection of a kidney, pyelouretheral segment and renal pelvis permitted to decrease the incidence of unsatisfactory results more than 2-fold.
