INTRODUCTION: The majority of diffuse idiopathic skeletal hyperostosis (DISH) involving the anterior margin of the cervical vertebrae is asymptomatic, but it can cause dysphagia. Improvements in swallowing after surgical treatment have been reported in several case series. However, the appropriate amount of osteophyte resection for this disease in terms of the pathophysiology of dysphagia is still unknown. The current report describes the appropriate surgical procedure for dysphagia secondary to anterior cervical hyperostosis, and discusses the etiology of dysphagia.
METHODS: This is a retros…

OBJECTIVE: The purpose of this study is to characterize sacroiliac joints (SIJs) findings at CT of patients with diffuse idiopathic skeletal hyperostosis (DISH), a condition characterized (using the Resnick classification criteria) by ossification of at least four contiguous vertebrae in the thoracic spine and preserved disk space, but without radiographic evidence of intraarticular SIJ abnormalities.
MATERIALS AND METHODS: Pelvic CT examinations of 104 patients with DISH (fulfilling the Resnick criteria on spinal CT) and 106 age- and sex-matched control subjects whose entire spine lacked CT …

This report described radiographic and CT features of atypical diffuse idiopathic skeletal hyperostosis in a 5 year-old, female Shih-tzu showing marked proliferative bone lesions in the appendicular skeleton with minor spinal changes. Continuous or flowing bony bridge formation of vertebrae is used as the gold standard for diagnosing diffuse idiopathic skeletal hyperostosis. However, this criterion seems not to be suitable for appendicular type diffuse idiopathic skeletal hyperostosis, as in the present case. Diffuse idiopathic skeletal hyperostosis is a progressive skeletal disease, and thus,…

Both spondylosis and diffuse idiopathic skeletal hyperostosis (DISH) are prevalent in humans and are considered distinct entities. Nowadays, the term spondylosis is in the biomedical literature mostly used when concurrently degenerative disc disease is present. In companion animals, many reports on spondylosis, often without intervertebral disc degeneration, are described. The nomenclature and the definitions of both spondylosis and DISH in biomedical and veterinary literature should be more in line to facilitate comparison. Spondylosis and DISH occur in dogs spontaneously and can co-occur in …

A 4-month-old intact female English Springer Spaniel presented to the University of Georgia Veterinary Teaching Hospital for evaluation of u … integrity, demonstrated normal reactive trabecular bone with locally extensive fibrosis. Calvarial hyperostosis was diagnosed based upon appearance on imaging, lesion unilaterality, absence of mandibular involvement, and histopathology. DISH (Diffuse idiopathic skeletal hyperostosis) of cervical spine is a rare condition which causes dysphagia in 23% of cases and occasionally dyspnea. The authors report the case of a 74 years old male, known with progressive dysphagia and recurrent episodes of dysphonia and dyspnea, who suffered a sudden episode of respiratory distress that need finaly tracheotomy after ineffective attempts of orotracheal intubation.

We report five cases of dysphagia caused by diffuse idiopathic skeletal hyperostosis or Forestier's disease of the cervical spine observed by clinical evaluations, X-ray and videofluoroscopy. Dysphagia in particular was present for solid foods in all patients. Three patients showed bolus aspiration into the airways, mainly in the post-swallowing phase and one patient with preserved epiglottic tilt showed post-swallowing penetration. The physiopathogenetic mechanisms affecting swallowing dynamics were mechanical compression of the pharyngeal lumen associated with abnormal epiglottic tilt, incom…

The authors report on the case of a 14-year-old boy who presented with bilateral visual impairment due to optic canal stenosis caused by hyperplasia of the bone marrow arising from anemia. The patient had hereditary hemolytic anemia with unstable hemoglobin of the Christchurch type. This congenital form of anemia caused hyperplasia of the bone marrow as well as hyperostosis of the entire calvarial bone, which in turn led to optic canal stenosis. The patient underwent surgical decompression of the optic canal, resulting in significant improvement in visual acuity. Pathological findings in the c…

Diffuse idiopathic skeletal hyperostosis (DISH) is a common disorder among older adults. It is characterized by ossification of the anterior longitudinal ligament of the spine and various extra-spinal ligaments. Although stiffness and decreased range of motion of the spine are the most common clinical presentations of DISH, extra-skeletal manifestations may also be present. We report the case of a 65-year-old man complaining of progressive dysphagia due to DISH. Barium swallow showed compression and lateral displacement of the cervical tract of the esophagus, secondary to compression by a larg…

BACKGROUND: The parietal and frontal bones are rarely affected in infantile cortical hyperostosis.
CASE REPORTS: Case n. 1: A 14-day-old boy developed a swelling of the left eyelid that extended to the face in a few days. It was tender and associated with fever. Laboratory findings were increased (RBC sedimentation rate and other signs of inflammation). CT scan of the sinuses, soft tissues and bones was normal, but at the age of 23 days minimal patches of rarefaction were seen on X-rays of the frontal bones. These lacunar areas gradually extended, with the inflammation persisted. A new CT sca…

The first reported Spanish family with autosomal dominant endosteal hyperostosis is presented and two members in two different generations studied. Neurological involvement with sensorineural hearing loss, chronic intracranial hypertension, and mild corticospinal tract abnormalities were found in one case with radiological evidence of progressive bone disease at follow-up. In addition to mild hydrocephalus, CT-scan of the head documented a reduction in size of the posterior fossa and encroachment of the foramen magnum. A pattern of selective increase in the bone fraction of serum alkaline phos…

The cases of nineteen patients with sternocostoclavicular hyperostosis were reviewed retrospectively. Of particular interest were the responses to antibiotics and prostaglandin inhibitors. The prostaglandin inhibitors relieved the pain within three to four weeks in sixteen of eighteen patients so treated. However, the inhibitors gradually became less effective in most patients. Oral antibiotics were more effective than the inhibitors in relieving the pain of eight of the eleven patients who were given antibiotics. Pustulosis palmaris and plantaris, commonly associated with sternocostoclavicula…

Heterotopic ossification following hip surgery occurred in three patients with ankylosing hyperostosis of the spine. No technical difficulty during surgery was encountered in these individuals. The occurrence of this postoperative complication, coupled with the appearance of bony outgrowths at sites of ligament attachment throughout the axial and extra-axial skeleton in patients with ankylosing hyperostosis of the spine, suggests the presence of an underlying ossifying diathesis, diffuse idiopathic skeletal hyperostosis (DISH). A significant number of patients with DISH possess the second segr…

Diffuse idiopathic skeletal hyperostosis (DISH) is an exceptional finding in athletes but may increase spinal injury risk. We report 2 former elite rugby players who, during amateur play, sustained cervical hyperflexion injuries revealing probable DISH. Case 1: transient paraparesis from cervicothoracic ankylosis and cord compression, treated with posterior decompression. Case 2: transient monoparesis with C1-C2 instability and anterior osteophytes, managed by Magerl fixation. Spinal rigidity from DISH heightens fracture and neural injury risk in collision athletes. Magnetic resonance imaging …
