A comparison of the efficacy and safety of pergolide and bromocriptine in the treatment of hyperprolactinemia.

UNLABELLED: Pergolide is a synthetic ergoline derivative with highly potent long-acting PRL-lowering activity, allowing therapy of hyperprolactinemia with a once daily administration of the drug. The results of two open-label, randomized controlled multicenter clinical trials are reported. Pergolide (taken once a day), was compared with bromocriptine (taken two to four times daily) regarding efficacy and safety in the reduction of PRL levels, the cessation of galactorrhea and amenorrhea, the improvement in sexual function, and tumor shrinkage in hyperprolactinemia without (trial I; 61 patients…

[Evaluation of the treatment of ovarian infertility in women with moderate hyperprolactinemia].

The authors evaluated the therapy by means of clomiphene++ and hCG in a group of 34 menstruating women aged 18-41 who suffered from ovarian infertility and hyperprolactinaemia not exceeding 100 ng/ml. Previous treatment of the women by means of bromoergocryptine decreasing the prolactin concentration did not provoked ovulation. It was found that moderate hyperprolactinaemia still blocked ovulation in one third of the women undergoing therapy. In a group of patients with a concentration of PRL from 50 to 100 ng/ml of blood serum there was a lower percentage of ovulation (62% v 67%) and subseque

Hyperprolactinemia, galactorrhea and amenorrhea in women with a spinal cord injury.

Six women with a traumatic spinal cord injury (SCI) developed hyperprolactinemia, amenorrhea and galactorrhea. Five of them had thoracic level lesions and 1 had a lumbosacral lesion. Two were postpartum and 1 was pregnant at the time of injury. Transient diabetes insipidus developed in 1 patient. Temporary administration of bromocriptine decreased prolactin levels, caused cessation of lactation and restored ovulatory cycles. The syndrome disappeared spontaneously in all 6 patients. Pituitary stalk concussion resulting from the trauma might cause this phenomenon, with the level of the cord inju…

[Serotonin, histamine and catecholamine levels in patients with the hyperprolactinemia syndrome].

A total of 76 patients aged 18-39 with hyperprolactinemia were studied for the blood levels of serotonin and histamine and for the urinary ones of epinephrine, norepinephrine, dopa and dopamine. Blood levels of serotonin and histamine were recorded increased whereas the urinary levels of catecholamines were decreased. No correlation was shown between the intensity in the production of lactogenic hormone and those abnormalities found in the levels of the above mentioned amino groups. The shifts observed were supposed to reflect those general tendencies which took place in the central regulatory…

Hyperprolactinemia is associated with increased immunoreactive somatomedin C in hypopituitarism.

Serum concentrations of immunoreactive somatomedin C were determined in 78 patients with pituitary tumors or craniopharyngiomas. Patients with large tumors, GH deficiency, and normal PRL levels (group 1) had low somatomedin C concentrations (mean, 0.23 U/ml; n = 23). Group 2 included patients with large PRL-secreting pituitary tumors and GH deficiency. This group had serum somatomedin C concentrations in the normal adult range (mean, 1.01 U/ml; n = 20). Patients with hyperprolactinemia and normal pituitary GH secretion (group 3) also had somatomedin C concentrations which were normal (mean, 1.…

[A case of craniopharyngioma with hyperprolactinemia in postoperative course (author's transl)].

Authors report a case of craniopharyngioma with hyperprolactinemia, which was never noticed before surgical operation and showed the amenorrhea-galactorrhea syndrome from 7 months after surgical procedure. The patient is a 44 years old housewife. By the medication of CB-154 (Bromocriptin), the high value of plasma prolactin came to normal range and the galactorrhea disappeared, but the amenorrhea persisted. Because author was able to except the possibility of combine of PRL releasing pituitary adenoma or ectopic malignant tumors from the various examination and laboratory findings, the mechani…

Clinical use of metoclopramide test in the diagnosis of women with hyperprolactinemia.

UNLABELLED: 14 women with elevated prolactin (PRL) serum levels (greater than 25 ng/ml) were given 2.5 mg of metoclopramide, by bolus intravenous injection, to evaluate its diagnosic potential as a stimulus for PRL release. Following metoclopramide injection there was a prompt increase in serum PRL in normal subjects and in patients with moderate PRL elevations associated with galactorrhea-oligomenorrhea. The women with amenorrhea-galactorrhea regardless of the presence of absence of a pituitary tumor, showed a blunted response. Metoclopramide failed to induce TSH secretion in all cases. IN C

[A case of oncocytic adenoma of the pituitary gland with galactorrhea and hyperprolactinemia].

A tumor of hypophysis, with suprasellar expansion, was removed surgically in a 68 years old woman with panhypopituitarism and galactorrhea. Light and electron microscopy studies have shown an oncocytoma. One year after pituitary ablation and cobaltherapy, the patient had normal pituitary functions and galactorrhea had disappeared. Since the prolactin cells were outside the adenoma and the prolactin secretion became normal after surgery, we can assume that galactorrhea was presumably due to the compression of the pituitary stalk by oncocytoma.

Hyperprolactinemia in sarcoidosis: incidence and utility in predicting hypothalamic involvement.

Patients with sarcoidosis have been reported frequently to have elevated concentrations of serum prolactin. On this basis, it was suggested that the hypothalamus might be a common site of involvement by sarcoidosis and that measurement of serum prolactin concentrations might serve as a sensitive indicator of hypothalamic disease. We measured serum prolactin concentrations in a group of 61 patients with sarcoidosis. Hyperprolactinemia was detected in only 2 of the entire group and was not observed in any of the 9 patients with central nervous system involvement. Because radioimmunoassayable pro…

Neuroendocrine Effects of Hyperprolactinemia: Focus on Dopamine and Serotonin Systems.

Hyperprolactinemia is characterized by unusually high levels of prolactin, a hormone primarily known for regulating reproductive functions. However, studies have shown that prolactin has also a significant impact on various neurotransmitter systems in the brain. This review examined the intricate relationship between hyperprolactinemia and neurotransmission, with a particular emphasis on the dopaminergic and serotonergic systems. Prolactin affects these systems in ways that can notably influence mood, behavior, and cognitive functions. One significant consequence of elevated prolactin is the d…

Effect of Hyperprolactinemia on Bone Metabolism: Focusing on Osteopenia/Osteoporosis.

Prolactin is a hormone secreted from lactotroph cells in the anterior pituitary gland to induce lactation after birth. Hyperprolactinemia unrelated to lactation is a common cause of amenorrhea in women of a childbearing age, and a consequent decrease in the gonadotropin-releasing hormone (GnRH) by a high prolactin level can result in decreased bone mineral density. Osteoporosis is a common skeletal disorder characterized by decreased bone mineral density (BMD) and quality, which results in decreased bone strength. In patients with hyperprolactinemia, changes in BMD can be induced indirectly by…

Hyperprolactinemia and Antipsychotic Therapy in Schizophrenic Patients with Hashimoto's Thyroiditis.

INTRODUCTION: Hyperprolactinemia (HPRL) is known as a side effect of some antidepressants and antipsychotics. These medicines are common in treatment of schizophrenia. Thus, HPRL is often observed in schizophrenic patients. It is also known that HPRL can occur in Hashimoto's thyroiditis due to prolactoliberin effect of thyroliberin. The clinical pathophysiology of the patients with the comorbidity of schizophrenia and Hashimoto's thyroiditis, receiving antipsychotics, is of special interest. It's fair to assume that these patients have higher risks of HPRL. To analyze risks of HPRL with antips…

Serial prolactin sampling as a confirmatory test for true hyperprolactinemia.

INTRODUCTION: Hyperprolactinemia may be due to physiological or pathological causes, and may be asymptomatic or induce hypogonadism, infertility, and/or galactorrhea. It is important to take prolactin samples while avoiding stress, as this may increase prolactin levels. Therefore, our aim was to assess the value of prolactin serial sampling after brachial vein cannulation. PATIENTS AND METHODS: Sixty-six patients (34.9±11.8 years of age, 92.4% female) with an initial elevated random prolactin level were included. A prolactin sample was drawn at baseline and after a 30min rest. RESULTS: The m…

Evidence for hyperprolactinemia in migraineurs: a systematic review and meta-analysis.

BACKGROUND: One of the hypothalamus-pituitary axis hormones which may play a crucial role in pathophysiology of migraine is prolactin which is secreted from anterior pituitary gland and synthesized by various immune system cells as well. Whether prolactin blood levels can affect the migraine pathogenesis is an open question. Therefore, investigating prolactin circulatory levels in migraineurs may pave the way to underpin the mechanisms of migraine pathophysiology at biochemical levels. In the current investigation, the prolactin blood levels in the migraine subjects were investigated using sys

Aripiprazole for the treatment of duloxetine-induced hyperprolactinemia: A case report.

BACKGROUND: It is well known that aripiprazole co-treatment effectively reduces antipsychotic-induced hyperprolactinemia. However, the effectiveness of aripiprazole to treat high prolactin levels induced by antidepressant drugs with serotoninergic activity, such as duloxetine, remains unknown. CASE PRESENTATION: An 18-year-old female diagnosed with major depressive disorder (MDD) was treated with 100 mg sertraline once daily. After two weeks, galactorrhoea was observed. Blood biochemical tests revealed an elevated serum prolactin level of 241 ng/mL. Physiological causes and additional potenti…
