The villi of human jejunum vary in size and shape during different functional conditions. In the base the lamina propria is isotonic with blood, in the tip hyperosmotic. Here we study electrophysiological and morphological effects of incubation in hypotonic, isotonic, or hypertonic solutions, and to test various isotonic fixatives for microscopy. Samples of jejunal mucosae, obtained during surgery in obese patients, were studied in Ussing chambers where electrical parameters were registered during incubation in Krebs solution at various osmolarities, and during fixation in formaldehyde, glutar…

In order to determine whether oxytocin release is controlled by an osmoreceptor mechanism identical with that for vasopressin release, the plasma oxytocin concentration and plasma osmolality were measured during intraatrial infusion and after intraventricular injection of various osmotic solutions in unanesthetized rats. Intraatrial infusion of 0.6 M NaCl Locke solution (L.S.) or 1.2 M mannitol L.S. elevated plasma oxytocin significantly, while 1.2 M urea L.S. caused only a small increase and isotonic L.S. did not change in plasma oxytocin. All hypertonic solutions produced significant and sim…

PURPOSE: In this study, the effects of hypertonic saline and ethanol as a single intracystic agents in the percutaneous treatment of liver hydatid cysts were compared.
METHODS: The 50 patients were separated into two groups as those administered 30% hypertonic saline alone as the intracystic agent in percutaneous treatment (33 patients, 52 cysts), and those administered 96% ethanol alone (17 patients, 26 cysts). Both groups were compared in terms of percentage of cyst volume reduction, complications, and treatment success.
RESULTS: The follow-up period was median 17.0 months (11.0-20.0) in t

OBJECTIVE: The beneficial effects of hypertonic saline on neuronal survival and on cerebral blood flow have been shown in several animal models of global and focal brain ischemia. Because of the potential benefits of hypertonic solutions, it is hypothesized that hydroxyethyl starch enhances cerebral blood flow and improves long-term outcome after cardiac arrest and cardiopulmonary resuscitation in an animal model.
DESIGN: Laboratory animal study.
SETTING: University animal research laboratory.
SUBJECTS: Fifty-nine male Sprague-Dawley rats.
INTERVENTIONS: Rats were randomized to receive eit…

Nutritional disturbances of protein metabolism in the liver are reviewed in relation to feeding experimental animals the following diets: a) … induces within hours an increase in hepatic protein synthesis in fasted animals. Administration of hypertonic solutions rapidly (within minutes) inhibits, while administration of hypotonic solutions rapidly increases, hepatic protein systhesis. These experimental findings are reviewed in terms of how alterations in regulatory controls of hepatic protein synthesis may …

Effects of i.p. injection of hypertonic solutions of GABA and mannitol on convulsant activity and latency to reduced or enhanced depending on whether picrotoxin was given i.p. or given i.v. or s.c. The principal cause of these changes appeared to be altered rates of absorption of picrotoxin. There was no evidence that cerebral dehydration afforded protection against picrotoxin.

A mathematical description of blood volume restoration after hemorrhage with resuscitative fluids, particularly hyperosmotic solutions, is presented. It is based on irreversible thermodynamic transport equations and known physiological data. The model shows that after a 20% hemorrhage, the rapid addition of a hypertonic (7.5% NaCl)-hyperoncotic (6% Dextran 70) solution amounting to one-seventh of the shed blood volume reestablishes blood volume within 1 min. Measurements of systemic hematocrit, hemoglobin concentration, and plasma osmolality taken from 13 experiments on anesthetized rabbits ve

The successful evaluation and treatment of the hypertonic patient depends on a clear understanding of the differences between effective and noneffective solute and the consequences of changes in these solutes for water distribution. Measured osmolality, calculated osmolality, tonicity, and sodium concentration must be evaluated in the approach to these patients.

PURPOSE OF REVIEW: To discuss recent updates in fluid management and use of hyperosmolar therapy in neurocritical care.
RECENT FINDINGS: Maintaining euvolemia with crystalloids seems to be the recommended fluid resuscitation for neurocritical care patients. Buffered crystalloids have been shown to reduce hyperchloremia in patients with subarachnoid hemorrhage without causing hyponatremia or hypo-osmolality. In addition, in patients with traumatic brain injury, buffered solutions reduce the incidence of hyperchloremic acidosis but are not associated with intracranial pressure (ICP) alteration.

The concept of small-volume resuscitation, the rapid infusion of a small volume (4 ml/kg BW) of hyperosmolar 7.2-7.5% saline solution for the initial therapy of severe hypovolemia and shock was advocated more than a decade ago. Numerous publications have established that hyperosmolar saline solution can restore arterial blood pressure, cardiac index and oxygen delivery as well as organ perfusion to pre-shock values. Most prehospital studies failed to yield conclusive results with respect to a reduction in overall mortality. A meta-analysis of preclinical studies from North and South America, h

STUDY OBJECTIVE: To elucidate the mechanism of hyperpnea and tachypnea, which are the common findings in cardiopulmonary patients.
RATIONALE: Recently, it was found that activating pulmonary afferents by directly injecting hypertonic saline solution into the lung periphery causes a vagally mediated neural hyperpnea and tachypnea, ie, the excitatory lung reflex. Since reactive oxygen species are released during a variety of pulmonary diseases, we examined whether hydrogen peroxide (H(2)O(2)), a common mediator in cardiopulmonary diseases, can initiate the same excitatory lung reflex.

BACKGROUND: Pseudomonas aeruginosa is the most common Gram-negative pathogen responsible for chronic wound infections, such as diabetic foot infections, and further exacerbates the treatment options and cost of such conditions. Hypertonic glucose, a commonly used prolotherapy solution, can accelerate the proliferation of granulation tissue and improve microcirculation in wounds. However, the action of hypertonic glucose on bacterial pathogens that infect wounds is unclear. In this study, we investigated the inhibitory effects of hypertonic glucose on multidrug-resistant P. aeruginosa strains i

The optimal composition of fluid for volume resuscitation in critically ill patients has been the subject of controversy for decades. Clinicians are faced with several options, including crystalloid solutions of varying tonicity, several colloid preparations (albumin and others), and blood products. Some of these solutions may be differentially distributed between the intra- and extravascular, and intra- and extracellular compartments, accounting for a variety of physiological effects. Two recently published meta-analyses concluded that colloids afford no survival benefit in critically ill pat

The effect of drop infusions of 10% glucose solution for parenteral nutrition, in a dose of 2 g/kg, on bile secretion was studied in experiments on dogs with chronic gallbladder-duodenal fistulas. It was demonstrated that the infusion of hypertonic glucose solution inhibits bile production quantitatively and qualitatively.

We describe the first case of severe hypernatremia associated to laparoscopic surgery for hydatid cyst in an adult patient after the use of hypertonic saline solution with complete resolution. Severe hypernatremia is an unusual fact at the immediate postoperative period but may have fatal consequences for the patient and need immediate action. The patient reached a serum sodium concentration of 179 mmol/L without adverse effects after 6 days of treatment. Laparoscopy could play a crucial role in Na+ absorption due to high intraabdominal pressure caused by the pneumoperitoneum and its limitatio

The objectives of this study, conducted in eight temporal bones from five autopsied cases of subarachnoid hemorrhage, were to delineate the communication between the cerebrospinal fluid space and each space of the epineurium, perineurium, and area surrounding the nerve fibers in the facial nerve and to identify the site of hypertonic diuretic solution infusion most likely to decrease the facial nerve swelling at the initial stage of facial palsy. In all eight specimens, blood cells had directly entered into each space from the labyrinthine segment of the facial nerve. The subarachnoid space ex
