Staphylococcus epidermidis infections.

Staphylococcus epidermidis, an organism routinely found on the skin and in the hospital environment, has become a primary pathogen in infections associated with prosthetic devices. Because these infections are indolent and often clinically silent, diagnosis and therapy are often difficult. Pathogens are often misidentified as contaminants. Their variable, often resistant antibiotic susceptibility pattern and the uncertain correlation of in-vitro beta-lactam sensitivity testing with therapeutic efficacy make selection of an effective antibiotic regimen difficult. Vancomycin combined with rifamp…

[Infections in gynecology].

After the author had reviewed present ideas about infections he outlined the specific character and the role of the normal flora to be found in the cervix and in the vagina, as well as the common characteristics of sexually transmitted diseases. The pathogenic role of bacteria that were previously considered to be "innocent" (type B streptococci, staphylococcus aureus, Gardnerella vaginalis) and Chlamydiae and mycoplasmas make it easier to understand the pathogenesis of basal gynaecological infections and of the urethral syndrome. The author then studies the aetiology of acute salpingitis that…

Disseminated gonococcal infections.

A four-year retrospective review of 55 patients with disseminated infections from Neisseria gonorrhoeae revealed that this complication occurs in young adults, with a predominance in women (80%). The most common manifestation of disseminated gonococcal infection was arthritis, which occurred in 47 patients (85.5%). The second most common manifestation of disseminated gonococcal infection was dermatitis, which occurred in 33 patients (60%). A thorough history, a careful physical examination, and a high index of suspicion are essential for the diagnosis. Disseminated gonococcal infection can be …

Gynecological chlamydial infections.

A review is presented of infections in the female genital tract caused by Chlamydia trachomatis. C. trachomatis primarily infects the cuboidal epithelium of the cervix. Chlamydial cervicitis is four to six time more common in Europe than gonococcal cervicitis. The infection is sexually transmitted and can cause urethritis in the male, endometritis and salpingitis in the female as well as infections in the neonate. The risk of salpingitis in women with chlamydia cervicitis has been estimated to be about 1:10, i.e. the same as for gonorrhoea. Chlamydial cervicitis is asymptomatic in about half o…

Human chlamydial infections.

Chlamydiae are obligate intracellular parasites, bacteria with a peculiar biology. They belong to the genus Chlamydia which includes two species: C. psittaci and C. trachomatis. A wide range of hosts, including birds, mammals and man can be infected by chlamydiae. The diseases chlamydiae can produce include psittacosis, lymphogranuloma venereum, trachoma, inclusion conjunctivitis, urethritis, cervicitis, pelvic inflammatory disease, and neonatal pneumonia. The diagnosis of chlamydial infection may be made by visualization of the organism in direct smears, isolation of the agent in cell culture

Deep neck infections.

From January 1967 to August 1978, 65 patients with cervical abscesses were referred to the ENT Clinic of Turku University Hospital. The origin of these deep neck infections was odontogenic in 19, tonsillitis or tonsillectomy in 14, trauma in seven, salivary glands in five and branchiogenic cysts in five and other known causes in three cases. In 12 cases the origin was unknown. The cervical abscesses of odontogenic origin were located mostly in the submandibular space (11/19). The rest of the deep cervical infections were mostly found in the parapharyngeal space (25/46). Etiological factors and…

Imported fungal infections.

Data on mycoses known to be imported into the United Kingdom are sparse. Estimates on the prevalence of fungal infections have to be based on indirect and incomplete figures, obtained from isolation figures and reports of individual cases to co-ordinating centres such as the Mycological Reference Laboratory and the Communicable Disease Surveillance Centre of the Public Health Laboratory Service. Imported species of dermatophytes account for less than 1% of the total number of isolations made annually at mycological laboratories throughout the U.K. A suggested prevalence of dermatophytosis in t…

Genital mycoplasma infections.

Three different species of mycoplasmas--M. hominis, M. fermentans, and Ureaplasma urealyticum--have been found to infect human genitalia. In the man, mycoplasmas appear to play a role in the etiology of nonspecific urethritis and prostatis. Mycoplasmas repeatedly have been cultured from the tubes of patients with acute salpingitis and a significant antibody response to the organisms during recovery has been reported. Obstetric infections and puerperal sepsis occasionally seem to be caused by mycoplasmas. These organisms also appear to induce spontaneous abortion as well as cause infertility in…
