OBJECTIVE: To explore the optimal therapeutic effect of infliximab (IFX) upon inflammatory bowel disease.
METHODS: Eight patients with inflammatory bowel disease (IBD), including 5 Crohn's disease (CD) and 3 ulcerative colitis (UC), were treated with IFX of 5 mg/kg body weight at 0, 2 and 6 weeks, followed by scheduled maintenance therapy every 8 weeks and followed up for a long-term period. Of these patients, 5 cases were resistant to corticosteroid, 2 cases corticosteroid dependency and one naive patient.

Oral manifestations of Crohn's disease (CD) are not uncommon, but they can be difficult to diagnose and treat. We describe a patient with long-standing CD and a lingual ulcer, which we attributed to CD. The oral lesions were unresponsive to conventional therapy such as steroids, mesalamine, and other topical agents. We report a case of a 40-year-old woman who had received infliximab for perianal Crohn's disease. After six infusions of infliximab, the patient developed staphylococcal pneumonia resulting in fatal adult respiratory distress syndrome. The case is discussed in the context of the toxicity profile of infliximab.

Infliximab is a biologic agent used to treat moderate-to-severe ulcerative colitis by inhibiting the TNF cytokine signaling. Off-target effects of TNF inhibitors include immune suppression and disruption of the tissue repair response. Previous case reports have linked infliximab to several patterns of acute pulmonary toxicity; however, the potential for severe infliximab-induced pneumonitis on long-term respiratory impairment remains largely uncharacterized.
