Artificial Intelligence and Large Language Models in Surgical Shared Decision-Making and Informed Consent: A Narrative Review.

Background: Artificial intelligence (AI) and large language models (LLMs) may make surgical information easier to access and understand. However, these tools cannot replace the clinician's role in shared decision-making or the requirements for valid informed consent. Methods: This narrative review follows the surgical patient pathway from diagnosis and explanation to option comparison, shared decision-making, and informed consent. It examines conventional digital tools, LLM-assisted communication, personalized prediction, direct consent studies, patient and professional perspectives and legal,

Biomedical research in Alzheimer's disease in a Latin American developing country: challenges in the informed consent process.

Alzheimer's disease (AD) is a significant global health issue, impacting 50 million individuals with dementia worldwide, a number projected to triple by 2050. Although research has been conducted for decades, there is no effective prevention, treatment, or early diagnostic method for AD. Research on AD in Latin America (LatAm) faces unique challenges compared to developed countries due to socioeconomic, cultural, and infrastructural factors. While the prevalence of dementia is rapidly increasing in LatAm due to demographic shifts, the region is underrepresented in research, diagnostics and car

Parental Perspectives on an Educational Tool for Values-Informed Biobanking Consent in Pediatric Oncology: A Pilot Project.

Parents of children with cancer describe experiencing gaps in their knowledge and recall after providing biobanking informed consent. Following the Standards for Quality Improvement Reporting Excellence in Education Guidelines, we developed a biobanking educational website. This pilot project assessed parental perspectives on the content that was most valuable and relevant to their decision-making. Surveys and a focus group revealed that the website adequately covered the values-informed topics relevant to consent. Such educational tools have the potential to enhance interactive communication

Informed consent in genetic and genomic studies in Sub-Saharan Africa: a systematic review of bioethical issues.

BACKGROUND: International collaboration on genetic/genomic studies has expanded dramatically. Informed consent remains a cornerstone for protecting participant autonomy. Over the past few decades, guidelines, recommendations, and theoretical discussions have been published on the informed consent process in cross-cultural contexts, particularly in Africa. These resources suggest best practices-ranging from assessing participant comprehension to engaging local communities. However, it remains unclear how well the current practices address the full range of informed consent challenges encountere

Women's Voices in Maternity Care: The Triad of Shared Decision Making, Informed Consent, and Evidence-Based Practices.

The United States is the only industrialized nation that has experienced dramatic increases in the use of maternity interventions resulting in poor birth outcomes. It is speculated that the increased rates of maternal mortality and other outcomes are attributed to the current maternity model of care focused on the overuse of interventions, such as induction of labor, in otherwise healthy pregnant women. The overuse of induction of labor to artificially speed up the birth process has been linked to an increase in preterm and cesarean births. The cost of these interventions and poor outcomes has

[Systematic review of complications for proper informed consent(12)decompression or fixation surgery of the lumbar spine].

A systematic review of the English- and Japanese-language literature related to complications and reoperation rates of spinal surgery for degenerative lumbar disease was undertaken for articles published between 1993 and 2012. From these references, key articles were selected to determine the incidence of clinical perioperative and postoperative adverse events for different types of degenerative lumbar diseases. The mortality rate after lumbar degenerative spinal surgery was 0.20% in the large-scale clinical studies evaluated. In this review series, the complication rates for lumbar canal sten

Informed consent for medical photographs. Dysmorphology Subcommittee of the Clinical Practice Committee, American College of Medical Genetics.

Patient images are used for many purposes in medical practice. They are placed in the medical record as an adjunct to clinical care, displayed to colleagues, students and other audiences in educational settings, and published in medical journals or other media as part of medical research. In each case it is not only prudent, but necessary for the patients’ protection and interest that appropriate consent be obtained. While medical journals invariably require written consent for photographs that may identify the patient, the format of the photograph consent form is usually not specified, nor is

Performance of a large language model in the informed consent process for participation in a clinical trial.

The performance of large language models (LLMs) in the informed consent process for a randomized clinical trial (RCT) remains unknown. The objective of this proof-of-concept study was to evaluate whether a large language model constrained to trial-specific documentation can deliver technically accurate, reproducible responses to informed consent questions with minimal hallucination, as a foundational validation step prior to participant-facing evaluation; and to assess the feasibility of using a second large language model to reproduce document-grounded accuracy assessments in alignment with h

Struggling with extensive informed consent procedures for cancer trials-is there even a benefit for the patients?

PurposeInformed consent procedures in clinical trials often differ in length and complexity to those in clinical routine care. Little is known about the benefit of extensive procedures as intended in clinical trials compared to procedures in routine cancer treatment.MethodsIn two different clinical studies performed at a comprehensive cancer center, we compared patients' comprehension and satisfaction of current informed consent procedures in routine clinical care with the level of comprehension and satisfaction of patients treated within clinical trials. Patients with a new cancer diagnosis a

Large Language Model-Assisted Review of Clinical Trial Informed Consent for Data Reuse: Retrieval-Augmented Generation Approach.

BACKGROUND: Regulatory frameworks such as the Belmont Report, the Common Rule, and the Declaration of Helsinki require informed consent to ensure participants understand a study's purpose and can make voluntary decisions about their involvement. Regulations, including the General Data Protection Regulation (Regulation [EU] 2016/679), further emphasize that consent must be freely given and revocable without disadvantage. Although informed consent forms (ICFs) are intended to be clear and accessible, they have become increasingly lengthy and complex. Large language models (LLMs) offer the potent

Informed Consent in Elective Orthopedic Surgeries: A Scoping Review of Medico-Legal Challenge and Team-Based Application.

INTRODUCTION: Despite established ethical principles, a critical synthesis of how informed consent deficiencies, leading to costly malpractice lawsuits in elective orthopedic surgery, relate to medico-legal outcomes, documentation pitfalls, and team roles is lacking. This scoping review aimed to investigate the status of obtaining informed consent by the treatment team in elective orthopedic surgeries and the related legal consequences. METHODS: Following the Arksey & O'Malley framework update by Levac et al., we conducted a comprehensive search across PubMed, Scopus, Web of Science databases

Informed Consent in Molecular HIV Epidemiology: A Rapid Review of the Literature to Identify Concerns and Recommendations.

In the United States, molecular HIV epidemiology (MHE) is a key part of the National HIV/AIDS Strategy, which incorporates the evaluation of HIV genetic sequences for cluster detection and response. Data collection for MHE occurs without any requirement for formal consent from patients, which has sparked ethical concerns among community members, ethicists, and advocates. We conducted a rapid review of the current literature on informed consent and MHE to understand the key concerns and recommendations. We searched 4 academic databases and included articles that examined the issue of informed c
