Middle cranial fossa trigeminal schwannoma resection through endoscopic transnasal maxillary sinus approach: A case report and literature review.

The endoscopic transnasal maxillary sinus approach is usually performed in resecting tumors located in the pterygopalatine fossa and infratemporal fossa, but is rarely used in the resection of lesions in the middle cranial fossa. Because of the complicated anatomical structure of the middle cranial fossa, trigeminal schwannomas (TSs) located in this region are usually dissected through conventional craniotomy surgical approaches; however, the endoscopic transnasal maxillary sinus approach can be used in resection of middle cranial fossa TSs. The current study presented the case of a 59-year-ol…

Endoscopic sublabial transmaxillary approach to the rostral middle fossa.

OBJECTIVE: The rostral middle fossa faces the temporal pole and is the endocranial anterosuperior aspect of the greater wing of the sphenoid … RESULTS: The approach was divided into three stages: entry into the maxillary sinus, entry into the infratemporal fossa, and entry into the middle fossa. A craniotomy of greater than 20 mm in diameter can be safely created in the rostral middle fossa. When coupled with image guidance, the approach provides the flexibility to tailor the size and location of the middle …

Wegener granulomatosis invading the cavernous sinus: a CT demonstration.

A case of localized cranial Wegener granulomatosis was imaged with high resolution computed tomography. The mass extended from the infratemporal fossa, through the basal foramina, and into the cavernous sinus, causing complete occlusion of the internal carotid artery. Computed tomography demonstrated absence of bone destruction and a normal parotid gland and nasopharynx. Although uncommon, Wegener granulomatosis should be considered in the differential diagnosis of a parapharyngeal mass with intracranial extension.

Facial Nerve Schwannoma Extending to Jugular Foramen: A Case Report.

Facial nerve schwannoma (FNS) is a benign, slow-growing schwannoma that originates from Schwann cells. Facial nerve schwannoma is the most c … it was diagnosed with jugular foramen (JF) space-occupying lesion. We removed the tumor through the infratemporal fossa type A approach and found that the tumor originated from the facial nerve. After the tumor resection, sural nerve transplantation was performed. The patient demonstrated postoperative facial palsy (House-Brackman grade VI) and was smoothly discharge …

Functional anatomy of the mandibular nerve: consequences of nerve injury and entrapment.

Various anatomic structures including bone, muscle, or fibrous bands may entrap and potentially compress branches of the mandibular nerve (MN). The infratemporal fossa is a common location for MN compression and one of the most difficult regions of the skull to access surgically. Other potential sites for entrapment of the MN and its branches include, a totally or partially ossified pterygospinous or pterygoalar ligament, a large lamina of the lateral plate of the pterygoid process, the medial fibers of the lower belly of the lateral pterygoid muscle and the inner fibers of the medial pterygoi…

Epidermal cysts of temporal fossa: an unusual complication of ear surgery.

Otologic procedures have been described as etiologic factors in the development of epidermal cysts in the middle ear as well as in the tympanic membrane. We recently treated two cases of large epidermal cysts of the temporal fossa with extension to the infratemporal space; both cases had histories of ear surgery on the same side as the cysts and therefore the cysts are presumed to have originated from implanted epidermal elements at the time of surgery.

Topography, syntopy and morphology of the human otic ganglion: a cadaver study.

The human otic ganglion (OG) is not readily accessible during ordinary anatomical teaching courses because of insufficient time and severe d … etailed analysis of location, syntopy and morphology was performed. We carried out this study in 21 infratemporal fossae of 18 cadavers and were able to identify the OG, the mandibular-, the inferior alveolar- and the lingual nerve in all of them. We found no significant variation regarding the location of the GO in the infratemporal fossa and its syntopy to the adja …

Principles and Outcomes of Compartmental Resection in Upper Gingivobuccal Cancers.

Gingivobuccal sulcus cancers present unique surgical challenges due to their complex anatomic relationships and propensity for extension into the masticator space and infratemporal fossa. Traditional wide local excision approaches often result in positive margins and local recurrences. This article reviews the principles, techniques, and outcomes of compartmental resection in upper gingivobuccal cancers, with emphasis on anatomic considerations and surgical approaches. Compartmental resection offers superior oncological outcomes compared to conventional approaches, with improved margin clearan…

Mature teratoma arising from the middle cranial fossa.

A 25-year-old man presented with a sudden spurt in size of a bony swelling around the external ear, which was present since birth. This was associated with a painful swelling of the right half of the face and pain, redness, and decreased vision in the right eye. Neurologic examination showed decreased visual acuity in the right eye and paresis of the fifth, seventh, and eighth cranial nerves. Neuroimaging showed an extra-axial, partly cystic tumor of mixed density, located entirely within the middle cranial fossa scalloping the temporal bone and extending into the right infratemporal fossa and…

[Abscess of the skull base--therapy and reconstruction after zygoma resection].

Abscesses of the base of skull, if not correctly managed, can lead to the intracranial spread of infection - a life-threatening complication. Infection can spread from the upper and lower molar teeth through the retromaxillary area or the parapharyngeal space to reach the temporal, infratemporal or pterygopalatine fossa. Our recommended treatment of abscesses in these fossae is wide abscess incision together with resection of the midsection of the zygomatic arch ("Partsch method"). Two case reports are included. The first report is of a patient with cerebral abscess caused by an oral infection

[The use of the temporal muscle for closing postoperative wounds].

The correct use of the musculus temporalis may close rather extensive defects of the base of the skull, yielding good functional and cosmetic results. For plastic purposes, the following types of a graft of the musculus temporalis are applied: 1) a simple interpolated graft; 2) a split-thickness interpolated graft; and 3) a free graft (by crossing the tendon of the musculus temporalis, which allows the muscle to be additionally displaced) for closure of defects after removal of tumors from the socket, maxillary, sphenoidal sinuses, infratemporal fossa, the bases of the middle cranial fossa, fo…

Juvenile Nasopharyngeal Angiofibroma With Skull Base Involvement: A Radiological and Clinical Insight Into a Classic Adolescent Presentation.

Juvenile nasopharyngeal angiofibroma (JNA) represents a rare, histologically benign yet locally aggressive vascular tumor that predominantly … the left sphenopalatine foramen, with extensions into adjacent sinuses, the pterygopalatine fossa, infratemporal fossa, and early skull base erosion. This case highlights the critical diagnostic and staging role of MRI in JNA, illustrating how characteristic imaging findings inform diagnosis, classification, and surgical planning.

Glioblastoma multiforme metastases to the masticator muscles and the scalp.

Glioblastoma multiforme (GBM) is the most common malignant primary brain tumor. Metastases outside the nervous system are a rare phenomenon. We present a pictographically striking case of GBM with metastases in the masticatory muscles of the infratemporal fossa and the scalp, in the context of a dense intracranial dissemination. Extracranial metastases of GBM have been reported anecdotally, while, to our knowledge, masticatory muscle metastases have been previously reported in only one case.

The combined transmastoid retro- and infralabyrinthine transjugular transcondylar transtubercular high cervical approach for resection of glomus jugulare tumors.

Complex tumors OF the glomus jugulare present a surgical challenge because of their difficult location, extreme vascularity, and involvement with multiple cranial nerves. Modern microneurosurgical and cranial base techniques have enabled safe total removal of these complicated tumors. We describe a one-stage transjugular posterior infratemporal fossa approach for radical resection of glomus jugulare tumors located around the jugular foramen, the lower clivus, and the high cervical region from an anterolateral direction. This approach is a combination of transmastoid, suprajugular, transjugular…

Benign triton tumor of the trigeminal nerve.

Benign triton tumor (neuromuscular hamartoma) is a rare tumor of the peripheral nerves, comprising mature neural and striated muscle elements. We report the third case, according to the literature, of a benign triton tumor affecting the Vth cranial nerve in a 4-year-old male child. The tumor was seen to involve the mandibular division of the trigeminal nerve in the infratemporal fossa and extend intracranially by eroding the base of middle cranial fossa. The mass was totally excised. In the tumor, bundles of axons intermingling with fascicles of relatively mature skeletal muscle were found. Th…

Skull base surgery in composite resection.

We extended the limits of standard jaw and neck dissection in large tumors of the oropharynx with extension to the soft and hard palate. Resection of the primary tumor with adequate margins and the lymphatics of the infratemporal fossa is required for tumor control in these extensive lesions. Splitting the lip in the midline and developing a large cervicofacial flap gives an excellent exposure to the region. The resection includes the hemimandible and the soft tissues of the intratemporal fossa. The internal carotid artery is followed to the skull base and all structures medial to this vessel,…

Analysis of Imaging Findings in a Patient with Squamous Cell Carcinoma of the External Auditory Canal Metastatic to the Dura with Trigeminal Nerve Involvement.

We report the case of a 56-year-old female recently diagnosed with well-differentiated squamous cell carcinoma of the external auditory canal. The patient underwent an 18F-fluorodeoxyglucose positron emission tomography/computed tomography for staging assessment. This examination revealed intense uptake in the right ear canals, tympanic cavity, eustachian canal, parapharyngeal area, and infratemporal fossa. Notably, we identified intracranial dural metastasis, which represents an uncommon site for metastatic spread in general.
