A Venue-Based Survey of Malaria, Anemia and Mobility Patterns among Migrant Farm Workers in Amhara Region, Ethiopia.

BACKGROUND: Mobile populations present unique challenges to malaria control and elimination efforts. Each year, a large number of individuals travel to northwest Amhara Region, Ethiopia to seek seasonal employment on large-scale farms. Agricultural areas typically report the heaviest malaria burden within Amhara thereby placing migrants at high risk of infection. Yet little is known about these seasonal migrants and their malaria-related risk factors. METHODS AND FINDINGS: In July 2013, a venue-based survey of 605 migrant laborers 18 years or older was conducted in two districts of North Gond

Leishmaniasis and its relevance to UK Armed Forces.

Leishmaniasis is an infectious disease caused by Leishmania protozoa, transmitted by the bite of phlebotomine sandflies. It causes a spectrum of clinical syndromes, of which the most common are cutaneous and visceral leishmaniasis. Clinical presentation is highly variable and is dependent on multiple factors, such as Leishmania species and patient characteristics (including immune competence). The relationship between these variables is poorly understood, and there is no single, evidence-based treatment for the disease. Currently management focuses on identification of the species, but this re

Early-Evening Biting by Anopheles stephensi in Southern Ethiopia: A Challenge for Bed Net Use as Vector Control in Africa.

Anopheles stephensi is rapidly expanding across Africa, posing new challenges for malaria control. Its biting time patterns, however, remain poorly characterized, raising uncertainty about the effectiveness of bed nets against this invasive vector. To address this gap, we investigated diel biting activity, feeding propensity, and flight behavior using complementary behavioral assays on females reared from wild-caught larvae in Hawassa City, Ethiopia. Flight monitoring assays revealed that An. stephensi exhibited pronounced activity at dusk, beginning between 18:00 and 19:00 with scotophase ons

Do women attending antenatal clinics who use only intermittent preventive treatment (IPTp) have higher Plasmodium falciparum prevalence compared to those who used both IPTp and slept under insecticide-treated nets?

BACKGROUND: As global malaria incidence continues to increase across sub-Saharan Africa, tightening usage of existing prevention strategies is crucial for protection of vulnerable populations. Nigeria alone accounted for more than a quarter of the 247 million cases in 2021. During pregnancy, Intermittent Preventive Treatment (IPTp) and use of Insecticide-treated nets (ITNs) are recommended for malaria prevention. METHODS: This study sought to determine whether the combined use of IPTp and ITNs offered better protection from Plasmodium falciparum infection compared to a single strategy alone.

Leveraging machine learning to predict mosquito bed net utilization among women of reproductive age in sub-Saharan Africa.

BACKGROUND: Malaria remains a major public health challenge, particularly in sub-Saharan Africa, where women of reproductive age are especially vulnerable during pregnancy and childbirth. To identify key predictors and improve predictive accuracy, machine learning algorithms such as Random Forest were applied, along with SHAP analysis, to a large multi-country DHS dataset, with class imbalance addressed using Tomek Links and Random Over-Sampling. METHODS: This study employed a weighted dataset of 153,015 participants from the Demographic and Health Survey (DHS) conducted across ten sub-Sahara

Relationship between malaria vector survival, infectivity, and insecticide-treated net use in western Kenya.

BACKGROUND: Significant effort and resources have been invested to control malaria transmission in sub-Saharan Africa, but it remains a major public health problem. For the parasite to be transmitted, the female Anopheles vector must survive 10-14 days following an infective bite to allow Plasmodium gametocytes to develop into infectious sporozoites. The goal of this study was to assess factors associated with wild-caught Anopheles survival and infection following host-seeking and indoor resting. METHODS: The study was conducted between January 2020 to March 2022 in a longitudinal cohort of 7

Mosquito Bed Net Use and Burkitt Lymphoma Incidence in Sub-Saharan Africa: A Systematic Review and Meta-Analysis.

IMPORTANCE: Burkitt lymphoma (BL) is one of the most common childhood cancers in sub-Saharan Africa and is etiologically linked to malaria. However, evidence for an effect of malaria interventions on BL is limited. OBJECTIVE: To investigate the potential population-level association between large-scale rollout of insecticide-treated bed nets (ITNs) in sub-Saharan Africa in the 2000s and BL incidence. DATA SOURCES: In this systematic review and meta-analysis, a search was conducted in the Embase, Global Health, and Medline databases and in cancer registry publications between January 1, 1990,

Agent-based modelling of complex factors impacting malaria prevalence.

BACKGROUND: Increasingly complex models have been developed to characterize the transmission dynamics of malaria. The multiplicity of malaria transmission factors calls for a realistic modelling approach that incorporates various complex factors such as the effect of control measures, behavioural impacts of the parasites to the vector, or socio-economic variables. Indeed, the crucial impact of household size in eliminating malaria has been emphasized in previous studies. However, increasing complexity also increases the difficulty of calibrating model parameters. Moreover, despite the availabi

'For the poor, sleep is leisure': understanding perceptions, barriers and motivators to mosquito net care and repair in southern Tanzania.

BACKGROUND: The rate of physical deterioration of long-lasting insecticidal nets (LLINs) varies by household practices, net brand and environment. One way to sustain the protection provided by LLINs against malaria is through day-to-day care, and repairing holes as and when they occur. To ensure LLIN coverage is high between mass campaigns and, as international donor funds decrease, personal responsibility to maintain nets in good condition is becoming more important. This study aimed to understand local barriers and motivators to net care and repair in southern Tanzania in a community that re

Efficacy of non-pyrethroid indoor residual spraying or intensive behaviour change communication in combination with long-lasting insecticidal nets for malaria control in west Africa: a pragmatic, cluster-randomised, controlled trial.

BACKGROUND: Since 2015, progress in the control of malaria has stalled owing to multiple factors, including the probable reduced efficacy of long-lasting insecticidal nets (LLINs) caused by insecticide resistance and plateauing LLIN use rates. This study aimed to assess the additional effect of non-pyrethroid indoor residual spraying (IRS) and intensive behaviour change communication (BCC) when combined with LLINs on malaria in rural west Africa. METHODS: This pragmatic, parallel-group, cluster-randomised, controlled trial took place in community settings in Burkina Faso and Côte d'Ivoire. Vi

Can malaria vector control accelerate the interruption of lymphatic filariasis transmission in Africa; capturing a window of opportunity?

BACKGROUND: The Global Programme to Eliminate Lymphatic Filariasis (GPELF) was launched in 2000, and nearly all endemic countries in the Americas, Eastern Mediterranean and Asia-Pacific regions have now initiated the WHO recommended mass drug administration (MDA) campaign to interrupt transmission of the parasite. However, nearly 50% of the LF endemic countries in Africa are yet to implement the GPELF MDA strategy, which does not include vector control. Nevertheless, the recent scale up in insecticide treated /long lasting nets (ITNs/LLINs) and indoor residual spraying (IRS) for malaria contro

Variation in attractiveness of human subjects to malaria mosquitoes (Diptera: Culicidae) in The Gambia.

During experimental hut trials to assess the efficacy of insecticide-treated bednets against malaria mosquitoes, we observed that human subjects varied consistently in their attractiveness to mosquitoes. Attractiveness was assessed by estimating the numbers of wild Anopheles gambiae Giles mosquitoes entering a hut in which a man was sleeping, and the numbers of human-bloodfed An. gambiae sensu lato collected from each hut each morning. Five trials were carried out at Wali Kunda in rural Gambia during 2.5 yr. During each 6-wk trial a man slept under a bednet in each of the six huts. Morning col…

A low-cost house eave screening technique complements insecticide-treated nets in reducing indoor human exposure to the bites of anopheles mosquitoes in a malaria endemic area of south forested Cameroon: results from a pilot study.

Currents insecticide-based vector control strategies are struggling to reduce malaria transmission. This pilot study investigated the potential of a low-cost eave screening technique to reduce human exposure to malaria vectors in a forested area of Cameroon. The pre-intervention phase consisted of collecting baseline data on malaria epidemiology including the human malaria prevalence, vector biology and the bio-efficacy of insecticide-treated nets (ITNs). During the intervention phase, selected houses were improved by screening open eaves with wire insect mesh stapled to wood lathe strips that

LLIN Evaluation in Uganda Project (LLINEUP)-effects of a vector control trial on Plasmodium infection prevalence and genotypic markers of insecticide resistance in Anopheles vectors from 48 districts of Uganda.

Pyrethroid bednets treated with the synergist piperonyl butoxide (PBO) offer the possibility of improved vector control in mosquito populations with metabolic resistance. In 2017-2019, we conducted a large-scale, cluster-randomised trial (LLINEUP) to evaluate long-lasting insecticidal nets (LLINs) treated with a pyrethroid insecticide plus PBO (PBO LLINs), as compared to conventional, pyrethroid-only LLINs across 104 health sub-districts (HSDs) in Uganda. In LLINEUP, and similar trials in Tanzania, PBO LLINs were found to provide greater protection against malaria than conventional LLINs, redu

LLIN Evaluation in Uganda Project (LLINEUP): a cross-sectional survey of species diversity and insecticide resistance in 48 districts of Uganda.

BACKGROUND: Long-lasting insecticidal nets (LLINs) are the principal tool for malaria control in Africa and are presently treated with a single class of insecticide; however, increasing levels of insecticide resistance threaten their success. In response to this threat nets have been developed that incorporate the synergist, piperonyl butoxide (PBO), which inhibits the activity of cytochrome P450s which is one main mechanisms of insecticide resistance, allowing resistance to pyrethroids to be reversed. However, data on the value and cost effectiveness of these nets is lacking. A large-scale cl
