Treatment priorities for septic shock.

Septic shock is an emergency that is best treated by establishing priorities beforehand. The first priority is early recognition. The earliest recognizable clinical presentation is fever and hyperventilation. The second priority is augmenting normal compensatory mechanisms by intravenous infusion of crystalloid, with measurement of the response so that vasoactive drugs can be instituted as needed. The third priority is selected antibiotic therapy and drainage of pus. The fourth is corticosteroid therapy if the patient's response has been suboptimal.

Prognostic factors to predict outcome following the administration of hypertonic/hyperoncotic solution in hypovolemic patients.

Hypertonic solutions effectively improve hemodynamic parameters in patients admitted to the emergency room. However, no significant differences in outcome were observed compared with standard isotonic treatment in most previously published studies. This study evaluates pretreatment prognostic factors that predict a beneficial effect of hypertonic solution in patients admitted to the emergency room with hemorrhagic hypovolemia in a prospective double-blind fashion. The patients (n = 212) were randomized upon admission to receive 250 mL intravenous (i.v.) bolus of hypertonic 7.5% NaCl + 6% dextr

Effects of salso-bromo-iodine thermal water in children suffering from otitis media with effusion: a randomized controlled pilot study.

BACKGROUND: Otitis media with effusion (OME) is an ear disorder defined by the presence of fluid in the middle ear without signs or symptoms of acute infection. The aim of this randomised and controlled pilot study was to evaluate whether the treatment with a watery salsobromo- iodine solution, administered by nasal douche, could induce ear healing better than isotonic saline in children with OME. METHODS: The study was randomized, single-blind, and controlled. Study group (40 children) was treated with salso-bromo-iodine thermal water solution and Control group (40 children) was treated with

[Gastric emptying of different liquid test meals measured by ultrasonography].

The rate of gastric emptying of different liquid solutions was measured in 80 voluntary normal young male subjects (mean age 19.9 +/- 1.6 years) by direct real-time ultrasonic imaging. Multiple US scans were repeated at regular intervals (15-30 minutes) until complete gastric emptying was established, after ingestion of 500 cc of 8 different liquid test meals: isotonic saline, 50% glucose, 5% glucose, 40 g of proteins, 20 g of protein, 40 g of fat, 20 g of fat and mixed (carbohydrates, proteins and fat). Eight groups of ten subjects were studied, each one with a different test meal. Three type

Effect of hypertonicity on augmentation and potentiation and on corresponding quantal parameters of transmitter release.

Augmentation and (posttetanic) potentiation are two of the four components comprising the enhanced release of transmitter following repetitive nerve stimulation. To examine the quantal basis of these components under isotonic and hypertonic conditions, we recorded miniature endplate potentials (MEPPs) from isolated frog (Rana pipiens) cutaneous pectoris muscles, before and after repetitive nerve stimulation (40 s at 80 Hz). Continuous recordings were made in low Ca2+ high Mg2+ isotonic Ringer solution, in Ringer that was made hypertonic with 100 mM sucrose, and in wash solution. Estimates were

Fluid Therapy in the Critically Ill Patient.

Critically ill patients may be at higher risk for fluid overload and the development of interstitial edema due to damage to the endothelial glycocalyx, myocardial dysfunction, vasoplegia, renal dysfunction, increased vascular permeability, and hypoalbuminemia. While isotonic crystalloids are commonly used for resuscitation of hypovolemic shock, their use should be limited in critically ill patients to avoid complications, including the development of interstitial edema. Blood products should be considered for resuscitation of hemorrhagic shock to limit the volume of crystalloids required. To l

Lactated Ringer's or Normal Saline for Initial Resuscitation in Patients Hospitalized With Acute Pancreatitis: A Retrospective Database Analysis.

OBJECTIVES: Fluid resuscitation is required in acute pancreatitis (AP) to prevent hypovolemia and organ hypoperfusion. Lactated Ringer's (LR) is a buffered crystalloid with possible advantages in AP versus normal saline (NS). We aim to assess outcomes in patients hospitalized with AP based on fluid used for resuscitation. METHODS: In this retrospective analysis, we identified hospital admissions to Veterans Affairs facilities for AP from 2011 to 2017 and grouped by initial resuscitation fluid: LR versus NS. Outcomes included major complications and mortality at 30 and 365 days. Multivariable

Effect of Intravenously Administered Crystalloid Solutions on Acid-Base Balance in Domestic Animals.

Intravenous fluid therapy can alter plasma acid-base balance. The Stewart approach to acid-base balance is uniquely suited to identify and quantify the effects of the cationic and anionic constituents of crystalloid solutions on plasma pH. The plasma strong ion difference (SID) and weak acid concentrations are similar to those of the administered fluid, more so at higher administration rates and with larger volumes. A crystalloid's in vivo effects on plasma pH are described by 3 general rules: SID > [HCO3-] increases plasma pH (alkalosis); SID < [HCO3-] decreases plasma pH (alkalosis); and SID

Preferential effects of low volume versus high volume replacement with crystalloid fluid in a hemorrhagic shock model in pigs.

BACKGROUND: Fluid resuscitation is a core stone of hemorrhagic shock therapy, and crystalloid fluids seem to be associated with lower mortality compared to colloids. However, as redistribution starts within minutes, it has been suggested to replace blood loss with a minimum of a three-fold amount of crystalloids. The hypothesis was that in comparison to high volume (HV), a lower crystalloid volume (LV) achieves a favorable coagulation profile and exerts sufficient haemodynamics in the acute phase of resuscitation. METHODS: In 24 anaesthetized pigs, controlled arterial blood loss of 50 % of th

Should chloride-rich crystalloids remain the mainstay of fluid resuscitation to prevent 'pre-renal' acute kidney injury?: con.

The high chloride content of 0.9% saline leads to adverse pathophysiological effects in both animals and healthy human volunteers, changes not seen after balanced crystalloids. Small randomized trials confirm that the hyperchloremic acidosis induced by saline also occurs in patients, but no clinical outcome benefit was demonstrable when compared with balanced crystalloids, perhaps due to a type II error. A strong signal is emerging from recent large propensity-matched and cohort studies for the adverse effects that 0.9% saline has on the clinical outcome in surgical and critically ill patients

Use of synthetic colloids in sepsis: a critical review on efficacy, safety and patient benefits.

In this narrative review, the studies and analyses are discussed that pertain to benefits and detriments of synthetic colloids versus natural colloids or crystalloids used for fluid resuscitation in sepsis and septic shock. The relative amount of fluid infusions used to reach clinical or hemodynamic end-points are reviewed, as well as potential toxicity of starch solutions on the kidney. Hence, it cannot be excluded that adverse effects partly offset beneficial hemodynamic effects that are similar to that of natural colloids, so that in most analyses a mortality benefit of synthetic colloid fl

Choice of replacement solution and anticoagulant in continuous venovenous hemofiltration.

BACKGROUND: Several types of replacement fluid and methods of anticoagulation have been employed for continuous renal replacement therapy, but there is no consensus on a preferred approach. We evaluated the indications for the selection of replacement fluid and anticoagulant among critically ill patients receiving continuous venovenous hemofiltration (CVVH) and assessed the effect of the selection on the efficacy of anticoagulation and complications. METHODS: We retrospectively studied 29 consecutive patients who received CVVH in the Medical Intensive Care Unit at Massachusetts General Hospit

Preservation of microelastic properties of dentin and tooth enamel in vitro--a scanning acoustic microscopy study.

OBJECTIVES: The aim of this study was to examine the influence of storage media on the elastic properties of dentin and tooth enamel with respect to the storage period. Several reports suggest that saline and other aqueous solutions may induce chemical reactions and the dissolution of minerals, which in turn may cause alterations of elastic tissue properties. METHODS: Three non-erupted human wisdom teeth were extracted and divided in three slices. Sections were stored in three different media for a maximum period of 21 days. During this time all sections were inspected by time-resolved 50-MHz

Crystalloids and colloids in trauma resuscitation: a brief overview of the current debate.

BACKGROUND: Controversy regarding crystalloids or colloids for resuscitation has existed for over five decades, and large numbers of clinical trials have failed to resolve the controversy. In fact, the limitations of these studies have intensified the debate. This overview aims to revisit the debate of fluid resuscitation in trauma patients by critically appraising the meta-analyses on the subject. METHODS: This study was a critical analysis of six meta-analyses found by MEDLINE search. RESULTS: Overall, the choice of fluid may have a small or no effect on mortality. In trauma, the use of co

Ganglionic and arterial release of neuropeptide Y by bullfrog sympathetic neurons.

Sympathetic C neurons in lumbar paravertebral ganglia of the bullfrog have previously been shown to be vasomotor in function and to express neuropeptide Y (NPY). In the present experiments, a sensitive radioimmunoassay was used to measure the NPY content of ganglia and the descending abdominal aorta and to measure the overflow of NPY evoked by depolarizing concentrations of K+. Paravertebral ganglia 9 and 10 contain 3.1 pg NPY/micrograms protein and the aorta contains 0.18 pg NPY/micrograms protein. During 20-min depolarizations in high K+ (58 mM) Ringer, the ganglia released approximately 5%
